2 2/9 1.0 PURPOSE This specification outlines the minimum requirements for the installation, identification and certification of permanent lifting devices (LD) and lifting attachments for use at Vale s operations. Recommended hoist and trolley inspection and testing are also outlined. 2.0 REFERENCE DOCUMENTATION The following documents were used in the development of this document or have instructions and procedures applicable to it. They shall be used in their most recent revision. SPEC SPEC SPEC O.Reg. 854 Structural Steel - Quality Requirements Paints - General Requirements Cranes OHSA Regulation for Mines and Mining Plants The following drawings form part of this specification: E Underground Monorail Supports Standard Details E Lifting Device Design Details and Notes.0 GENERAL REQUIREMENTS 4.1 DEVICES All lifting devices shall be designed by a Professional Engineer and installed in compliance with the Occupational Health and Safety Act, Regulation for Mines and Mining Plants. The Engineer (or his/her designate) must visit the site to verify the integrity of the various components of the lifting device and to scrutinize the support system and elements. Where lifting devices are attached to rock faces in mines, a Rock Mechanics Engineer is required to substantiate the integrity of the supports. Each lifting device is assigned a serial number by Vale Central Engineering and entered into the lifting device registry. Design - Upon completion of the lifting device design package, it is issued to the plant through the Plant Engineering Coordinator (PEC). This is applicable for the installation of a new lifting device or repairs to an existing lifting device.

3 /9 Installation - Upon completion of the lifting device installation, a competent person is required to certify that the identification, fabrication and erection, including the supporting structure, were performed in accordance with the design documents. This certification does not include the trolley or the hoist which are considered to be proprietary. The trolley and hoist must be suitable for the application and be in good working order. All trolley and hoist components that are installed on the lifting device shall be inspected and tested prior to use. Each component that may affect the safe operation of a lifting device shall be examined and tested by a competent person before initial use, and thereafter at intervals not exceeding one year CRAWL BEAMS SPECIAL REQUIREMENTS Crawl beam type cranes (using trolleys between bridge beam and runway beams) are NO LONGER used for new installations. Reference details on drawing E , are design guides for certification of existing crawl beam devices. For all new installations, use single girder crane c/w rigid end trucks, see SPEC ATTACHMENTS The original design brief shall be kept in the Central Engineering Non-regulated Attachments File. Lifting Device Forms E 148 and E 149 ARE NOT to be used for non-regulated attachments. Copies of any letters or memos shall be kept in the Non-regulated Attachment File. 4.0 SERIAL NUMBER AND REGISTRY Vale Central Engineering has a designated lifting device coordinator who is responsible for assigning LD serial numbers, maintaining the LD registry and files, and providing plastic LD identification tags. When a lifting device has been identified for design check and recording, it is assigned a serial number and is entered in the register for the appropriate plant and building. The registry also tracks the presence of the design documentation. Plastic LD identification tags are provided by the LD Coordinator to the designer in accordance with drawing E

4 4/9 5.0 DESIGN ADMINISTRATION The designer requests a LD Serial Number from the LD Coordinator using the LD-1 form. Upon completion of the design, forms E-148, E-149, and the plastic LD identification tags are issued to the Plant Engineering Coordinator (PEC) for issue to the Maintenance Superintendent of the facility or when part of a project, to the Project Manager. If the lifting device or the support system requires structural revisions to upgrade the device to the desired capacity, the necessary engineering and detail drawings or sketches shall be included with the package. A copy of the design calculations and the design package are filed in the lifting device files. The Maintenance Superintendent or the Project Manager is responsible for arranging for implementation of the design documents, however, the PEC must follow up to ensure implementation. 6.0 DEVICE IDENTIFICATION 7.1 GENERAL Lifting Devices shall be identified as follows: Lifting devices which include crawl beams, runway beams, monorails and jib cranes shall receive a final coat of paint conforming to Federal Standard Colour #1591 (OSHA Safety Yellow) The maximum rated capacity XX Ton shall be painted in 6 high (minimum) black letters on each side of the web of the lifting device, crane beam, monorail or jib boom. Identification tags are to be permanently installed on all lifting devices. 7.2 IDENTIFICATION TAGS Tag(s) bearing the lifting device identification number xx-xxx-xxx shall be attached to the web of the lifting device beam.

5 5/9 The tags are made from 2" wide X 10" long red plastic with 1" high white stencilled letters. Provide 1/8" diameter holes at each end for fastening screws. Attach the tags(s) to one side of the lifting device beam web adjacent to the rated capacity painted on the lifting device with #10 S.S. 04 self-tapping screws (or a suitable rust resistant fastener), one at each end. Refer to drawing E for identification tag layout and details. 7. ATTACHMENTS Each non-regulated attachment shall have the maximum rated capacity permanently marked on the item. Methods of marking may vary (tag, paint, stamp, weld, etc.) to suit the size of the attachment. For ready identification, each attachment shall be assigned a serial number. The serial number has three parts: the plant number, the building or area code number, and a sequential number. Example: The A in the attachment sequential number is used to differentiate the number from those numbers used for "lifting devices". This attachment serial number will be assigned by the Lifting Device Coordinator who administers the master register. 7.0 DEVICE CERTIFICATION When a new lifting device is installed or an existing lifting device is repaired, painted and identified, form E-149 is signed as Inspected and Certified and returned via the PEC to the Central Engineering LD Coordinator for filing. Inspection and Certification includes the following: Insure that: The lifting device is painted and identified per Section 7 All supports have been fabricated and erected in accordance with the design drawings, scope and specifications. Span lengths do not exceed those indicated on the design drawings. The structural steelwork has been fabricated in accordance with CAN/CSA-S16

6 6/9 All welding has been performed in accordance with CSA W59 Lifting device section sizes, configuration and connection details have been fabricated and erected in accordance with the design drawings. For underground installations, ensure that a Rock Mechanics Engineer has reviewed the integrity of the rock, rock bolts and supports. 8.0 HOIST AND TROLLEY INSPECTION AND TESTING When a new trolley and hoist are installed on a lifting device, the trolley and hoist shall be inspected prior to initial use, and thereafter at intervals not exceeding one year. The dates, findings and names of the competent persons performing these examinations shall be recorded and the records shall be kept available at the plant or mine for inspection. The person who performs a hoist and trolley inspection must, as a minimum: Ensure the hoist and trolley capacities are clearly identified and match the rated capacity of the lifting device. Ensure that the supporting lifting device has been designed by a Professional Engineer and that form E-149 has been completed and signed. Ensure that the trolley has been installed properly on the lifting device and done so in accordance with the manufacturer s instructions and specifications. Ensure that all operational functions are in place. Ensure all lift hooks are equipped with latches if required and hooks are not deformed, worn or damaged. Ensure hoist ropes or chains, sheaves and hook block are not damaged or worn. Ensure hoist ropes or chains are suitable for the rated capacity of the lifting device. Ensure that all functional labels for operator control are in place. Prior to initial use, operational and running tests shall be performed for all new, reinstalled, modified or rebuilt equipment. These operational and running tests shall include: Test of all hoist/trolley motions including hoisting and travelling. Physical testing of limiting and indicating devices. Testing of all circuits, controls, interlocks and sequence of operations.

7 7/9 Physical test of hoisting and travelling motions with the trolley/hoist carrying a load equal to the rated capacity of the lifting device. All specified speeds must be obtained during this test. Physical load test of hoisting and travelling motions with the trolley/hoist carrying 125% of the rated load. Specified speeds need not be obtained, but the hoist/trolley must show itself capable of handling the load without difficulty. 9.0 REMOVING A DEVICE FROM SERVICE Any lifting device that is removed from service shall have its end stops removed and the device painted grey as per appropriate Vale paint specification.
