2 INTENT OF REQUEST FOR BID The intent of this request for bid is to solicit competitive bids from qualified contractors to furnish and install a single ply roofing membrane per the accompanying documents. REQUEST FOR BID ADDENDA In the event that modifications, clarifications or additions to the request for bid become necessary, all vendors will be notified and receive written addenda. ACCEPTANCE OF BIDS San Juan School District reserves the right to accept or reject any or all bids. VENDOR SELECTION Vendor selection will be based on qualified lowest bidder. NOTIFICATION OF VENDOR SELECTION Vendors will be notified of bid results. Bids will remain on file at the Buildings and Grounds Office for review by interested parties for seven days after award of contract. BID PREPARATION COSTS All costs incurred in the preparation and presentation of the bid shall be wholly absorbed by the vendor. All supporting documentation and manuals submitted with this bid will become the property of San Juan School District unless requested otherwise by the vendor at the time of submission. SITE VISIT There will be a mandatory bid walk. All bidders must visit the site to familiarize themselves with the scope of work based on the bid documents. No exceptions will be given to vendors that do not familiarize themselves with the scope of work. On Tuesday, April 10 at 10:00 A.M. there will be a scheduled walk at ARL, 535 North, 100 East, Blanding, Utah.

3 SCOPE OF WORK a. Remove existing single ply membrane and install new membrane on approximately 62,000 sq ft of roof b. Replace any saturated or damaged roof structure or insulation as needed c. Replace or eliminate cap metal as identified at walk-through d. Owner will remove and replace all swamp coolers e. All parapet walls and HVAC curbs to be wrapped Vendor is responsible to verify all measurements and existing conditions prior to bid ROOFING SPECIFICATIONS a. Membrane to be 80 Mil PVC membrane, color: white, mechanically fastened b. Underlayment as required by manufacturer c. Fasteners and termination bars as required by manufacturer d. Cladded drip edges as required by manufacturer; color to be approved by owner e. Sealant: polyurethane, elastomeric sealant/adhesive f. 4 SPECIFICATIONS DOCUMENT Specifications for roofing work for San Juan School District (Hereafter referred to as SJSD) Buildings and Grounds Department. Performance Requirements a. Provide an installed roofing membrane and base flashing system that does not permit the passage of water, and will withstand the design pressures calculated in accordance with the most current revision of ASCE 7. Submittals a. Specify manufacturer and type of mechanically fastened membrane to be installed. b. Bid Bond (5%) of proposal total. c. Schedule Quality Assurance a. Installer's Qualifications: Installer must be certified by primary roofing manufacturer. b. Manufacturer's representative shall provide a comprehensive final inspection after completion of the roof system with SJSD representative. c. Contractor must be prepared, willing and accommodating to random on-site inspections of all work, at any time, by SJSD's representative during job progress. Insurance/Liability/Safety/Security a. The contractor must secure and maintain liability insurance (General and Auto) of an adequate amount, against claims for injuries to persons or damages to property which may arise from or in connection with the performance of the work hereunder. Insurance must be maintained for the duration of the project. Contractor must provide proof of insurance upon demand by SJSD (endorsement or policy) and agrees to, at all times, protect, indemnify, defend and hold harmless SJSD and SJSD employees from any and all claims. b. SJSD will assume no liabilities for the contractor including any of the contractor's representatives, employees or properties. c. Contractor shall, at all times, keep the work area in a neat, clean, safe and secure condition to avoid the risk of loss, theft, vandalism or sabotage. d. Any damages occurring during the completion of this project, incidental or otherwise, to SJSD or adjacent properties must be repaired at the contractor's expense and to the owner's satisfaction.

5 Notice to Proceed a. Notice to proceed will be in the form of a Purchase Order or Letter of Acceptance from SJSD Buildings and Grounds Department. Do not proceed without a Purchase Order or Letter of Acceptance. b. Funding for projects will be available July 1st. Installer may start prior to that date. Payment Bonds a. Payments will be made as work is completed, and after SJSD representative approves of the work. Ten percent (10%) of the total project costs will be retained until project completion, clean-up, inspection, warranty and approval is made by district representative. a. Vendors must provide a performance and payment bond prior to the commencement of the work. Claimant will be listed as San Juan School District. Pre-Installation Conference a. Prior to scheduled commencement of the roofing installation and associated work, a meeting at the project site with the installer, SJSD's representative, roofing manufacturer representative and any other persons directly involved with the performance of the work will be held. Purpose of meeting is to review foreseeable methods and procedures related to roofing work. Regulatory Requirements a. All work shall be performed in a safe, professional manner, conforming to all federal, state and local codes. Delivery, Storage, and Handling a. Follow manufacturer's written guidelines. Warranty a. Special Warranty: Manufacturer's standard form, 15 year, without monetary limitations, in which manufacturer agrees to repair components of membrane roofing system that fail in materials or workmanship within specified warranty period. Failure includes roof leaks. I. Special warranty includes roofing membrane, base flashing, roofing membrane accessories, fasteners, underlayments and other components of membrane roofing system. II. Manufacturer's warranty must cover wind gusts up to 60 mph. Schedule a. Date of completion: August 7, 2018

6 Execution a. Swamp coolers to be removed and replaced by SJSD. b. Securing necessary equipment, unloading and placement of materials to be coordinated by installer. c. Deposit, removal and disposal of trash bins to be coordinated by installer. d. 110 power may be utilized by installer where available. Any other power needs to be provided by installer. e. Remove existing single ply membrane at specified areas within scope of project. Mechanically Fastened Roofing Membrane Installation a. Install roofing membrane according to roofing system manufacturer's written instructions and specifications. Base Flashing a. All flashing in relation to roof penetrations, expansion joints, parapets and building walls will be installed according to manufacturer's standard details. b. Appropriate flashing details will be determined at walk-through. c. All roof top equipment must be removed to allow membrane to wrap top of curb. Flashing and Counter flashing Materials a. Sheet Metal I. Iron or steel with baked on enamel finish, color to match existing. i. 22 gauge for hold down clips. ii. 24 gauge for all others.
