NESHAPs ACM Report - Property E April 30, 2013 ASBESTOS INSPECTION REPORT Field inspections were performed on April 8 & 23, 2013 at the subject property located at th Street in Rock Island, IL to determine the presence of asbestos containing materials (ACM). Suspect materials identified during the inspection were sampled and analyzed in accordance with federal and state NESHAPs requirements. This report itemizes materials tested and provides laboratory results of those materials. The approximate square footage / linear footage of the materials testing positive for ACM are provided together with the approximate location, where appropriate. Suspect bulk ACM samples, both friable and non-friable, were analyzed using polarized light microscopy to determine if the suspect ACMs contain more the 1% asbestos. Attachments include laboratory analytical reports, photographs of sample locations, the Township Assessor s information for the building, a schematic of the building layout, and the credentials of the inspectors. Inspection Summary The scope of inspection included the interior and exterior of the commercial structure. It was observed to be occupied by Payday Loans at the time of the inspection. The Property is targeted for demolition and redevelopment by the City of Rock Island. No-destructive sampling was employed to gather representative samples of suspect ACMs (asbestos containing materials). The inspection included apparent homogeneous sampling locations of suspect materials, including layers within materials. Random samples were collected from cove base/mastic, ceiling tiles, ceiling tile mastic, drywall, and carpet mastic. A minimum of three samples were collected from each homogeneous area. Samples were delivered to EMSL laboratories at its Chicago location to be analyzed for the presence of asbestiform material. The laboratory analyzed 33 bulk samples, including samples with multiple layers. The asbestos inspection was conducted in accordance with OSHA regulation 29 CFR , in specified areas agreed upon by the CLIENT. Analysis of the samples was completed by Polarized Light Microscopy Dispersion (PLM) Staining Method EPA/600/R-93/116. This survey did not include the internal components, material in storage within the building, or materials considered outside the definition of building materials. Care should be taken when removing, restoring, or disassembling these items. Description of the Buildings in the Scope of Work The survey took place April 8 & 23, 2013 at th Avenue, Rock Island, IL. The commercial structure is one story in height. There were no additional structures associated with the building, and no evidence of additions to the original wood frame structure. Per the Assessor s report, the building was constructed in 1980 and comprises approximately 1,768 square feet. The building foundation was observed to be a concrete slab on grade, with no basement. The frame is wood with rolled fiberglass insulation and exterior wood façade. The eastern portion of the roof is pitched with shingles and a metal drip edge; the western portion of the roof is flat with a plastic membrane. 1

NESHAPs ACM Report - Property E April 30, 2013 The interior was observed to have a combination of drywall and wood paneling walls, suspended ceilings, and a combination of floor coverings. The east side of the building was observed to have newer carpet laid over concrete; the concrete underlay was confirmed in three individual locations. The two bathrooms and a west storage area were observed to have ceramic tile floors. A drywall ceiling is present above the suspended ceiling in the eastern portion of the building. Glued-on ceiling tiles were observed on this drywall ceiling. The structure is heated and cooled with a forced air system with an HVAC unit and condenser mounted on the roof. Suspect asbestos materials sampled and analyzed under the prescribed scope of work are listed below. The following discussion includes individual materials determined to contain a minimum of 1% ACM: CTM-03 The ceiling tile puck mastic above CT-03 in the approximate eastern half (884 square feet) of the building was determined to contain a regulated quantity of asbestos. This material was found to contain 2% Chrysotile asbestos and is considered non-friable in its observed condition. The ACM material was observed to be stuck between acoustic ceiling tiles and drywall. Limitations to the Inspection The inspection was conducted systematically in accordance with NESHAPs requirements. The roofmounted sign contained caulking that was not accessible for sampling. Also not accessible was the attic above the eastern pitched roof; rolled fiberglass insulation was observed above the suspended ceiling in areas. EnviroNET cannot warrant that all ACM material was identified and analyzed, only that apparent materials were appropriately profiled. Because the Property was occupied, sampling there was aimed at inconspicuous locations. Carpet was pulled back from corners in most if not all rooms. EnviroNET surveyed accessible (interior and exterior) suspect asbestos containing materials, excluding the inventory, appliances, material in storage within the building, or materials considered outside the definition of building materials. Based on the experience of the inspectors and the observations made, the conclusions of this report are believed to be accurate and true, despite the limitations as described above.
