3 HAZARDS, VULNERABILITY AND RISK ASSESSMENT REPORT (HVRA) Provides the scientific information related to Quake Earthquake Hazards: Fault Location, Ground Shaking, Liquefaction, Fire Following Earthquake Risk Parameters: Casualty Estimates, Distribution of damage to buildings, critical facilities, high loss facilities Potential damage to Transportation systems and lifelines Identification of hotspots for Flood, Quake and Combined

4 Structures COLAPSED and DAMAGED Due to ground shaking In the Post Earthquake Assessment structures collapse/damage 18,782- fatalities 76,863- injured 68,619- families displaced

5 The Hotspots map shows the Top 5 Barangays suffered the most Collapse Buildings and have the most numbers of FATALITIES

11 STANDARD OPERATING PROCEDURES 0-24 Hours.Conduct of master listing and intake interviews of affected families (children, elderly, PWDs), issue identification cards per family, clustering of affected families into groups, assigning them to their respective tents and provide necessary orientations, further assessment and tasking. Provide other basic services such as Critical Incidence Stress Debriefing (CISD), daycare session for children, among others.

12 Refer sick victims to Health Sector. Continue to manage the community kitchen and the provision of hot meals to evacuees. The camp manager shall perform inventory of supplies and other needs and makes necessary request to the sector lead. In case of donations, the camp manager shall acknowledge receipt of supplies, inspect and records quantity and contents, and assist donors in the distribution of food packs. The camp manager shall provide feedback and updates to the sector lead. Basic Social Services SIMEX in the 4 th quarter NSED last November 14, 2014.

16 NEEDS AND ACTIVITIES INVENTORYFORM 6 Established trained personnel for Camp Management, Stress Debriefing, Health Services Conduct meeting to Private Business Sector for Public and Private partnership. Coordinate with Peoples Organization thru QC Community Relation Office (CRO) for training activities. Organizations to be utilize for disaster management. Set meeting with Super Market Association, Drug Store Association for signing of Memorandum of Agreement (MOA) in relation to the sustainable food and medicine supply in a disaster management.
