1 RE: ITB: PORTLAND COMMUNITY COLLEGE SW 49 th Avenue Portland, Oregon Sylvania Campus Bookstore Remodel Dated: February 22, 2017 April 11, 2017 ADDENDUM NO. 4 This addendum, together with the original invitation to bid forms and specifications shall form the complete invitation to bid. This addendum sets forth only the changes and additions which are to be made in the original invitation to bid. The original documents remain in full force and effect except as specifically modified in this addendum. Section IV, Page Eighteen, Paragraph D, Sub-Paragraph 1: ADD the following: The statement addendum number 4 received and complied with must appear on the envelope under the Bid title. Appendix A, Bid Form, Page 1 of 5 (Revised), Alternate 4, Paint exposed metal on stairs and stair guardrails: CLARIFICATION FOR ALTERNATE 4: All currently painted balusters and framing (including that on the underside of the stairs) to be repainted. Stainless steel cable and railings to remain as is. Railings for the second floor balcony should match the stair railings. BID: APPENDIX A SYLVANIA CAMPUS BOOKSTORE REMODEL BID FORM DUE DATE: 2:00 PM, APRIL 19, 2017 (To be returned) The Undersigned hereby bids to furnish to Portland Community College all materials, services and labor necessary to perform all Work for Portland Community College in strict accordance with the terms, conditions and requirements as specified in the Invitation to Bid and agreed upon. Total Lump Sum Base Bid $ Alternate 1 ADD $ (Total amount from page 5 of 5) Provide resilient floor per Section of the Technical Specifications, in all locations labeled FL-1 on plans. Alternate 1 DEDUCT $ Concrete polish as spelled out in Section of the Technical Specifications Alternate 2 ADD $ Remove and replace existing Carpet and Rubber Base, and provide new paint in offices 107, 109, 110, and conference room 108; and recycle carpet as specified in ITB. Alternate 3 ADD $ Replace all acoustic ceiling panels in existing suspended Ceilings in retail area. Provide new Armstrong MESA Second Look Panels per PCC Capital Project Standards. Existing grid to remain. Alternate 4 ADD $ Paint exposed metal on stairs and stair guardrails.

The minimum number of days bid prices must be valid is sixty (60) days. 4. BID SECURITY: X Required Not Required Enclosed is bid security for not less than ten percent (10%) of the total amount bid. 5. PERFORMANCE AND PAYMENT BOND: The Undersigned agrees, if awarded a Contract, to execute and deliver to Portland Community College, within ten (10) days after the award of the Contract, a satisfactory performance and payment bond in an amount equal to one hundred percent (100%) of the Contract Amount. 6. CONTRACT EXECUTION: The Undersigned agrees that the bid security accompanying this bid is left in escrow with the Owner, that the amount thereof is the measure of liquidated damages which the Owner will sustain by the failure of the Undersigned to execute and deliver the above named Contract and Performance and Payment Bond; and that if the Undersigned defaults in either executing the Contract or providing a Performance and Payment Bond within ten (10) days after receipt of the Notice of Award of the Contract is provided to him/her, then the bid security may become the property of the Owner at the Owner s option; but if this bid is not accepted within forty-five (45) days of the submission date, or if the Undersigned executes and timely delivers said Contract and Performance and Payment Bond, the bid security shall be returned.

4 Page 3 of 5 (2nd Revision) 7. PREVAILING WAGES: The Undersigned agrees that ORS 279C.840 will be complied with. Pursuant to ORS 279C.840(1), the hourly rate of wage to be paid by any Contractor or Subcontractor to Workers upon all public works shall be not less than the prevailing rate of wage for an hour s work in the same trade or occupation in the locality where the labor is performed. Pursuant to ORS 279C.840(4), every Contractor or Subcontractor engaged on a project for which there is a Contract for a public works shall keep the prevailing rates of wage for that project posted in a conspicuous and accessible place in or about the project. The Commissioner of the Bureau of Labor and Industries shall furnish without charge copies of the prevailing rates of wage to Contractors and Subcontractors. 8. MINORITY AND WOMEN-OWNED SUBCONTRACTOR PARTICIPATION: a. The Undersigned certifies that he/she will subcontract percent of the total Contract amount to ethnic minority-owned subcontractors. b. The Undersigned certifies that he/she will subcontract percent of the total Contract amount to women-owned subcontractors. c. The Undersigned agrees that if he/she is selected to perform the Work, he/she shall, by the close of the next working day following such notifications submit to the College a listing of names of minority-owned and womenowned subcontractors who will be used and the nature and dollar amounts of their participation. 9. CONSTRUCTION CONTRACTORS BOARD REGISTRATION: All bidders must be registered with the Oregon Construction Contractors Board (CCB) prior to submitting bids for public improvement projects. A bid for a public improvement Contract may not be received or considered by the contracting agency unless the Bidder is licensed by the CCB.
