The Board of Directors has hired HSA Engineers & Scientist, a consulting engineering firm, to review the construction of the common elements of our Ventura Condo buildings. To assist in the investigation, you are urged to complete this questionnaire. Please check yes or no in the appropriate column to answer the questions. Feel free to add comments, especially describing or identifying specific locations of any problems. This questionnaire relates only to the COMMON ELEMENTS of the Ventura Condo buildings. It does NOT include the clubhouse, pool and other common areas that are the responsibility of the Master Association. Questions regarding in-suite elements are only included as they may point to deficiencies in the larger, common element systems. UNIT NUMBER: NAME (OPTIONAL):

Additional Comments: *1 Some amount of noise transfer is expected between Condominium units. Please only respond "YES" to these questions if you hear normal conversations or other sounds generated by normal everyday living activities. Also list the type of noise you hear (i.e. conversations, phone, TV, alarm clock, etc.)

Please identify only severe condensation that occurs during moderate weather conditions. Condensation moisture problems are common concern for many homeowners. These may not be indicative of construction problems. Instead, homeowners may need to take steps to reduce condensation by controlling humidity in cold weather.
