1 Charter Township of Shelby Timothy Wood Acting Building Director Phone: (586) Van Dyke Fax: (586) Shelby Township, MI OUTLINE PROCEDURES FOR ERECTING A SINGLE FAMILY RESIDENCE PERMIT PROCEDURE A building permit shall be secured by the OWNER or BUILDER. Submit the information indicated below and allow at least two weeks for plan review and processing. The permit applicant will be contacted when the permit is ready to be picked up. An application fee of $ is required at the time the application is submitted. 1) Permit Complete all applicable sections of application. Missing application - information will cause delays. 2) Plot Plan - See Attachment A for Plot Plan requirement outline (4 copies) 3) Construction Provide foundation plan, floor plan indicating all proposed Plans uses and room dimensions, elevations, and cross section(s) (3 copies) indicating details of footings, foundation walls, framing, exterior wall coverings, roofing, etc. Architect or engineer sealed plans are required for houses that exceed 3,500 s.f. in area. 4) Tree Permit A tree permit must be obtained for all new houses prior to the issuance of a building permit. Check with the Building Dept. to see if a subdivision permit has already been issued. 2 Registration All residential builders must be registered. All electrical, plumbing, and mechanical contractors must be registered. Property owners must sign a Responsibility Affidavit for homeowner permits. Certificate of House Grade and Location An as-built Preliminary certificate of house grade and location (Grade Certificate) shall be submitted prior to calling for a garage footing inspection. Allow sufficient time for Department approval, prior to calling for footing inspections. A Final Grade Cert shall be submitted for review and approval prior to final building inspection. Special Conditions In some instances, special approvals may be necessary from the Township Board, Planning Commission, or the Zoning Board of Appeals. Separate Permits Required for: Electrical, Heating, A/C, Gas Piping, Plumbing, and Pre- Fabricated Fireplaces. All construction shall comply with the 2000 Michigan Residential Code. SPECIAL NOTICE REGARDING DEED RESTRICTIONS AND EASEMENTS Please be advised that construction activities that are permitted by the Building Department may be further regulated by Subdivision Restrictions or Easement Agreements applicable to the subject property. In some cases, activities permitted by the Building Department may be prohibited by subdivision restrictions or easement agreements. You are strongly advised to check that all current and future lot improvements are in conformance with any applicable subdivision restrictions and easement agreements. Further, you should obtain approval of the subdivision association (if applicable) prior to construction. The laws pertaining thereto generally make the property owner, or agent, responsible for such, and failure to conform may subject you to prosecution. The installation of doors and other points of ingress and egress in proximity to easements are done at the property owner s risk. No present or future construction and/or lot improvements of any kind shall be permitted within an easement. To obtain information on recorded subdivision restrictions and easements, the Macomb County Register of Deeds provides the most complete property records and can be reached at (586) In many cases, the Township Clerk s office has copies of restrictions that may apply. The permit holder is responsible to secure the appropriate approvals for placement of a fence in an easement area and compliance with all applicable subdivision restrictions. The authorization of a permit by the Building Department shall not be construed as authority to violate any easement or subdivision restrictions. 3 THE FOLLOWING INFORMATION MUST APPEAR ON THE PLOT PLAN NOTE: If homes have been constructed on either side of a proposed home, the plot plan shall show the existing improvements in order to insure: a) Proposed grading is compatible with the existing b) Verify uniform garage placement. 1. Variances from the approved grading plan will be considered on a case by case basis when submitted with the initial plot plan 12. Side entrance garages require special consideration with regards to brickledge, garage floor elevation, driveway slope and adjacent side lot line grades. 13. Existing trees ADDITIONAL NOTES A licensed land surveyor or professional engineer must prepare plot plans Exception: A homeowner builder may prepare plot plan provided it contains all information as required above. Swales must extend along the sides and/or rear of properties, in order to direct surface drainage to a catch basin or ditch. All grading must conform to approved subdivision grading plan. As built grade information shall be indicated on the preliminary and final grade certificates. A township provided cover sheet, which has been sealed by an engineer or land surveyor, MUST accompany the preliminary and final grade certificates. rvsd 05/11 Page 3 of 8

I am the Applicant on the attached Application for a Tree Removal Permit submitted pursuant to the provisions of the Tree and Woodland Preservation Ordinance of the Charter Township of Shelby and have personal knowledge of the facts set forth in the aforesaid Application and this Affidavit. 2. No trees six (6) inches or greater in diameter when measured at four and one-half (4-1/2) feet above the existing grade exist on the site; 3. I understand receipt of the aforesaid Application by the Township grants my permission and consent for the Charter Township of Shelby to conduct on-site inspections and investigations; and, 4. I understand that if one or more trees are found to exist on the site contrary to what is stated in this Affidavit, a tree location survey will be required before my Application will be considered. 5 Energy Code Worksheet for new Single-family Residential Building Building Department Van Dyke, Shelby Township, MI Phone (586) Fax (586) To facilitate permit issuance and enable the plan reviewer to verify compliance with the applicable provisions of the 2009 Michigan Uniform Energy Code (MUEC), please complete this form (Parts I, II and III) and submit it along with your application for a new single-family residential building permit. Project Address: Part I - Mandatory Provisions The following requirements apply to all new single-family residential buildings. Indicate that you understand and will comply with the following provisions by checking the associated box. MUEC Sec. # Description Applicant Prior to final inspection, a permanent energy code certificate shall be posted on or in the electrical distribution panel. Such certificate shall be on a label approved or provided by the Building Department. The building thermal envelope shall be durably sealed to limit infiltration. The sealing methods between dissimilar materials shall allow for differential expansion and contraction. The following shall be caulked, gasketed, weatherstripped or otherwise sealed with an air barrier material, suitable film or solid material: All joints, seams and penetrations Site-built windows, doors and skylights Openings between window and door assemblies and their respective jambs and framing Utility penetrations, knee walls and attic access openings Dropped ceilings or chases adjacent to the thermal envelope Walls and ceilings separating a garage from conditioned spaces. Behind tubs and showers on exterior walls Rim joist junction Other sources of infiltration Building envelope air tightness and insulation installation shall be demonstrated to comply by either testing or visual inspection. Indicate the method you will use by checking one of the following boxes: Testing option (blower door) Prior to final approval, provide results of blower door test indicating that the tested air leakage is less than seven air changes per hour (ACH) when tested with a blower door at a pressure of 50 pascals (1 psf). Visual inspection (by Building Department) Request for the Building Department to conduct visual inspections to verify conformance. All sealing shall be completed and ready for inspection at the time of insulation inspection. Dept. Use Only rvsd 05/11 Page 5 of 8

6 MUEC Sec. # Description New wood-burning fireplaces shall have gasketed doors and outdoor combustion Applicant Dept. Use Only air. Windows, skylights, and sliding glass doors shall have an infiltration rate of no more than 0.3 cfm per square foot, and swinging doors no more than 0.5 cfm when tested in accordance with NFRC 400 or AAMA/WDMA/CSA 101/I.S.2/A440 and shall be listed and labeled by the manufacturer. Labels shall remain on windows until after insulation inspection. Recessed lighting in the thermal envelope shall meet 1 of the following: (a) Type IC rated, manufactured with no penetrations between the inside of the recessed fixture and ceiling cavity and sealed or gasketed to prevent air leakage into the unconditioned space. (b) Type IC or non-ic rated, installed inside a sealed box constructed from a minimum 0.5-inch-thick gypsum wallboard or constructed from a preformed polymeric vapor barrier, or other air-tight assembly manufactured for this purpose, while maintaining clearances of not less than 0.5 inch from combustible material and not less than 3 inches from insulation material. (c) Type IC rated and admitting not more than 2.0 cubic feet per minute of air movement from the conditioned space to the ceiling cavity when tested in accordance with ASTM E 283. The lighting fixture shall be tested at 1.57 psi pressure difference and shall be labeled. If using the Total UA or Simulated Performance alternatives, the area-weighted average maximum fenestration U-factor permitted shall be 0.48 for vertical fenestration and 0.75 for skylights. The average figures shall be indicated in the supporting documentation required for the alternative method chosen. If using the prescriptive method, mark not applicable (N/A). Controls. At least one thermostat shall be provided for each separate heating and cooling system. (a) A programmable thermostat shall be provided for forced air systems. (b) Heat pumps having supplementary electric-resistant heat shall have controls that, except during defrost, prevent supplemental heat operation when the heat pump compressor can meet the heating load. Sealing. All ducts, air handlers, filter boxes, and building cavities used as ducts shall be sealed. Joints and seams shall comply with Section M of the Michigan Residential Code (MRC). N/A N/A Building framing cavities shall not be used as supply ducts Mechanical system piping capable of carrying fluids above 105 degrees or less than 55 degrees shall be insulated to a minimum R All circulating service hot water piping shall be insulated to at least R-2 and shall have an automatic or manual switch to system off. Outdoor air intakes and exhausts shall have automatic or gravity dampers that close when the ventilation system is not operating. Heating and cooling systems shall be sized in accordance with Section M of the MRC. A heating/cooling plan and supporting documentation shall be submitted prior to rough mechanical inspection. Snow melt systems and pools have special requirements. If applicable, and provide supporting documentation indicating compliance. Otherwise, mark not applicable (N/A). N/A N/A rvsd 05/11 Page 6 of 8

7 Part II - Compliance Paths In addition to the mandatory requirements, the energy code requires you to choose one of three alternative compliance paths to demonstrate code compliance. Indicate the path you choose below by checking one of the following boxes and completing the instructions. Prescriptive (as prescribed by the code) If you choose to use the prescriptive method of compliance, you may demonstrate compliance by completing the attached Prescriptive Compliance Report Form. Sign the compliance statement below and attach a copy of the completed Prescriptive Compliance Report Form along with this form when submitting for a building permit. Total UA Alternative (trade-off method) Compliance with the Total UA Alternative method may be demonstrated by completing a compliance report using REScheck software provided free of charge at energycodes.gov. Sign the compliance statement below and attach a copy of the completed compliance report with this form when submitting for a building permit. Please note that the building plans shall show the same materials and methods you use to complete the REScheck form. For example, if you use basement wall insulation in REScheck, such insulation should be clearly indicated on the building plans too. Simulated Performance Alternative (performance analysis) Certain commercially available compliance software (e.g. REM/RATE, etc.) may be used to demonstrate that the proposed construction will have an annual energy cost that is less than or equal to the energy cost of the standard reference design. Please see Section 405 of the code for specific criteria. Such software shall generate a compliance report that documents that the proposed design complies and shall include information outlined in Section Sign the compliance statement below and attach a copy of the completed compliance report with this form when submitting for a building permit. Part III - Compliance Statement I have read and completed the above form and will insure that the actual construction complies with the Michigan Uniform Energy Code. A wood frame or equivalent retainer is required around the access when loose fill insulation is used. 1 The fenestration U-factor excludes skylights means R-13 cavity insulation plus R-5 insulated sheathing. If structural sheathing covers 25% or less of the exterior, R-5 sheathing is not required where structural sheathing is used. If structural sheathing covers more than 25% of exterior, structural sheathing shall be supplemented with insulated sheathing of at least R-2. 3 The second R-value applies when more than half the insulation is on the interior. 4 Or insulation sufficient to fill the framing cavity, R-19 minimum. 5 The first R-value applies to continuous insulation, the second to framing cavity insulation; either insulation meets the requirement. 6 R-5 shall be added to the required slab edge R-values for heated slabs.
