INTRODUCTION In accordance with 23 CFR 172 et al, the Village of Lisle Public Works Department seeks Statements of Qualifications ( SOQ ) from professional engineering firms interested in Phase III Construction Engineering services for the Benedictine Parkway Improvement Project. The Village is seeking the most qualified firm ( Consultant ) to perform the required professional construction engineering and observation services for this project without the consideration of fees. The selection of professional consultants shall be based on the firm s professional qualification experience, and expertise of key personnel to be assigned to the project. Responses to this RFQ should clearly demonstrate a thorough understanding of the project specifics, challenges, and detail the proposed approach to successfully completing the project. Responses that are generic, nonspecific, or simply reiterate the tasks listed in the RFQ will be viewed as less desirable. Submittals that reflect thoughtful, innovative, comprehensive, and cost effective understanding in developing a proposed approach typically are viewed more favorably than a generic response. II. PROJECT SUMMARY The Benedictine Parkway Improvement Project consists of the hot mix asphalt surface removal and replacement of Benedictine Parkway along with the addition of a right turn lane from Benedictine Parkway onto Maple Avenue. The addition of the right turn lane will require temporary traffic signals, proposed traffic signals, earth excavation, full depth pavement, curb and gutter removal, proposed curb and gutter, fire hydrant relocation, and maintenance of traffic requirements. Also included is the improvement and upgrade of all sidewalk corners to current ADA requirements. A copy of the Pre-Final Plans and Specifications are attached to this RFQ. These documents are not approved for construction and should only be used to become familiar with the scope of the project. III. SCOPE OF SERVICES The Construction and Phase III Engineering for this project will be funded in part with federal Surface Transportation Program (STP) dollars allocated to the Village of Lisle. The construction contract will be let and administered by the Illinois Department of Transportation ( IDOT ). Phase III Engineering will need to follow the requirements of IDOT as they relate to federally funded projects. The scope of services shall include pre-construction activities; as-built surveys; verification of quantities; utility coordination and conflict resolution; shop drawing review; resident engineering services; meetings and coordination; material testing; inspection, and certification; record preparation and maintenance of documentation; pay estimate submittals; change orders; and other activities associated with construction engineering for roadway construction that would enable the project to be completed in a timely manner.

3 Quality Assurance ( QA ) oversight of Contractor s Quality Control must be performed by the respondent firm or one of its sub-consultants. The firm must provide all necessary vehicles and equipment to perform the required services. The respondent firm must be prequalified by IDOT for Special Services Construction Inspection. Construction Engineering Services will be required from the Bid Letting Date through final acceptance of the construction contract. The anticipated timetable for this project will be a Bid Letting in June 15, 2018 and a construction completion date of October 26, 2018 plus ten working days. IV. SUBMITTAL INSTRUCTIONS Interested firms shall submit one (1) original and two (2) copies of their qualifications in a sealed envelope plainly marked with the title of the RFQ and the submitter s full legal name. It shall be addressed and delivered to the place and before the time set forth above. Qualifications may be delivered by mail or in person. Submittals received after the time specified above will be returned unopened. V. SUBMITTAL REQUIREMENTS Firms interested in providing any of the above Scope of Services to the Village shall submit to the Village the following: Letter of Interest; and Statement of Qualifications ( SOQ ) SOQ must include the following information: 1. The name of the firm, corporate office address, closest office address, and a brief history; 2. A statement that the firm is able to provide all of the services in the Scope of Services list above, with current, in-house staff, or if not, a specific listing of those services the firm will provide in-house, and those it proposes to provide through sub-consultants; 3. Narratives of the individual who will be assigned as the Village s main point of contact and other key personnel who might be assigned to provide the scope of services outlined above, as well as an organizational chart of all persons employed locally by the firm who will be part of this scope of work. 4. A listing of other municipalities for whom the firm currently provides (or within the past year) similar services, including your primary contact at each municipality; 5. Related project experience of the firm during the last five years, including project name and description, assigned resident engineer for the project, client name, client contact information, month and year completed, construction cost, and the total fee charged the client;

Copies of the firm s registration with the State of Illinois as an Illinois Professional Design Firm and a copy of the firm s prequalification with the Illinois Department of Transportation indicating they are prequalified in the category of Special Services Construction Inspection; 7. Any additional information not listed above which you believe may be useful and helpful to determining the qualifications of your firm, provided however, that the total submittal not exceed thirty single sided pages. VI. EVALUATION PROCESS Submittals will be evaluated and ranked based upon the following criteria: Overall Quality of SOQ; Personnel Qualifications; Experience & References; Proximity to project Location; Technical Approach The Village intends to review and evaluate each SOQ based upon the aforementioned evaluation criteria. After review and evaluation, the Village will shortlist no less than three (3) firms and may identify certain firms with whom the Village desires to conduct an interview. Upon preliminary selection of the most qualified firm, the Village may conduct negotiations concerning the final Scope of Work, proposed pricing, the delivery of the Services, or any other contractual terms. The Village shall not be responsible for any costs incurred by any person or entity to prepare, submit or present its response to this request. Additionally, the Village reserves the right in its sole and absolute discretion to reject any and all statements, to waive any irregularities in any submitted statement or to accept any statement, which will best serve the interest of the Village. VII. QUESTIONS Any inquiries or requests for clarification shall be directed to the Procurement Services Manager, Joe Catalano, via to procurement@villageoflisle.org no later than 2 p.m. on Tuesday, January 30, Responses will be provided in an addendum published to the Village s website at No clarification will be provided verbally, either in person or over the telephone, and no one other than the Procurement Services Manager is authorized to provide clarification on behalf of the Village. VIII. AWARD SCHEDULE The Village intends to select a preferred consultant no later than February 16, A draft Construction Engineering Agreement must be submitted to the Illinois Department of Transportation by March 5, Once the draft is reviewed by IDOT, it will be brought to the Village Board for approval.

5 IX. CERTIFICATIONS By submitting an SOQ in response to this RFQ, Consultant hereby represents and warrants as follows: A. Independent Contractor: For purposes of the services described herein, the Consultant is an independent contractor and is not, and may not be construed or deemed to be an employee, agent, or joint venturer of the Village. B. Compliance with Laws: All Services must be provided, performed, and completed in accordance with all statutes, ordinances, codes, and regulations applicable to the Services. Consultant shall fulfill all obligations due under the Freedom of Information Act at no cost to the Village. C. Permits and Licenses. The Consultant must obtain and pay for all permits and licenses, registrations, qualifications, and other governmental authorizations required by law that are associated with the Consultant s performance of Services. D. Representation of No Conflicts. The Consultant represents that (1) no Village employee or agent is interested in the business of the Consultant or this contract, (2) as of the effective date neither the Consultant nor any person employed or associated with the Consultant has any interest that would conflict in any manner or degree with the performance of the obligations under this contract, and (3) neither the Consultant nor any person employed by or associated with the Consultant may at any time during the term obtain or acquire any interest that would conflict in any manner or degree with the performance of the obligations under this contract. E. Not Barred. The Consultant represents that the Consultant is not barred from contracting with a unit of federal, state or local government. F. No Collusion. The Consultant represents that the only persons, firms, or corporations interested in this contract as principals are those disclosed to the Village prior to the execution of this contract and that this contract is made without collusion with any other person, firm, or corporation.
