Submittal Shop and Field Reports (Uploaded Daily/Weekly to Website). 20

21 Structural & Architectural Steel Documentation CCE Responsible for Field Bolting Procedure and Testing Bolts must be in a sealed container with all necessary paperwork (CMTR s, Rotational Capacity testing by Producer, etc.) Proof of IDOT testing is required Testing of the bolts in the field is required prior to use (Material Compliance) Testing of the bolts in the field as installed is also required (Installation Compliance) QA is available for Questions as needed QA Responsible for Shop Bolting Procedures and Testing Verify proper testing by IDOT QA will typically have oversight on a percentage of the bolts installed Using the same procedure for testing Material Compliance and Installation Compliance as required by the CCE in the Field. 21

34 Miscellaneous Material Documentation RFINT Form RFINT Request For Inspection of Nursery Trees CCE Manual Section XI.K.1.a Contractor Submits Three Copies to CCE CCE Reviews for Completeness and Forwards to CDOT Division of Project Development Seven Week Lead Time Required CDOT Senior Forester Inspects and Accepts Once Accepted CCE Assigns a T- Number and Approves CCE Returns a Copy to the Contractor CCE Retains a Copy CCE Forwards Two Complete Submittals to CDOT QA All Trees Delivered Must Have a CDOT Seal Attached. 34

36 Miscellaneous Material Documentation - RFIPM RFIPM Request For Inspection of Plant Material CCE Manual Section XI.K.1.b Same Steps as Detailed for the RFINT Process, except Three Copies of the Completed RFIPM Forms are Sent to CDOT - Division of Engineering, and Planting Material is Accepted by CDOT s Horticulturist Planting Material Not Planted Within 60 Days of Inspection Will Require Re-Inspection Next Season. Additionally, Plant Material Not Planted by October 15 Will Require Re-Inspection Next Season. 36
