Program of Requirements Texas Tech University Research Section 1 General Space Information List all Building(s), Room Number(s), and planned use (Research Lab, Research Lab Service, or Office) Biosafety Levels (BL) BL 1 Defined and characterized strains of viable microorganisms not known to cause disease in healthy adult humans BL 2 The broad spectrum of indigenous moderate risk agents present in the community and associated with human disease of varying severity BL 3 Indigenous or exotic agents with a potential for respiratory transmission and which may cause serious and potentially lethal infection *Limited BL 3 Texas Tech University Availability BL 4 Dangerous and exotic agents that pose a high individual risk of life threatening disease, that may be transmitted via the aerosol route, and for which there is no available vaccine or therapy *BL 4 is not available at Texas Tech University

Section 2 Environmental Characteristics Please complete the following chart using the most accurate information available. a. Room or Laboratory Specifications Ceiling Height If there are special requirements, state clear height in feet and inches: Circulation State any special needs to accommodate ambulatory and non ambulatory workers, equipment, or movable machinery: Doors Are large door openings required? If so, specify size: Security Specify and additional security requirements including Access Control and/or Monitoring Capabilities: Signage What, if any, signs are required for the building due to the nature of your research? Other considerations? Please be specific.

Section 2 Environmental Characteristics (continued) Please complete the following chart using the most accurate information available. b. Heating, Air Conditioning, Ventilation, and Safety Standard for building is all that is required? Must be air conditioned 24x7? Accurate temperature control is required? If so, state range to Temperature and limits of plus or minus of. Accurate humidity control required? If so, give range from to %RH and limits of plus or minus %RH. Humidity Other? Please specify here: Special exhaust is required for: Excessive heat Exhaust Vapors Smoke Radioactivity Any other safety features required (click all that apply): Alarms Safety shower Safety Features Eyewash Volatile liquid storage Compressed gas storage Other specify here:

Sound attenuation required? Sound Will this research activity produce noise disturbing to others? What are any additional lighting requirements? Please check all that apply. Natural daylight Above average intensity Lighting Ultraviolet Dimmer control Blackout capability Fire Noxious fumes Acids Contamination Hazards Electrical Spillage Waste radioactivity Others? Specify all that apply here: Do any instruments or other equipment have substantial floor loading requirements? Please identify equipment and requirements. Floor Loading Are there any instruments, such as SEMs or TEMS that are sensitive to electromotive force and/or vibration? List all sensitive equipment here. EMF/Vibration

Section 3 Communications and Audio/Visual Requirements Please include brand name equipment, model #s, size, and quantity for all communication and a/v equipment. Communications and Audio/Visual Requirements How many handsets are needed? Telephones Attach a diagram for placement. Do you prefer paging and intercom to be included in the communications package? What are your TV needs? Please be specific as to brand name, model #, etc. Attach a diagram for room placement. Closed Circuit TV/Cable TV Projection Do you require overhead projection equipment and a screen? List brand name, model #, size of screen. Include a diagram for placement in the room. Specify type, quantity, and sizes. Attach a diagram for placement in the room. Chalkboards/Dry Erase Boards Specify type, quantity, and sizes. Attach a diagram for placement in the room. Tack Boards Please list any equipment, features, renovation, construction, etc., that is not listed elsewhere. Other

Section 4 Equipment Every laboratory requires fixed and movable casework or benches for storage and work areas. Equipment is also an integral part of any laboratory. In the following chart, please be as specific as possible. Your needs may dictate that we contract with an outside agency to render drawings, build cabinets, shelves, etc., and install them in the research facility. a. Fixed Equipment Laboratory Include drawings with dimensions, if possible. casework or benches Include diagrams for placement on walls. Indicate dimensions. Installed shelving Mounted or attached equipment Include exact specifications for mounting equipment. Indicate if special mounting kits are required for equipment. Does the equipment require certain utility services? Utility services If so, be specific as to the type of utility and when and how it is accessed. What are the power requirements for mounted or attached equipment? Power requirements What are the power requirements for freestanding equipment? Please specify any requirements not listed elsewhere. Other

b. Movable Equipment Have all items that affect space needs been addressed? Are dimensions and hand drawings provided for custom built storage, work surfaces, shelves, supply cabinets, etc.? Develop a movable equipment list with estimated prices to submit with this POR. Does the movable equipment require specific safety training, clothing, or cost which will be included in grant funding? If so, outline those specifics here: What utility and power services are required for movable equipment? Provide a diagram for outlets, voltage requirements, etc. A Note about Equipment Maintenance Contracts: For all fixed and movable equipment, the cost of maintenance must be included in grant funding. Most research laboratory is highly specialized and in house maintenance providers are not trained or funded to work on highly sensitive or unique equipment. In fact, to do so in some cases would nullify the equipment warranty. You must ensure that equipment inspections and maintenance costs are funded by the grant and that you have detailed, written checklists for all of your research equipment.
