1 PROJECT OVERVIEW Exhibit A MnDOT Contract No SCOPE OF WORK AND DELIVERABLES Reconstruction of US65 in City of Albert Lea from Newton Avenue to I-35 State Project (SP) The State is in need of professional, technical assistance to provide preliminary and detail design for reconstruction of US Highway 65 (US65) northbound and southbound in the City of Albert Lea. The project spans approximately 2.7 miles, from the junction of Newton Avenue to the Interstate 35 (I-35) ramps in the City. The Contractor s tasks will include, but are not limited to, completion of environmental documentation, preliminary and detail of hydraulics, geometric layout, public involvement and coordination, vehicular and pedestrian traffic facilities (such as signal additions, Americans with Disabilities Acts (ADA) ramps, and accessible pedestrian signals (APS)), traffic control, design memorandum, field surveys, utility coordination and relocation plans, construction limits, road plans, special provisions and permits. 1. PROJECT MANAGEMENT (Activity Code 1010) Project management will include work necessary for communication and completion of the project tasks on time and within budget. The CONTRACTOR s Project Manager or their primary duties will not be reassigned without the written consent of the State s Project Manager. The CONTRACTOR s staff will have the training and expertise necessary for the work tasks to which they are assigned Administration Administration of the project will include meetings, progress reports, invoicing, preparation of contract amendment requests (if necessary), cost and schedule updates, billing preparation, other non-technical work, communication with the necessary project personnel, sub-consultant management, and all other work to ensure all the project tasks are completed on time, within budget, and in accordance with State and Federal laws, rules, and regulations. Progress reports and invoices will be submitted on a monthly basis. The CONTRACTOR will submit invoices in a timely manner. The CONTRACTOR shall hold weekly Coordination Meetings (teleconference) with MnDOT's Project Manager (assume 1hr per week until plan turn in) Schedule Management The State will maintain a project schedule throughout the duration of design activities while this contract is in effect. The CONTRACTOR will review the State s schedule as requested by the State s Project Manager and provide timely updates to in-progress or completed activities. At the State s request, the CONTRACTOR will also provide a narrative Recovery Plan if any milestone described in Sec. 15 Plan Review and Approval is more than 10 working days behind schedule Kick-off Meeting The Kick-off meeting will establish a communications protocol and discuss the project schedule. The CONTRACTOR will receive available existing information from the State. There will be State provided survey and mapping data, previous studies completed for the area, a brief discussion of known issues, and a review of the project schedule. The CONTRACTOR will prepare a meeting agenda and minutes Project Management Team Meetings Meetings will be held monthly during the preliminary and detail design. With the CONTRACTOR s Project Manager present, the project schedule and budget status will be reviewed. 2 and addressed. The CONTRACTOR will prepare an agenda and facilitate the Project Management Team meetings. The CONTRACTOR will also prepare and distribute draft minutes for review/comment to all meeting participants within 2 business days after each meeting and final minutes within 10 business days after each meeting Stakeholder Coordination Meetings The CONTRACTOR will hold up to seven (7) stakeholder coordination meetings with the State s Project Manager, other State staff, and stakeholders, as needed. These are supplemental meetings beyond those explicitly called for elsewhere in this Scope of work. The CONTRACTOR will prepare agendas and facilitate the coordination meetings. The CONTRACTOR will also prepare and distribute draft minutes within 2 business days after each meeting and final minutes within 10 business days after each meeting Quality Control Management The CONTRACTOR will maintain a Quality Management Plan (QMP) that specifies how the CONTRACTOR will perform Quality Assurance (QA) and Quality Control (QC) activities throughout the contract duration to ensure delivery of quality design in a timely manner and in conformance to contract requirements established for the project. The QMP shall be submitted to the State s Project Manager for reference within 20 working days of the Notice to Proceed. The CONTRACTOR will perform Discipline Coordination Reviews and quality control checking of each submittal. Deliverables The CONTRACTOR will: 1. Schedule and attend meetings. 2. Hold weekly conference call with State s Project Manager. 3. Prepare meeting agendas, displays, and minutes. 4. Prepare and submit monthly progress and status reports. 5. Provide updates to P6 project schedule. 6. Provide timely copies of critical correspondences and project issue data. 7. Submit Work Plan. 8. Quality Management Plan (QMP) The State will: 1. Review and respond to submittals. 2. Coordinate internal reviews. 3. Provide copies of project correspondences, project studies, and general project information. 4. Attend meetings. 5. Approve deliverables. 6. Monitor quality control. 7. Maintain P6 Project Schedule 2. PUBLIC AND AGENCY INVOLVEMENT (Activity Code 0054) With prior authorization from the State s Project Manager, the CONTRACTOR will perform direct contacts with State functional units, City of Albert Lea, Freeborn County, and other outside agencies to collect information needed for project development and final plans. CONTRACTOR will provide copies of communications with State functional units, City of Albert Lea, Freeborn County, and other outside agencies to State s Project Manager. 3 The CONTRACTOR will provide coordination for public, agency, and stakeholder involvement meetings. Tasks will include scheduling meetings, preparing meeting notices, and coordinating venues. The CONTRACTOR will provide staff and technical project information and displays for use during public meetings. 2.1 Public Involvement Public Information Meetings The CONTRACTOR will lead, provide support and necessary information for public outreach activities through open houses. The CONTRACTOR will organize and conduct up to three (3) Public Information Meetings in the project area, upon direction from the State Project Manager, to disseminate project information to the General Public. The CONTRACTOR will arrange and pay for adequate facilities, prepare, print, and mail meeting notices for up to fifty (75) addressees, and prepare exhibits and presentations. The CONTRACTOR will prepare a record of public comments and responses, with assistance from the State. The Public Information Meetings (PIMs) will be open house type and no formal, extended presentation will be required Small Group Meetings The CONTRACTOR will organize and conduct up to three meetings with small groups of landowners, business owners, elected bodies, and other Public Interest groups to discuss the project upon direction from the STATE s Project Manager. The CONTRACTOR will provide a summary of activities and outcomes from each meeting. The CONTRACTOR will prepare exhibits and presentations as needed. The content and timing of the meetings will be at the discretion of the State s Project Manager. 2.2 Agency Involvement The CONTRACTOR will prepare for and attend three Council meetings for the City. The content and timing of the meetings will be at the discretion of the State s Project Manager. Given the potential for change in access and Right-of-Way acquisition, it is likely that municipal consent will be required. This may also require an additional meeting with local business groups The CONTRACTOR will prepare for and attend up to two meetings with the Albert Lea Chamber of Commerce The CONTRACTOR will prepare for and attend up to two meetings in Project Area with agencies, such as Minnesota Department of Natural Resources (MnDNR), Minnesota Pollution Control Agency (MPCA) and Army Corps of Engineers (COE), that have concerns with potential impacts of the project. The first meeting is intended, at the onset of the project, to identify any concerns of the agencies about potential project impacts and to seek their inputs for addressing such concerns. The second meeting is intended for the CONTRACTOR to report on how the concerns identified were addressed. 2.3 Public and Media Contact The State will manage all public and media contacts (if any) for the project and will be responsible for disseminating information regarding public messaging to the relevant media venues. The CONTRACTOR will be responsible for drafting any public notices or advertisements that are required for Public Information Meetings. 2.4 Project Web Documents The CONTRACTOR will provide project information in web ready formats for the State s use in keeping the general public informed. The CONTRACTOR will not be responsible for hosting, or maintaining a website, but will be responsible only for supplying web ready documents. 1. 4 2. Documents and graphics provided to the STATE for website use will be in accordance with the STATE s online web requirements. Maps, graphics and text documents intended for printing or online viewing will be provided as Adobe PDF files. 2.5 Deliverables The CONTRACTOR will: 1. Prepare a public and agency involvement plan. 2. Prepare for, attend and present at Public Open Houses. 3. Prepare for, attend and present at the Agency Involvement meetings 4. Prepare City Council meeting agendas, minutes and attendance sheets. 5. Prepare Business meeting agendas, minutes and attendance sheets. 6. Obtain mailing list of affected owners, if any. The State will: 1. Assist and attend Public Information Meetings. 2. Assist and attend Agency Involvement meetings 3. Assist and attend Chamber meetings and Business meetings 4. Distribute Media releases of the Public Information Meeting for media distribution. 5. Manage Public and Media contacts and provide public messaging to relevant parties/venues. 6. Assist the CONTRACTOR in providing responses to public comments. 3. DATA COLLECTION (Activity Code 6265) 3.1. Design/Location Surveys The State will furnish previously completed Digital Terrain Mapping (DTM) files, survey control point information, and other previously completed mapping files. The CONTRACTOR will review the supplied survey information and will identify where supplementary survey work is required to complete the design deliverables, acquire additional data/information, or demonstrate the location of proposed features. The CONTRACTOR will perform supplementary design/location surveys. Supplementary survey work completed by the CONTRACTOR will use the same datum(s) and project controls that are identified in the State supplied mapping files. The CONTRACTOR will notify the State s Project Manager at least five (5) business days before performing any supplementary survey work in the field. The CONTRACTOR will verify survey datums used for data furnished by Third Parties (if any), such as hydraulic models or affected utilities. The CONTRACTOR will ensure that third party information (if any) is consistent with datums in use by the Project and consistent with Quality Control checks detailed in the QMP. The CONTRACTOR will update the base mapping to incorporate the supplemental survey information Traffic Forecasts The State has completed traffic forecasting for the design year and traffic forecasts will be furnished to the CONTRACTOR. No supplemental traffic counts or forecasting are required Wetlands All wetland delineations, if any, will be completed by the CONTRACTOR and furnished to the State in MicroStation.dgn V8i format Deliverables The CONTRACTOR will: 1. Review State furnished survey data. 2. Identify to the State s Project manager all supplementary survey work necessary. 3. 5 4. Verify consistency of Project controls relative to datums/controls in use for Third Party supplied data (if any). 5. Provide updated base mapping. 6. Perform wetland delineations, if needed, and any wetland permitting, if required. The State will: 1. Complete a cultural resource review and documentation. 2. Provide planning documents and transportation plans. 3. Provide existing right-of-way drawings and plats. 4. Provide utility Permits within the Project Limits. 5. Review the previously delineated wetlands, if any. 6. Provide record or As-Built plans of roadways, existing traffic signal phasing 7. Provide traffic data including available intersection turn movements, ADT Forecasts. 8. Provide culvert Hydraulic Recommendations (if any) including all culvert replacement, lining and repairs. Recommendations based on needs of inspection reports by State. 9. Provide staging/detour recommendations 10. Provide digital design files to be provided by the State. Files will be in the same format as the CONTRACTOR's submittal requirements (Microstation V8i and GEOPAK) complying with current MnDOT CAD standards. 11. Provide existing digital terrain model (DTM). 12. MnDOT has surveyed existing utilities and will provide this information to the CONTRACTOR. 4. ENVIRONMENTAL DOCUMENTATION (Activity Code 1071) 4.1. Class II (CATEX) Document The CONTRACTOR will prepare a Class II Environmental Document (CATEX) for approval in accordance with the requirements of MnDOT Prequalification work type 1.1, unless otherwise noted. The CONTRACTOR will perform on-going coordination with District 6 during development, processing, and approval of CATEX document. Assumptions: 1. Project meets the definition of a Class II Action (programmatic Categorical Exclusion) under NEPA. 2. Environmental document is a programmatic, long-form Categorical Exclusion (CATEX) Determination document. 3. Proposed improvements do not meet the threshold for mandatory EAW under MEQB rules. If MnDOT (as RGU for projects on state trunk highway system) determines that a discretionary EAW is necessary, additional scope will be prepared. 4. Assumes District 6 has completed Early Notification Memo (ENM) review process and will provide ENM responses for completion of CATEX. 5. Assumes District 6 will coordinate draft and final CATEX reviews and approvals with MnDOT Office of Environmental Stewardship (OES) and FHWA. 6. Assumes no Section 7 (Endangered Species Act) consultation required. Assumes a determination of no effect from OES. 7. Assumes the CONTRACTOR will complete all wetland delineation services, and the STATE will apply for all pertinent permits. 8. Assumes a qualitative MSAT analysis per Interim Guidance on Air Toxic Analysis in NEPA Documents Feb. 3, 2006 (or current) FHWA Memo. Assumes no quantitative air quality modeling (MSAT and CO) required. 9. Assumes a no historic properties determination to be provided by MnDOT Cultural Resources Unit (CRU) in response to ENM. 6 archaeology), if required, will be prepared by CRU. 10. Phase I environmental site assessment (ESA) requirements to be determined by MnDOT Environmental Investigation Unit (EIU). Assumes a Phase I ESA, if required, will be prepared by EIU. The CONTRACTOR will perform a review of the project area, using available mapping and on-line data, to confirm conditions of affected environment. The CONTRACTOR will compile data from the City of Albert Lea, Freeborn County, District 6, and other sources, including project background, funding, permit requirements. The CONTRACTOR will prepare description of existing conditions and proposed improvements, description of project cost, anticipated funding sources, anticipated schedule, and key contacts for environmental document. The CONTRACTOR will evaluate potential impacts of the project on parks, recreation areas, and trails and summarize Section 106 determination provided by CRU. Assumes no Section 6(f)/LAWCON involvement. Assumes a no historic properties determination. Any additional required cultural resources studies will be completed by CRU. The CONTRACTOR will identify vegetation, fish and wildlife impacts, and measures to minimize harm (assumes no impacts). District 6 to provide DNR response to ENM, including NHIS results. District 6 to provide OES response to ENM regarding federal threated and endangered species. Assumes determination of no effect finding. The CONTRACTOR will address excess materials, geology, groundwater, and earthborne vibrations. The CONTRACTOR will summarize drainage and water quality impact analysis for inclusion in CATEX document, address wetlands and floodplain impacts (assumes none), and address erosion control. The CONTRACTOR will consult with EIU regarding contaminated properties. Assumes EIU will prepare a Phase I ESA if necessary. Prepare Form EDD-1 (Environmental Due Diligence Form #1) and submit to EIU for review. Summarize contaminated properties review for inclusion in CATEX document. The CONTRACTOR will address visual impacts, land use impacts, farmland, tribal, social and economic impacts and identify any controversial issues. The CONTRACTOR will address access changes, traffic detours, and maintenance of traffic. The CONTRACTOR will coordinate with OES, FHWA, and MPCA staff regarding analysis methodology (if necessary) and prepare discussion of NAAQS criteria pollutants, qualitative CO analysis, and air quality conformity per MnDOT guidance. The CONTRACTOR will perform a qualitative MSAT analysis per Interim Guidance on Air Toxic Analysis in NEPA Documents Feb. 3, 2006 (or current) FHWA Memo. The CONTRACTOR will address right of way impacts (permanent and temporary right of way needs). The CONTRACTOR will prepare an environmental justice analysis and summarize findings in CATEX. Assumes no low income and/or minority populations within the study area. The CONTRACTOR will address bicycle and pedestrian movements, including accessibility. The CONTRACTOR will prepare graphics for CATEX document including project location map, USGS map, layouts, typical sections, drainage plan, and other supporting graphics. 7 The CONTRACTOR will prepare a draft CATEX document for District 6, OES, and FHWA review. This assumes that District 6 will submit draft CE to OES and FHWA and also assumes a concurrent OES/FHWA review. The CONTRACTOR will revise draft CATEX based on District 6, OES, and FHWA comments. The CONTRACTOR will prepare final CATEX for District 6, OES, and FHWA final review and approval. Assumes that District 6 will distribute final CATEX document to OES and FHWA for approval. The State will provide responses to the Early Notification Memo received for use in completing the CATEX document. The CONTRACTOR will submit the draft CATEX to the State for review and comment. Comments will be returned to the CONTRACTOR in writing. The CONTRACTOR will incorporate the State s review comments on the draft CATEX and prepare a Final CATEX for approval Phase I & Phase II Environmental Site Assessments (ESAs), Regulated Waste Assessments The State will conduct any required contaminated soils and regulated waste investigations. Any findings requiring contract Special Provisions will be drafted by the State. The State will provide any findings, reports, or required Special Provisions Deliverables The CONTRACTOR will: 1. Prepare a draft purpose and need statement for District review and incorporate review comments. 2. Prepare and submit a draft CATEX for District review. 3. Incorporate review comments and submit a Final CATEX for approval by November 19, Respond to agency inquiries. 5. Provide wetland review and determination, if necessary. The State will: 1. Provide Early Notification Memo and agency responses received to date. 2. Review draft CATEX and provide comments. 3. Obtain signatures for CATEX approval. 4. Conduct any required Environmental Site Assessments and Regulated Waste Assessments. 5. Complete Wetland permits, if necessary. 5. UTILITY COORDINATION (Activity Code 1195) 5.1. Utility Coordination The CONTRACTOR will perform utility coordination for the project in accordance with the MnDOT Utility Accommodation and Coordination Manual, Steps 3 through 9. Steps 1 and 2 will be completed by the State. The CONTRACTOR will function as the project manager for the steps indicated herein. Plans and other contract documents prepared by the CONTRACTOR for project letting will adhere to the requirements of Utility Coordination Step 12, as detailed in the MnDOT Utility Accommodation and Coordination Manual. The CONTRACTOR will organize and conduct up to two (2) Utility Coordination Meetings. The CONTRACTOR will provide meeting agendas, exhibits, and minutes. The CONTRACTOR will provide up to 2 individuals for attendance at the Utility Coordination Meetings. 8 will be held at the District 6 Office (2-hour meeting duration). The CONTRACTOR will prepare Utility Special Provisions. Exhibit A MnDOT Contract No Deliverables The CONTRACTOR will: 1. Serve as the project manager for Utility Coordination Steps 3 through 9, as detailed in the MnDOT Utility Accommodation and Coordination Manual. 2. Prepare contract documents in accordance with Step 12 of the MnDOT Utility Accommodation and Coordination Manual. 3. Schedule and facilitate up to two (2) Utility Coordination Meetings. 4. Provide agendas and minutes for Utility Coordination Meetings. The State will: 1. Provide the Utility Identification information (Steps 1 and 2). 2. Attend Utility Coordination Meetings. 3. Send out notice and orders to utility companies (steps 13 and 14) 4. Provide available existing utility information 5. Provide a single set of consolidated utility plan comments 6. INTERSECTION CONTROL EVALUATION (ICE) The CONTRACTOR will prepare an ICE Report. Intersection control alternatives will be evaluated at the US 65/Garfield Avenue intersection which is currently signalized and is to have a signal replaced with this project 6.1. Data Collection 1. STATE will collect peak hour turning movement data for the intersection analysis and provide AM and PM peak hour turning movement data. 2. STATE will provide 48-hour traffic volume data High Level Operations Comparison of Intersection Control Types (Activity Code 1808) CONTRACTOR will forecast traffic to the anticipated design year (2030). CONTRACTOR will analyze the intersection alternatives, including the following: 1. Existing and Future operations of the No Build alternative (side-road stop). CONTRACTOR will complete the analysis using HCM methodology (Synchro). 2. Future All-Way Stop Control analysis using Highway Capacity Manual (HCM) methodology (Synchro). 3. Future Traffic Signal level of service (LOS) analysis (with optimized cycle length and splits as determined via Synchro) using HCM methodology. 4. Future Roundabout LOS Analysis using RODEL software. Deliverable CONTRACTOR will provide a table summarizing LOS and average vehicle delay by intersection approach and turn movement comparing the existing conditions and the listed alternatives Safety Analysis / Expected Crash Reduction (Activity Code 1808) Utilizing traffic data provided by State (and supplemental data, if needed), CONTRACTOR will prepare a safety comparison to identify relative safety benefits of the alternatives. This analysis will be based on a review of the most recent five years of data on vehicle crashes within 300 feet of the study intersection. Existing annual total crash rates will be determined for the study intersection as well as the number of correctible crashes per year. This comparison will be based on an estimate of future crashes per alternative as determined via crash reduction factors (if available) or crash rates. 9 Deliverable CONTRACTOR will provide a table summarizing the safety comparison of the improvement alternatives Signal Warrant Analysis and Intersection Control Evaluation (ICE) Report (Activity Code 1808) CONTRACTOR will compile the data from the tasks above and prepare an ICE report according to published State requirements Deliverable CONTRACTOR will provide an ICE report completed according to published State requirements Meetings The CONTRACTOR will attend up to three (3) project meetings with to discuss the study process and results Deliverable CONTRACTOR will prepare agendas and minutes for kickoff, intermediate, and project results meetings Preliminary Roundabout Sketch (Activity Code 1140) Using aerial photography of the site, CONTRACTOR will prepare a concept roundabout sketch. This two-dimensional concept can be used to show potential impacts on the surrounding area Deliverable CONTRACTOR will provide drawing of concept roundabout over an existing aerial photography Task 3b - Preliminary Signalized Intersection Sketch (Activity Code 1251) Using aerial photography of the site, CONTRACTOR will prepare a concept signalized intersection sketch. This two-dimensional concept can be used to show potential impacts on the surrounding area Deliverable CONTRACTOR will provide drawing of concept signalized intersection over an existing aerial photography Preliminary Identification of Environmental/Site Issues (Activity Code 1071) CONTRACTOR will collect available data for potential environmental and site related elements that may be affected by the intersection improvements. CONTRACTOR will visit the site for a field walkthrough to evaluate potential issues and to confirm the general accuracy of the aerial photos in use Deliverable CONTRACTOR will identify potential issues and include them on the Layouts Identify Right-of-way Needs (Activity Code 1021) CONTRACTOR will collect available data from State and local agencies regarding the location of the public rights-of-way. The information will be superimposed onto the Intersection Layouts. CONTRACTOR will sketch and quantify the potential right-of-way needs for the Concept Sketch Deliverable CONTRACTOR will prepare a Right-of-Way Needs Map including the Intersection Layouts with potential needs identified and quantified Preliminary Cost Estimates (Activity Code 1140) CONTRACTOR will quantify the major items needed for construction of the Preliminary Roundabout Sketch and the Preliminary Signalized Intersection Sketch and prepare preliminary estimates of probable construction costs. 10 Deliverable CONTRACTOR will prepare preliminary cost estimates for each identified alternative Roundabout Geometric Design and Layout with RODEL (Activity Code 1140) If the study of the intersection improvement options identifies a roundabout as the preferred option, CONTRACTOR will complete a more detailed RODEL model. The geometry from the Preliminary Roundabout Sketch will be incorporated into RODEL with more precision. The RODEL analysis will identify operational capacity of each approach and will help identify the sensitivity of the geometric aspects in the design. Based on the RODEL analysis, CONTRACTOR will refine the Geometry and prepare a Geometric Layout including: 1. Location Map 2. North arrow and graphic scale 3. City/County limits 4. Basic topography in the form of an aerial photograph provided by State or other public source 5. Existing Speed Zones 6. Traffic data 7. Approximate in place right-of-way based available data from State or other public sources 8. In place roadway widths 9. Roadway names 10. Environmentally sensitive areas 11. Proposed Roundabout Geometry including face of curb, edge of pavement, pavement markings, primary dimensions and radius labels, taper labels, lane widths, traffic arrows 12. Proposed entrances and closures 13. Walks and trails 14. Proposed typical sections 15. Roundabout fast paths analysis 16. Critical design vehicle turning movements 17. Narrative summary of potential profile considerations This Layout will be prepared for use in this study only. Should a roundabout become the selected geometry for this intersection, preparation of an official Staffapproved Preliminary Geometric Layout, including State reviews and required revisions, will be supplemental Deliverable CONTRACTOR will provide an updated Geometric Layout and RODEL output data for AM/PM peak hours and for 50/85% confidence levels. 7. PRELIMINARY ROADWAYS DESIGN (Activity Code 1140) The CONTRACTOR will complete the preliminary design activities Lane, Turn-Lane, and Median Alternatives The CONTRACTOR shall evaluate the proposed conceptual median closures and median access modifications as provided by the State. This evaluation will require an ICE report. The CONTRACTOR shall determine recommendations for final design. Construction limits shall be provided to the State for review and approval Preliminary Geometric Layout The Contractor will prepare one Level 2 Preliminary Geometric Layout for State review to include any intersection or median access modifications. The Contractor will complete and submit the current version of the layout checklists before submitting the Preliminary Geometric Layout to the State. 11 State will review the Preliminary Geometric Layout and provide written comments to the Contractor to be incorporated into the Final Geometric Layout for Staff Approval Preliminary Cost Estimate The Contractor will prepare a preliminary cost estimate for the entire project and submit it with the Preliminary Geometric Layout. This cost estimate will be similar in format to MnDOT's LWD method Preliminary Drainage Design The CONTRACTOR will complete a hydraulic analysis for proposed storm sewer, culvert pipes, and ditches for the roadway. Peak discharges shall be determined by using the rational method and Atlas 14 data. A drainage area map with contours shall be prepared for new culverts, ditches or new storm sewer. The CONTRACTOR will prepare a Drainage Design Report as detailed in the State Drainage Manual, Section Preliminary ADA Design The CONTRACTOR will complete a preliminary ADA design based off of notes from the project Field walk as completed by the District and ADA Operations. The CONTRACTOR shall complete ADA design to a level to provide construction limits. Construction limits shall be provided to the State for review and approval Final Geometric Layout Upon receipt of Preliminary Geometric Layout review comments from the State, the Contractor will assess the comments for incorporation into the Final Layout. The Contractor will prepare, in writing, a Layout Comment Response Memo detailing a response to each review comment (i.e. will incorporate, needs further discussion, etc.) and will submit the memo to the State before initiating work on the Final Geometric Layout. Upon the State s concurrence of the Layout Comment Response Memo, the Contractor will incorporate review comments and submit the Final Geometric Layout to the State by the date indicated in Section 14, or as otherwise approved by the State Construction Limits Map The CONTRACTOR will prepare a construction limits map for review and approval by the State. The CONTRACTOR prepare for and attend a Construction Limits hand-off meeting with District staff Deliverables The CONTRACTOR will: 1. Evaluates the existing geometrics at the junction of TH 65 and Garfield Avenue. 2. Provide geometric alternatives. 3. Prepare and submit an electronic spreadsheet and three (3) hardcopies of a preliminary cost estimate for the project. 5. Prepare and submit preliminary ADA design. 6. Prepare and submit a drainage area map with contours in MicroStation.dgn V8i format. 7. Prepare and submit a Drainage Design Report. 8. Prepare and submit Final Geometric Layout. 9. Prepare and submit 3 hard copies and 1 electronic version (PDF) of Final Geometric Layout with profiles and preliminary cross sections for the State approval and signature. 10. Prepare a Construction Limits map and submit 1 hardcopy and 2 electronic copies (PDF, DGN). 11. Attend one meeting to present construction limits to District Staff. 12 The State will: 1. Review the geometric alternatives and construction limits and submit comments. 2. Review the Preliminary Geometric Layouts and provide written comments. 3. Review the hydraulics designs and Drainage Design Report and provide comments. 6. Review, sign, and distribute the Final Geometric Layouts. 8. DESIGN MEMORANDUM (Activity Code 1140) The CONTRACTOR will complete the Design Memorandum prior to beginning 30% Detail Design Plans 8.1. Documentation The CONTRACTOR is responsible for all documentation relating to the Design Memorandum Deliverables State s Deliverables: 1. Review/Approve Final Design Memorandum document. Upon Staff Approval of the final Construction Limits Map, the CONTRACTOR will initiate Final Highway Design. The CONTRACTOR will perform a quality assurance/ quality verification of roadway plans at each plan set phase (30%, 60%, 90% and 100%). 30% Plans Includes the design and coordination to develop the 30% plan set which will include the following plan sheets: 1. Title sheet 2. In place utilities and tabulations 3. Preliminary typical sections 4. In place Topography, Utility and Right of Way (RW) Plans 5. Alignment plans 6. Preliminary Construction Plan 7. Final Construction Plans Final construction plans will be prepared in conformance with the form and content of the following: 1. Proved a Sample Plan 4. Provide plan review comments in written form. 10. HYDRAULIC DESIGN The CONTRACTOR will perform the following tasks as part of TASK 9.0. The CONTRACTOR will cover all activities necessary to analyze and design the hydraulic system within the project area. The plan will also show the appropriate labels, direction of surface flow, and direction of water flow into the drainage structures. The CONTRACTOR will provide the following deliverables: Storm Sewer (Catch Basins) Replacements Design The storm sewer or catch basin replacement design will follow the procedures in State s Drainage Manual and the Standard Specifications for Highway Construction. The storm sewer design will follow the procedure in the STATE s Drainage Manual, Standard Plates Manual and the Standard Specifications for Highway Construction, and Federal Highway Administration HEC No. 22, Urban Drainage Design Scope of Work and Deliverables: Reconstruction of US65 in City of Albert Lea from Newton Avenue to I of 30

15 Manual, September The CONTRACTOR will be required to use GEOPAK Drainage software. Storm sewer will be made of concrete gasketed pipe, and located so that potential future excavation would result in minimal traffic disruption of the mainline of the highways. The CONTRACTOR will be required to use State s spreadsheet format for both the catch basin spacing and the storm sewer pipe computation sheets. The drainage areas will be modeled with consideration of future development and increased runoff associated with development. The CONTRACTOR will develop drainage area maps for each storm drain inlet with pertinent data such as boundaries of the drainage area, runoff coefficients, time of concentration, and land use with design curve number. The drainage plan sheets will document the roadway cross slope and super elevations in accordance with State s standard plan. Water Resource Engineering (WRE) related notes, tabulations, and specifications will be prepared by the CONTRACTOR according to District 6 s sample format plan and will be provided in a format that is easily transferable. Additionally, the CONTRACTOR will design permanent storm water treatment if it is required by the MPCA Construction Storm Water Permit and provide sizing calculations Culvert Design Culvert design will follow the STATE s Drainage Manual, Standard Plates Manual, and the Standard Specifications for Highway Construction and will be designed in accordance with the Federal Highway Administration Hydraulic Design Series No. 5 Hydraulics Design of Highway Culverts, September 1985, including future modifications and new editions thereof, unless otherwise specified. The CONTRACTOR will be required to utilize HydroCAD or XP-SWMMM and HY8 or CulvertMaster software. The CONTRACTOR will prepare drainage plans, culvert profiles and tabulations to be placed in the final construction plans, in accordance to the District 6 sample format. The CONTRACTOR will develop drainage area maps for each culvert with pertinent data such as boundaries of the area, land use with curve numbers, and the time of concentration. 11. STRIPING AND PAVEMENT MARKINGS, SIGNING, TRAFFIC CONTROL, SIGNAL AND LIGHTING (Activity Code 1140) Striping and Pavement Markings Plan The CONTRACTOR will perform the following tasks as part of TASK 9.0: 1. Provide plans showing permanent striping and markings. All markings will be multi-component, poly-preform or thermoplastic. Markings will include centerlines, lane lines, stop lines, signal-ahead messages, turn arrows, crosswalks, and other markings or messages that the geometric design may dictate. 2. Include appropriate detail sheets in the plans that will better define striping and messages and their dimensions and locations. 3. Prepare, and include in the plans, pavement marking layouts for intersections with complex elements. All other markings will be defined by standard details. 4. Prepare quantity tabulations sheet in the plans showing the various pavement marking items, their locations, quantities at each location, and the total quantity of each pavement marking item Signing The CONTRACTOR will perform the following tasks as part of TASK Prepare a conceptual signing layout for the overall project and submit it to the State Project Manager for review and comment prior to developing plan sheets. The concept layout will help ensure uniform signing throughout the project corridor and compliance with the State s signing requirements. The concept layout will depict existing signing and proposed signing. This includes field verifying in-place sign locations, types, and ages. 2. Prepare plans showing required signing for the completed project. Appropriate signs are anticipated by the State to include regulatory signs, warning signs, guide signs, route markers, delineators, and miscellaneous signs. Required sign types may include, but are not limited to, Types C and D signs. Plans will be prepared at a scale of 1" = 100'. Confer with the State regarding legends to be shown on each sign, and design each sign using SignCAD (Version PV 8.24 and Standards Version: SV 3.00 or newer) computer software and the criteria contained in the 2013 version of the MnDOT Traffic Guide Sign Design Manual, including future modifications and new editions thereof, unless otherwise specified. 4. Provide the detailed design of each sign. 5. Prepare data sheets of each sign type that include the number, size, area, and quantity of each sign, the total quantity of each sign type, and appropriate notes. 6. Incorporate sign placement details and structural details into the plans 7. Show on the signing plan existing pertinent signs that are to be relocated, salvaged, or left in place Traffic Control Plan Construction for this project will be done under traffic. Detouring of traffic will not be permitted without approval of the State. The CONTRACTOR will prepare traffic control plans for this project, consistent with the Minnesota Manual of Uniform Traffic Control Devices (MMUTCD), Manual for Temporary Traffic Control Zone Layouts, and State s District 6 practices, including the following details: 1. Confer with State regarding the staging and schedule. 2. Prepare traffic control plans that will correspond with each construction stage. The plans will include notes, symbols and abbreviations, layout plans for each stage showing signs, pavement markings, channelizing devices, traffic barriers, delineators, temporary curbs, attenuators, and pavement marking removals as appropriate. 3. Prepare tabulations that will summarize the markings, signs, and other traffic control devices and their locations. 4. Use typical details when appropriate instead of depicting every device when standard conditions exist and a detail provides adequate information Traffic Signal The CONTRACTOR will perform the following tasks as part of TASK 9.0: 1. Provide signal plans for the new signal at the US 65/Garfield Avenue intersection. Signal plans will be in State plan format, coordinate correct, and will contain all necessary design information including (but not limited to) the following: quantity tabulation, signature block, abbreviations, symbols, details (standard & special), intersection layout(s) and wiring diagram(s), source of power, advance warning flashers, mast arm signing and pavement markings, interconnect, utility plans sheets and For Information Only signal plans. The pay items will follow STATE standards. The STATE will provide the in-place signal plans at the intersection. 2. Coordinate design of any new sources of power required with the appropriate power companies. This includes acquiring a written estimate from the power company for any work needed Lighting The CONTRACTOR will perform the following tasks as part of TASK 9.0: Prepare a preliminary lighting layout for review and approval by the STATE prior to preparing final design sheets. The lighting is located between Newton Ave. and Shell Rock Street within the project limits. Feasibility of continuing circuitry of circuits and reuse of existing feedpoints for these lighting units shall be verified by the CONTRACTOR. The CONTRACTOR will determine and design the lighting feed points and configuration of the lighting circuitry for all lighting within the project limits. Lighting will be spaced for roadway needs Prepare plan sheets showing layout, standard plates that apply to the lighting construction, symbols and abbreviations, quantity tabulations, service cabinets, lighting unit(s) and luminaries(s) detail, plan view including wiring diagram and source(s) of power, and a sheet showing existing utilities. MAINTENANCE OF TRAFFIC (MOT) (Activity Code 1808) Traffic Management Plan (TMP) The CONTRACTOR will prepare a TMP with limited traffic analysis utilizing the State s checklist. A draft TMP will be submitted for the State s review. The State s comments will be incorporated into a final TMP document Meetings The CONTRACTOR will hold up to three (3) meetings with the State and select stakeholders to discuss construction staging and MOT requirements at a time under the discretion of the MnDOT Project Manager. The CONTRACTOR will provide meeting agendas, exhibits, and minutes. Meetings will be held at State District 6. Deliverables The CONTRACTOR will: 2. Prepare and submit a draft TMP for State review. 3. Revise the draft TMP based on State review comments. 4. Submit a final TMP. 5. Hold up to three (3) MOT Meeting. 6. Provide agenda, exhibits, and minutes for the MOT Meetings. The State will: 3. Provide a District TMP template (checklist) for the project. 4. Review the draft TMP and provide comments. 5. Attend MOT Meetings. 6. Coordinate with internal MnDOT staff regarding traffic control and construction staging and assist the CONTRACTOR in resolving issues. 13. SPECIAL PROVISIONS (Activity Code 1250) The CONTRACTOR will be responsible for producing the Division S, Division SS (Signals), Division SL (lighting), and Division ST (signing) Special Provisions. Each provision will contain a description, materials, construction requirements, method of measurement, and basis of payment for each item. Deletions from, and additions to Standard Specifications will be written and included as necessary.
