9 8 General Provisions for The Work 8.1 Governing Regulations 2013 CMS, including Supp Specs **Note: Addendum to modify the body of the scope is forthcoming, eliminating references to supplemental specs that have been rescinded as part of 2013 CMS. Complete list in section 8 of scope. 9

10 Changes to 2013 CMS SS 802 has now been rescinded, Item 603 has been rescinded, drainage work will be under Item 611 now. The SS covering QC/QA concrete have been rescinded, they are now part of the standard concrete items. SS 823 has been rescinded and incorporated into the standard layout stakes spec. 10

11 8 General Provisions for The Work 8.2 Basis of Payment Primary work items are paid as a Lump Sum unless a unit line price item has been established in the Scope of Services The DBT shall be required to furnish the Department with a complete breakdown of lump sum bid items, including materials to be used in the work Misc: Special-Roadway includes any items of work not specifically itemized elsewhere in the scope. Construction Erosion Control shall be paid as per SS 832, at the established unit prices. 11

13 8.2 Basis of Payment Structure Removal Includes all work associated with removal of existing structure PIC Maintaining Traffic Lump Sum Includes all work to maintain traffic within the work limits. This includes maintenance of the existing/proposed pavement once traffic shifts have been placed. Discussion of pay item list, reference handouts. 13

18 AT1 10 Environmental CE Level 4 Categorical Exclusion 10.1 Waterway Permits: 404/401 Have been authorized by the US Army Corps of Engineers and the Ohio Environmental Protection Agency Shall not perform any work in and/or place any fill in streams or wetlands beyond that allowed by the US Army Corps of Engineers and the Ohio Environmental Protection Agency. DBT shall perform work per 404/401 permits including all special conditions of permits 18

21 11 Right of Way (ROW) A temporary R/W easement has been obtained on the north side of Turkey Run to facilitate construction of the driveways. There is one known existing encroachment, a waterline shown running under the forward span of the bridge. Removal of this shall be included in the bid. Notification of the property owner is required one month prior to removal. Owner is planning to install a replacement line via permit, replacing the line is not included in this project, but the contractor may be contacted by the property owner to coordinate the work. 20

22 12 Utilities Section 12.1 lists utilities and contact info. Design should attempt to avoid utility impact wherever practical. DBT is responsible for all utility coordination. This includes coordinating relocation of facilities if required by the plans. 21

28 14 Design and Construction Requirements: Location & 14.4 Roadway Design Follow ODOT L&D Manual DBT shall provide a minimum 6 graded shoulder and 4 treated shoulder All existing guardrail along SR 674 within project limits is to be replaced using proper length of need calculations per L&D Manual Grading requirements shown in standard construction drawings for all end treatments and attenuator locations shall be used 27

29 14 Design and Construction Requirements: Location & 14.4 Driveways Design Property owner on the north side of the bridge has commercial drives accessing both sides of SR 674 DBT shall provide driveways with adequate width and turning radius to admit semi-tractor trailers to both driveways at all times Proposed driveway material shall match that of existing and in no case shall the drives be replaced with a lesser thickness than that of the existing DBT shall not prevent property owner from crossing roadway to access property on other side DBT shall provide mailbox supports per L&D Vol. 1 note R118 for existing mailboxes disturbed by the work 28

30 14 Design and Construction Requirements: Location & Design 14.4 Subgrade Stabilization DBT is responsible for designing and constructing subgrade conforming to CMS 204. Stable subgrade is defined as subgrade where proof rolling results in permanent rutting of ½ inch or less and elastic rebound movement of ½ inch or less. The DBT shall not utilize chemical stabilization in areas where the subgrade soils contain soluble sulfates in concentrations exceeding 3000 ppm as tested according to the TEX-145-E method and as described in Supplement 1120, Mixture Design for Chemically Stabilized Soils 29

31 14 Design and Construction Requirements: Location & 14.5 Drainage Replace ALL: Design Under drains, Catch Basins, Aggregate Drains Aggregate Drains DBT shall provide at 25 spacings as per L&D Vol. 2 DBT shall provide ditch erosion protection mat as minimum treatment for any section of ditch reworked DBT shall be careful to stay within existing right of way for placement of proposed ditch at Left between existing right of way and proposed toe of slope DBT to provide Notice of Intent (NOI) and Best management Practices (BMPs) per current standards

32 14 Design and Construction Requirements: Location & 14.9 Fencing Design Existing fencing is present within expected work limits to prevent livestock from entering roadway DBT shall always maintain positive separation between livestock and roadway All fence within work limits north of Turkey Run shall be considered to be retaining livestock; DBT shall maintain the same number of gates in the fences as are existing at beginning or construction DBT shall contact the property owner prior to replacing fence. Cattle grates to be carefully relocated. 31

Existing structure is not to be used as part of the proposed structure Proposed structure shall increase deck width from face to face of rail Must be designed to facilitate future part-width maintenance of traffic for bridge maintenance Shall cause no rise in the 100 year flood elevation upstream of bridge 33

35 17 Project Schedule Requirements 17 The current edition of CMS 108 shall be met or exceeded. Include review times in the progress schedule. 34

36 18 Plan Submittals and Review Requirements 18.1 Plan Components Plan signatures Refer to section Quality Control DBT Responsibility 18.3 Electronic Submission Requirements: ODOT will provide an ODOT project website to upload the electronic submissions. Submissions greater than 200MB must be submitted in multiple files. DBT shall provide paper copies of all construction plans as per plan distribution table 18.7: Construction Plans 35

37 18 Plan Submittals and Review Requirements 18.5 Major Design Decision Separate submittals for concurrence with major design decisions made after stage 2 Review are required DBT must advise District Project Manager in writing of any additional decisions during the course of the design 18.6 Stage 2 Review Submission The Consultant shall submit Stage 2 detailed design plans as per L&D Design Manual, Volume 3 for review by ODOT The department shall have 21 working days from receipt to review complete submission The department shall provide spreadsheet summarizing all comments and any markups for each submittal and return to DBT 36

38 18 Plan Submittals and Review Requirements 18.7 Construction Plans Electronic submittal. 5 working days review period. Following the receipt of an Accepted or Accepted as Noted electronic submission, the DBT shall provide full size (22 x34 ) and/or half size (11 x17 ) paper prints of each plan submission to the followingparties: Number of half size sets ODOT District 6 Planning and Engineering 4 ODOT Central Office, Division of Construction 1 Management Each affected utility or railroad company 1 Number of full size sets Plan Distribution Addresses See the scope for addresses for each plan recipient. 37
