Application is filled out incorrectly which would cause the County Survey Crew to revisit the site to set additional grade stakes or determine pipe and/or catch basin sizes. Failure of Inspection: Failure of 1 st or interim inspections (e.g. joints not wrapped or sealed; off grade; exposed steel; lacking proper BMPs; blocked ditch; etc.) Failure of Final Inspection: Do not call in for a final inspection until everything is complete. All catch basins must be mortared, grates in place, mitered end section including steel is complete. Permit Expired: Must have final inspection within 60 days of Survey Crew setting line and grade. 2 Procedures to Apply for an Online Sarasota County Culvert or ROW Use Permit Obtain a Culvert or ROW Use Permit Application from the following link: under the Land Development Application Packets category. If you are already a registered user of our online services, login as always using your login and password; otherwise, click on the following link to create an Online Account as the GENERAL PUBLIC. Make sure to create security questions so your password can be recovered if necessary: Click on the following link to sign in to apply for a Culvert or ROW Use Permit: Click on Apply for Permit and follow the prompts to create your submittal and upload your documents. Original ROW Bonds must be submitted to the above address for review and processing, after uploading into the system. Culvert Permit - you will not be able to print your permit card until the fee is paid. Once it shows paid in the system, you can print the permit. Your application will not be processed until the fee has been paid. 3 CULVERT INSTALLATION INFORMATION YOU MUST CALL (941) FOR STAKING AND INSPECTIONS. THE WHITE PERMIT CARD MUST BE MOUNTED ON A STAKE AND POSTED IN PLAIN SIGHT IN THE CENTER OF THE PROPOSED DRIVEWAY INSTALLATION. Failure to post the permit in the appropriate location may result in the delay of your project and a reinspection fee. Pipe size, length (20 minimum for plastic pipe, 24 minimum 32 maximum for concrete pipe) and appropriate elevations will be determined by the Survey Party Chief. Culvert pipe shall be installed, inspected and approved prior to issuance of a Building Permit. Driveways shall be no closer than 25 feet to any intersection as measured from the property lines. In addition, driveways shall be no closer than 25 feet to any stop signs or stop bars. **Site Survey with Driveway Location shall be submitted with Application** FEES: Culvert or swale permit fee: $ Restaking/Resurveying fee: $ Reinspection fee: $ Records fee: $ THIS PERMIT IS VALID FOR 60 DAYS AFTER THE SURVEY CREW SETS THE LINE AND GRADE. SPECIFICATIONS: 1. Reinforced concrete pipe (RCP) joints are to be flexible and watertight under all conditions of service, by using a continuous o-ring rubber gasket or equivalent. All joints are to be covered with a filter fabric jacket per FDOT Roadway Design Standard Index Annular rings and matching bands with two (2) rods and lugs on each section of pipe are also acceptable. 3. Corrugated High Density Polyethylene Pipe (HDPE), coupling bands, fittings and mitered end sections must meet manufacturer's specifications. All joints are to be covered with a filter fabric jacket. Best Management Practices (BMP) (silt screens, hay bales) must be used and maintained until disturbed ditch line has been stabilized with sod. Upon completion of sodding, all BMP s shall be removed. DRIVEWAY CULVERT REQUIREMENTS: 1. A minimum of one catch basin will be required for each lot when placed pipe lengths exceed Culvert construction shall be with SOLID PIPE and the length will be determined by the width of the driveway. 3. Pipe size will be determined by County Staff from the depth of the ditch and the existing pipes upstream and downstream. FormIPS13 - Revised 12/30/15 1

4 4. Where applicable, currently used headwalls shall be removed. 5. Ditch elevations will be provided for the entire lot frontage. It is the responsibility of the permittee to grade and sod the entire ditch from property line to property line to match the elevations provided. 6. When pipe is laid, connected with flexible material and joints are covered with filter fabric jacket, call (941) for an inspection. If the inspection passes, then the pipe may be covered with approved clean fill. 7. Pipes with a diameter of 24 or greater must have concrete mitered end sections and pipes with a diameter of 30 or greater must have grates installed (See FDOT Roadway Design Standards Index 272 and 273). No headwalls shall be permitted in the County right-of-way. FULL FRONT REQUIREMENTS: 1. A minimum of one catch basin will be required for each lot. Additional catch basins may be required depending upon the length of the full front project and existing conditions. A determination will be made by County Staff. 2. If existing driveway culvert is substandard (smaller than 12 x18 or 15 round), it must be replaced to current standards at the time of the full front installation. 3. A steel inspection is required for catch basins. After the floor and lid have been formed and steel installed, call for an inspection prior to pouring. 4. When the box has been complete, the pipe has been covered and the sod installed, you may call for your Final Inspection. 5. For CATCH BASIN construction, refer to FDOT Roadway Design Standard Index 232 with Cast Iron Grates or the attached catch basin detail. ELEVATION INFORMATION: 1. The elevation and off-set information will be either painted with high-visibility paint directly on the roadway, marked on grade stakes or both. 2. Definitions: a. O/S 4' = 4' off-set from grade stakes or control mark to center of pipe or swale. b. C-3' or Cut 3' = Inside bottom of pipe, or flow line of swale must be 3' below the reference line or mark. c. T.O.G. = Top of Grate d. OT = Open Throat e. F/L = Flow Line f. Swale Inv. = Invert for Swale Driveway 3. The dimensions used above are examples.
