The unaltered portions of the CONTRACT DOCUMENTS shall remain in effect. Please acknowledge receipt of this Addendum on your Bid Form and return a signed copy with your Bid Form. The updates and changes are as follows: A. Pre-bid Meeting Minutes: The Meeting Minutes of the pre-bid conference held on July 25, 2018 is attached to this Addendum. B. Request for Information: Requests for Information (RFI s) about this Bid shall be submitted as questions in writing or via to Town of Colma, 1198 El Camino Real, Colma, CA 94014, Attention Abdulkader Hashem, ahashem@colma.ca.gov. All RFI s must be Received by 2:00 P.M., Friday, August 3, C. Business Licenses: A business license is required for the contractor that is awarded the bid. The application can be found on the Town s website at: Please note that there is a $29 fee associated with the application. 1

2 D. Bid Manual Revisions: 1. Bid Schedule Revisions (Pages 15-16): i. Item 4 - Remove Concrete Curb: Change Estimated Quantity from 300 LF to 330 LF. ii. Item 16 Import Fill: Change Estimated Quantity from 100 CY to 110 CY. iii. Item 29 Concrete Curb: Change Estimated Quantity from 96 LF to 130. E. Specification Revisions: 1. Section 109, Part 2 Products, B (Page TS-27). Manufacturing time has been changed as noted below: i. b. Allow 75 days for order delivery from the manufacturer. 2. Section 110, Part 2 Products, C (Pages TS-43 TS-44). Color Mix Composition has been replaced with: i. 50% Royal Blue, and 50% Sky Blue ii. 50% Beige, and 50% Eggshell iii. 50% Pearl, and 50% Light Gray F. Plan Additions: 1. Addition of a Sketch Design for the Playground Protective Surfacing. G. RFI s and Responses: The following RFI s and Responses are provided as matter of information to clarify issues raised by some of the Bidders about the Bid Documents. Item RFI s & Responses Q1. What is the engineer s estimate for the project? A1. The engineer s estimate for Sterling Park Playground Improvement Project is in range of $350,000 to $400,000. Q2. What is the square footage of the playground surface? A2. The are of the playground area (protective surfacing) is approximately 3,600 square feet. Q3. Have any other addendums been issued (By 7/19/2018)? No. However, it is the responsibility of each prospective bidder to check A3. A. Review Bid Documents: Posted on Colma Website. i. Notice Inviting Bids 1. Reviewed the Scope of Work for supplying and installation of playground equipment, fitness apparatus and site furnishing. 2. The playground equipment and fitness apparatus should be manufactured by Columbia Cascade (Sole Source). The Protective Surfacing should be manufactured by ECORE International or approved equal. 3. Bidders must be registered with Department of Industrial Relations (DIR). 4. Contractors/or subcontractors must have minimum 3 years of experience in installing playground equipment with minimum 5 similar projects. Bidder should be Class A Licensed Contractor. 5. Bid Schedule a. RFIs are due August 3, 2018 b. Bid submission date is August 15, Project Schedule a. It is anticipated to start the Project in early September 2018 b. The project must be completed within 98 Calendar days from the date of NTP, no later than December 17, c. Allow 75 day production lead time (plus 3-4 day transit time) for order delivery from Columbia Cascade after receipt of signed order, approved submittals, and one-third deposit check with agreement to terms on balance due. Will add a legend showing the new Sidewalk d. Will provide details on how the guardrail fence will be terminated at North end. 2. Construction Details (Sheet 7 of 17) a. Sidewalk with Concrete Curb Detail - the Protective Surface Thickness should be corrected to minimum 2.5 instead of 3. b. Sidewalk with Concrete Curb Detail - the Permeable Material in should be corrected from 12 to minimum 6. e. A detail for Sidewalk without Curb will be added. 3. New Playground Equipment, Drawing No. P A2 (Sheet 9 of 17) a. Notes/Updated Drawing of Pipeline Equipment (excluding previous notes on bare-post retrofit) will be revised by ParkPacific/Columbia Cascade. Technical Specifications 1. The Concrete Mix Design of the playground equipment footing should be as per the manufacturer s specifications. If not available by Columbia Cascade, it should be as per the Technical Specifications of this bid. 2. The Protective Surfacing should be installed with min thickness 2.5 (2 inch for the base layer and 0.5 for the top layer) based on CFH of 72. However, a total 3 Protective Surfacing can be used for some products as per its CFH requirements, but the permeable material should be maintained at a minimum 6 underneath the surface. B. Open Forum/Q&A/Walk-through 1. Engineer s Estimate Range for the project is in range of 350,000- $400, Building Permit from Town of Colma is required for the construction of the project at no fee. 3. It is apparent that the surface grading for the existing playground area is above 2%. It is recommended to maintain the gradient below 2%. Town will check the grading plans and advise for any revisions in an Addendum. 4. Town will issue the revisions in two Addenda; the First one by Friday, July 27 th and the Second after the RFIs deadline, August 3 rd. Page 2

10 1.05 PRODUCT DELIVERY, STORAGE AND HANDLING A. Play equipment shall be delivered in good condition in original unopened packages with labels intact and unloaded at job site in such a manner that no damage occurs to the product during hauling, handling or unloading at the job site. B. Protect material from adverse weather WARRANTY A. All materials and workmanship of all materials specified in this Section are subject to warranty requirements. Warranty shall apply for a minimum period of one year from the date of the Notice of Completion of the Project. During and at the end of this period, all required repairs and adjustments, including replacement of defective material, shall be made including all repairs to other work made necessary thereby, without additional expense to the Town. B. Play equipment warranty shall be minimum five (5) years from the date of shipment on the entire structure, as per the standard manufacturer s warranty. Complete copy of the manufacturer s warranty documents must be submitted to the Town prior to final acceptance of the project by the Town. PART 2- PRODUCTS 2.1 GENERAL A. The play area layout and design as shown on the Plans are based on products currently available from the manufacturer Columbia Cascade. Products of any other manufacturers will be not be considered. B. Provide play equipment as follows: 1. PipeLine Playground Equipment a. Replacement Parts similar to drawing No. P A2 dated , as shown on the Plans, two standard color CASPAX-7 powder-coated finishes, BROWN Thermoplastic-coated Perforated Steel SofDeks, TAN or YELLOW plastic components. b. Allow 75 days for order delivery from the manufacturer. 2. PipeLine Fitness Cluster Group B (Model No B) a B Group-B Apparatus, standard color CASPAX-7 powder-coated finish, Cedar colored 100% postconsumer HDPE recycled plastic benches, WHITE graphics on BLUE TS-27

11 and lubricating oils, acids and organic solvents. Though not commonly used in water play areas, pool surrounds and similar applications, dissolved minerals and other chemicals (hydrochlorides) may cause surface discoloration over time. This condition, should it occur, is not considered to be a product failure. Contractors should use an aliphatic binder, which does not produce a yellowish shading on the top layer. Consult manufacturer for more information WARRANTY Playground surface shall maintain required impact attenuation characteristics and be guaranteed against defects in workmanship and materials for a period of no less than (5) Five Years from date of completion of work. Typical wear, abuse, or neglect will be excepted. Maintenance requirements must be maintained for duration of warranty period. PART 2 PRODUCTS A. Playground surface shall consist of synthetic materials meeting the requirements of this specification. The surface should be Playpour components manufactured by ECORE International, or approved equal and must be IPEMA certified. B. Playground Surface shall be 2-layer poured in place, seamless system comprised of a base layer of 100% post-consumer recycled SBR rubber & polyurethane binder and a top layer of EPDM rubber and polyurethane binder. The porous system is field- applied in any configuration and dimension to achieve required fall heights BASE LAYER A. Playpour Primer: Polyurethane. B. Playpour Poured In Place Base: Blend (86% rubber & 14% polyurethane) of strand and granular 100% recycled SBR (styrene butadiene rubber) and polyurethane. Based on ASTM F1292 requirements for critical fall height of six (6 ) feet, the base layer thickness should be of minimum 2.0. C. Color: Black 2.02 TOP LAYER A. Playpour Primer: Polyurethane. B. Playpour Poured-In-Place Top: Blend (82% rubber & 18% polyurethane) of recycled EPDM (ethylene propylene diene monomer) and polyurethane. Nominal thickness of 0.5", with a minimum 0.325" and a maximum 0.625". C. Color Mix Composition: 1. 50% Royal Blue, and 50% Sky Blue TS-43

12 PART 3 EXECUTION 2. 50% Beige, and 50% Eggshell 3. 50% Pearl, and 50% Light Gray Comply with the instructions and recommendations of the protective surfacing manufacturer EXAMINATION A. Verification of Site Conditions: Verify that substrate conditions are suitable for installation of the poured in place surfacing. Do not proceed with installation until unsuitable conditions are corrected. B. Drainage: Proper drainage is critical to the longevity of the protective surfacing system. Inadequate drainage will cause premature breakdown of the poured system in affected areas; and void the warranty PREPARATION A. Existing Substrate Preparation: Remove any loose or delaminated material that would be deleterious to application of the new surface. Fill cracks in existing concrete with cementitious patching compound. B. Surface Preparation: Using a brush or short nap roller, apply primer to the substrate perimeter and any adjacent vertical barriers (such as playground equipment) at the rate of 300 ft2/gal INSTALLATION A. Do not proceed with playground surfacing installation until all applicable site work, including substrate preparation, fencing, playground equipment installation, sign installation and other relevant work, has been completed. B. Base Layer Installation: 1. Using screeds and hand trowels, install the base layer at a consistent density of 29 pounds, 1 ounce per cubic foot to the specified thickness (2.0 ). 2. Allow base layer to cure for sufficient time so that indentations are not left in the base layer from applicator foot traffic or equipment. 3. Do not allow foot traffic or use of the base layer surface until it is sufficiently cured. 4. Primer Application: Using a brush or short nap roller, apply primer to the base layer perimeter and any adjacent vertical barriers that will contact the surfacing system at the rate of 300 ft2/gal.
