Birmingham, AL phone fax ADDENDUM #0 POJECT: Alabama State University enovations to Life Sciences Building DATE: September, 207 JOB NO.: ASU # ABC # HAPC #60 This addendum supersedes all conflicting portions of the drawings and specifications. Contractors shall include all omissions, additions, and adjustments of all trades as may be necessary because of each change, substitution, or omission. Such omission, addition or adjustment shall conform to recognized and accepted engineering practice: General. All references to The Director (Technical Staff, of the State Building Commission), shall change to The Owner, unless noted. 2. All references to General Contractor shall change to Contractor. 3. This Project is subject to the requirements of the Davis-Bacon Act. The Contractor must provide all documentation required to confirm compliance with said Act. 4. The Pre-Bid Conference minutes are attached to this Addendum. The information included in them hereby becomes part of the Contract Documents. Specifications. Cover: A. Bid Date / Time / Location: Wednesday, September 6, 207 at 2:00pm CST Alabama State University Procurement Department Physical Plant Building, oom West Fifth Street Montgomery, Alabama Introduction of ASU and HAPC Attendees: Scott Standerfer ASU, Facilities Director, Owner s representative (not present) Arlene Thompson ASU, Director of Procurement Shunquilla Moore ASU Procurement Staff Associate Bruce Herrington Herrington Architects PC (HAPC), Architect Patrick Collins Whitaker & awson, LLC, Mechanical Engineer 3. Eligibility: As per Contract Documents Attendance at the Pre-Bid Conference is not mandatory for bidder eligibility. Must be a licensed General Contractor in the State of Alabama. General Contractor s License (Put license number on outside of Bid Proposal envelope). 4. Bid Preparation / Bid Guaranty / Delivery of Bids: As per Contract Documents. Use enclosed Form of Proposal. No alterations shall be made to bid proposal unless modified by Addendum. Bid Guaranty in the form of Bid Bond or Cashier s Check; 5% of bid amount, not to exceed $0,000. Must use ABC Form C-4 for Bid Bonds. Bid Bond must be executed by a Surety Company duly authorized and qualified to make such bonds in the State of Alabama, payable to Alabama State University. Cashier s check must be drawn on an Alabama bank. Bids to be delivered to Alabama State University Procurement Department (address above) if hand delivered prior to the Bid Date and Time (September 6, 207 at 2:00pm CST). Hand delivery is preferred by the Procurement Department, as this will preclude delays a bid envelope may experience in the ASU mail room. It is the Contractor s responsibility to ensure its Bid is received. eturn Bid Form and Bid Bond with bid submittal. 5. Performance and Payment Bonds: As per General Conditions, Article 38 Performance and Payment Bonds. Performance Bonds, 00% of contract amount (submitted by GC after the Agreement Between Owner and Contractor is awarded).

(submitted by GC after the Agreement Between Owner and Contractor is awarded). 6. Addenda: No Addenda have been issued to date. Minutes of this Pre-Bid Conference and any pertinent items discussed shall be issued in an Addendum following this Pre-Bid Conference. Any further Addenda necessary after Pre-Bid will be uploaded to Alabama Graphics digital plan room. (Mr. Herrington will respond separately to Ms. Thompson s inquiry as to how to gain access to the Alabama Graphics digital plan room.) 7. Pre-Bid equests for Information or Clarification: The last day for questions is Friday September, 207 at 2:00pm (Noon) Central Standard Time. Any questions received after that time will not be considered. All requests for information must be submitted in writing and may be ed to the Architect (attention: Bruce Herrington) at bruce@herringtonarchitects.com. 8. Project NTP/Mobilization: Owner anticipates issuing a Letter of Intent to Contract to the low bidder by September 8, 207. Owner anticipates issuing a Notice to Proceed within 7 days after execution of the Agreement Between Owner and Contractor. Upon the effective date of the Notice to Proceed, the Contractor will have authorized access to and use of the construction areas in the building and will have ninety (90) calendar days to complete the Work. 9. Liquidated Damages: Liquidated Damages will not be assessed for late Substantial Completion of the Work. 0. Builder s isk Coverage: As per the General Conditions of the Contract Contractor furnished. Contractor also responsible for deductibles.. Project Observation / Material Testing / Test and Balance: Testing to be performed as indicated in specifications Coordination of testing. Unless otherwise indicated, Contractor shall give testing agency a minimum of 24-hrs prior notice for activities requiring testing/observation. 2. Project Coordination / Communication: All correspondence between Owner / Contractor / Architect or Architect s consultants shall come through the Architect from Contractor to the Owner. Include Project name Alabama State University enovations to Life Sciences Building and Project numbers (ASU # , HAPC #60) on all project related correspondence. 3. On-Site Personnel: Contractor will at a minimum provide the following on-site staffing for this project: A) Lead Superintendent Submit resume within 24 hours of Letter of Intent for approval. Superintendent must be someone that is authorized to make decisions. Superintendent does not necessarily have to be a non-tool-working superintendent, but he needs to be able to monitor activities while working. 4. Safety: The Contractor shall be responsible for all project safety. Neither the Owner, Architect, nor any other Consultant employed by the afore-mentioned parties will be responsible for the Contractor s safety precautions, means, methods, techniques, sequences, or procedures. Child Labor Law: All Contractors shall adhere to the Child Labor Law. (No workers on the project under the age of 8). 6. Davis-Bacon This project is funded via Title III funding. Therefore, the Contractor will be responsible for adhering to all requirements of the Davis-Bacon Act, including wage rates and reporting requirements. 7. Meetings: Safety Meetings: Contractor shall be responsible for coordination, content, scheduling, etc. of safety meetings with his employees. Owner / Consultant / Contractor (OAC) progress meeting to be held per project requirements - once a month at a minimum. POJECT SPECIFIC EQUIEMENTS: 8. Completion Time for Project: Time for Completion: The Contractor will have ninety (90) calendar days from Date of Commencement indicated in Notice to Proceed to complete the Work. Anticipate Notice to Proceed from the Owner within seven (7) days of the execution of the Agreement Between Owner & Contractor. The Owner anticipates holding a Pre-Construction Conference within 5 days of the execution of the Agreement Between Owner & Contractor. Specific date and time are to be determined. 9. Soil Erosion Control, Dust Control, and BMP: As per Sheet A0.0 and Project Specification Sections Summary and Temporary Facilities and Controls. Contractor is responsible for implementation of soil erosion and dust control measures in compliance with Best Management Practices as defined by the Alabama Department of Environmental Management. 20. Existing Conditions / Constraints: As per Project Specifications Section Summary and Section Execution. Contractor is responsible for verifying existing conditions prior to construction. The building was constructed around ; therefore, the Owner and the Architect anticipate that no asbestos is present in the building. If the Contractor believes it has found asbestoscontaining materials, stop work in the area immediately and contact the Architect. Architect will contact the Owner, who will retain a testing agency. 2. Existing Utilities: As per Sheet A0.0 and Project Specifications- Sections Temporary Facilities and Controls and Execution. Verify location of utilities prior to construction. 22. Parking for Contractors and their Subs and workers: As per Project Specification Section Summary Parking will be allowed on site. Contractor is to obtain Parking Passes from Campus Police, located at 452 Carter Hill oad, Montgomery, Alabama The campus is enclosed by security fencing. As-Built Drawings: As per Project Specifications Section Project ecord Documents. Contractor is required to provide As-Built drawings at the end of the project. 25. Owner Comments All comments or questions were offered during the presentation and discussion of item through Contractor Comments The Contractor asked if there are limitations on the dates and times for working. o After a brief review of the Project Manual, Mr. Herrington stated that limitations on work hours were not stated in the Specifications. o Those present discussed the potential limitation of work days by ASU, including the following: Fridays before home football games Saturdays of home football games Days on which final examinations are scheduled A final determination from ASU was not available by the time/date these minutes needed to be published (as part of Addendum #). The Owner and Contractor will address this topic prior to execution of the Agreement Between Owner and Architect. No other questions or comments were offered other than those discussed elsewhere in these Minutes. 27. Consultant Comments Patrick Collins offered a brief background of the Project, and an overview of the Scope of Work. o He reviewed the drawings with those present, explaining the majority of the Work is HVACcontrols-related. Additionally, parts replacement at two pieces of equipment is included. (It is acceptable for the entire piece of equipment [i.e. the pump] to be replaced; however, the Contractor will need to submit its proposed equipment for review and approval by the Architect and its Consultant.) o There is one Alternate Bid, which is testing and balancing of all HVAC systems. Contractors wishing to access the building during the bid period can contact Scott Standerfer ( ) or his Assistant Sandra Jackson ( ) to gain authorization to enter the building. This concludes the substance of the meeting. If any attendee believes any portion of thiese minutes to be in error, please notify the author in writing.
