6 GENERAL NOTES AND CONDITIONS STORM SEWER 1. ALL EARTHWORK, GRADING AND PAVING SHALL BE PERFORMED IN ACCORDANCE WITH STANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION IN ILLINOIS, STATE OF ILLINOIS DEPARTMENT OF TRANSPORTATION, CURRENT EDITION, AND ALL REVISIONS AND SUPPLEMENTS THERETO, AND THE REQUIREMENTS AND SPECIFICATIONS OF THE CITY OF DEKALB. IN ADDITION, REFERENCE THE PROJECT MANUAL FOR ADDITIONAL SPECIFICATION REQUIREMENTS. WHERE IN CONFLICT, THE WRITTEN SPECIFICATIONS SHALL GOVERN. 2. THE DEKALB SCHOOL DISTRICT MUST BE NOTIFIED AT LEAST TWO (2) WORKING DAYS PRIOR TO THE COMMENCEMENT OR RESUMPTION OF ANY WORK. 3. THE CONTRACTOR SHALL KEEP CAREFUL MEASUREMENTS AND RECORDS OF ALL CONSTRUCTION AND SHALL FURNISH THE OWNER WITH RECORD DRAWINGS UPON COMPLETION OF HIS WORK. 4. THE CONTRACTOR SHALL VERIFY THE LOCATION OF ALL UTILITIES IN THE FIELD PRIOR TO CONSTRUCTION. THIS INCLUDES SANITARY SEWER, WATER MAIN, STORM SEWER, TELEPHONE, ELECTRIC, GAS, AND CABLE TELEVISION, IF ANY. THE J.U.L.I.E. NUMBER IS THE CONTRACTOR SHALL HIRE A PRIVATE LOCATING COMPANY AT HIS EXPENSE TO LOCATE ALL PRIVATE UTILITIES ON SITE. THIS EXPENSE SHALL BE INCLUDED IN THE CONTRACTOR'S BID. 5. ANY EXCESS DIRT OR MATERIALS SHALL BE DISPOSED OF BY THE CONTRACTOR OFFSITE AS DIRECTED BY THE OWNER AT THE CONTRACTOR'S EXPENSE. 6. ALL STRUCTURES, INLETS, PIPES, SWALES AND ROADS MUST BE KEPT CLEAN AND FREE OF DIRT AND DEBRIS AT ALL TIMES. 7. THE CONTRACTOR IS RESPONSIBLE FOR MAINTAINING ADEQUATE SIGNS, BARRICADES, FENCING, TRAFFIC CONTROL DEVICES AND MEASURES, AND ALL OTHER MEASURES THAT ARE NECESSARY TO PROTECT THE SAFETY OF THE SITE AT ALL TIMES. ALL TRAFFIC AND PEDESTRIAN CONTROL MUST BE MAINTAINED AT ALL TIMES IN ACCORDANCE WITH CURRENT MUTCD AND STATE ILLINOIS STANDARDS. 8. THE CONTRACTOR, BY AGREEING TO PERFORM THE WORK, AGREES TO INDEMNIFY AND HOLD HARMLESS THE OWNER, THE ENGINEER, THE CITY OF DEKALB, AND ALL AGENTS AND ASSIGNS OF THOSE PARTIES, FROM ALL SUITS AND CLAIMS ARISING OUT OF THE PERFORMANCE OF SAID WORK, AND FURTHER AGREES TO DEFEND OR OTHERWISE PAY ALL LEGAL FEES ARISING OUT OF THE DEFENSE OF SAID PARTIES. 9. ALL ELEVATIONS ARE USGS DATUM. 10. ANY FIELD TILES ENCOUNTERED DURING CONSTRUCTION SHALL BE RECORDED SHOWING SIZE, LOCATION, AND DEPTH BY THE CONTRACTOR, AND EITHER RECONNECTED AND REROUTED OR CONNECTED TO THE STORM SEWER SYSTEM. THE OWNER SHALL BE NOTIFIED IMMEDIATELY UPON ENCOUNTERING ANY TILE. 11. THE CONTRACTOR SHALL FIELD VERIFY THE ELEVATIONS OF THE BENCHMARKS PRIOR TO COMMENCING WORK. THE CONTRACTOR SHALL ALSO FIELD VERIFY THE LOCATION AND ELEVATION OF EXISTING PIPE INVERTS, CURB OR PAVEMENT WHERE ING INTO EXISTING WORK. THE CONTRACTOR SHALL FIELD VERIFY HORIZONTAL CONTROL BY REFERENCING PROPERTY CORNERS TO KNOWN PROPERTY LINES. NOTIFY THE ENGINEER OF DISCREPANCIES IN EITHER VERTICAL OR HORIZONTAL CONTROL PRIOR TO PROCEEDING. 12. PROPERTY CORNERS SHALL BE CAREFULLY PROTECTED UNTIL THEY HAVE BEEN REFERENCED BY A PROFESSIONAL LAND SURVEYOR. 13. CONTRACTOR SHALL USE THE OWNER'S ENGINEER, ARC DESIGN RESOURCES FOR CONSTRUCTION LAYOUT SERVICES AND SHALL CONTACT ARC DESIGN RESOURCES DIRECTLY TO NEGOTIATE REQUIRED SCOPE OF SERVICES AND FEE. CONTRACTOR SHALL INCLUDE ALL NECESSARY CONSTRUCTION LAYOUT IN HIS BID. 14. OWNER WILL HIRE AND PAY FOR THE COST OF AN INDEPENDENT TESTING AGENCY TO TEST THE COMPACTION OF GRANULAR BASE, CONCRETE AND ASPHALT PAVEMENT. 15. THE CONTRACTOR SHALL INCLUDE A TOTAL OF 200 CUBIC YARDS OF UNDERCUT FOR REMOVAL OF UNSUITABLE SOIL AND REPLACEMENT WITH COMPACTED GRANULAR FILL IN THEIR BASE BID. A UNIT COST SHALL BE PROVIDED FOR THIS WORK. ACTUAL FIELD MEASUREMENTS SHALL BE USED TO ADJUST THE FINAL PAYMENT DUE TO THE CONTRACTOR, WHETHER LESS OR MORE THAN THE NOMINAL AMOUNT INCLUDED IN THE BID PRICE. DEMOLITION NOTES 1. DISPOSAL OF ALL MATERIALS SHALL COMPLY WITH ALL STATE, LOCAL AND FEDERAL REGULATIONS. ALL WASTE MATERIAL SHALL BE DISPOSED OF OFF SITE. CONTRACTOR SHALL BE RESPONSIBLE FOR REMOVAL OF ALL MATERIALS FROM THE SITE ALONG WITH ALL ASSOCIATED PERMITS AND REGULATORY REQUIREMENTS. 2. THE CONTRACTOR SHALL MAINTAIN ALL EXISTING UTILITY SERVICES TO ADJACENT LOTS. INTERRUPTION OF SERVICES TO ADJACENT LOTS SHALL NOT OCCUR WITHOUT PROPER APPROVAL. 48 HRS NOTICE SHALL BE GIVEN TO THE PROPERTY OWNERS PRIOR TO THE CONNECTION OF THE NEW SERVICES. 3. THE CONTRACTOR SHALL MAINTAIN ALL ADJACENT EXISTING PARKING, SIDEWALKS, DRIVES, ETC. TO BE CLEAR AND FREE OF ANY CONSTRUCTION ACTIVITY AND/OR EXCAVATED AND HAULED MATERIAL TO ENSURE EASY AND SAFE PEDESTRIAN AND VEHICULAR TRAFFIC TO AND FROM ADJACENT SITES. 4. THE CONTRACTOR SHALL PERFORM A FULL-DEPTH SAW CUT ALONG THE PERIMETER OF PAVEMENT REMOVAL THAT ABUTS EXISTING PAVEMENT THAT IS TO REMAIN. 5. ANY DAMAGE SUSTAINED BY ITEMS THAT ARE TO REMAIN IN PLACE SHALL BE REPAIRED OR REPLACED TO THE OWNER'S SATISFACTION AT NO COST TO THE OWNER. 6. SIDEWALK REMOVALS SHALL EXTEND PAST THE NOTED LIMITS TO THE NEAREST JOINT. DO NOT CUT A HALF PANEL OF SIDEWALK. CONTRACTOR TO MAKE ADJUSTMENT IN FIELD. 1. ALL STORM SEWER PIPE GENERICALLY SHOWN CAN BE OF ANY OF THE FOLLOWING: REINFORCED CONCRETE CULVERT PIPE CLASS IV, PVC SDR 35, HDPE DOUBLE WALL (ADS N-12), OR PVC SCHEDULE 40 AS A NO COST APPROVED ALTERNATE. IF SHOWN ON THE PLANS AS A SPECIFIC MATERIAL TYPE, CONTRACTOR SHALL USE THAT TYPE. 2. ALL STORM SEWERS UNDER AND WITHIN TWO FEET OF ANY EXISTING OR PROPOSED PAVEMENT OR SIDEWALK SHALL BE BACKFILLED WITH GRANULAR BACKFILL MATERIAL IDOT GRADATION FA-6 OR APPROVED EQUAL. (GRADE 8 OR GRADE 9). 3. ANY FIELD TILES ENCOUNTERED SHALL BE RECONNECTED OR CONNECTED TO THE STORM SEWER SYSTEM. THE SIZE, LOCATION, AND DEPTH OF THE TILES SHALL BE RECORDED BY THE CONTRACTOR AND IMMEDIATELY REPORTED TO THE OWNER, CITY, AND THE ENGINEER EARTHWORK, GRADING AND PAVING ALL PROPOSED SIDEWALK AREAS SHALL BE STRIPPED OF ALL TOPSOIL AND UNSUITABLE MATERIAL AND EXCAVATED OR FILLED TO WITHIN 0.10 FEET OF DESIGN SUBGRADE. 2. THE SUBGRADE OF PAVEMENT AREAS SHALL BE FREE OF ALL UNSUITABLE MATERIAL AND SHALL BE COMPACTED TO A MINIMUM 95 PERCENT OF STANDARD PROCTOR DENSITY. 3. THE SUBGRADE SHALL BE PROOF ROLLED, INSPECTED AND APPROVED PRIOR TO PLACING THE BASE MATERIAL. 4. THE CONTRACTOR SHALL BE RESPONSIBLE FOR REMOVAL OF SPOIL MATERIAL FROM THE UNDERGROUND CONTRACTORS, BACKFILLING AND COMPACTING SOIL BEHIND THE CURB AND GUTTER, PREPARING THE ROADWAY SUBGRADE, PROOF ROLLING, PLACING TOPSOIL TO A MINIMUM DEPTH OF 4 INCHES TO FINISHED GRADE IN THE PARKWAYS AND ALL OTHER TASKS AS DIRECTED BY THE OWNER OR ENGINEER. 5. THE CONTRACTOR IS RESPONSIBLE FOR MAINTAINING POSITIVE DRAINAGE AT THE CONCLUSION OF EACH WORKING DAY. 6. THE CONTRACTOR IS RESPONSIBLE FOR THE FINAL SUBGRADE PREPARATION, PROOF ROLLING, THE PAVEMENT BASE, BINDER, AND SURFACE, AND ALL FINAL CLEAN-UP AND RELATED WORK ASSOCIATED WITH THE PAVING OPERATION. 7. CURING AND PROTECTION OF ALL CONCRETE SHALL BE IN STRICT CONFORMANCE WITH THE PROVISIONS OF SECTION OF STANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION IN ILLINOIS. SOIL EROSION AND SEDIMENT CONTROL 1. TOTAL AREA OF BARE SOIL DISTURBED IS LESS THAN 1 ACRE SITE SO THE NPDES GENERAL PERMIT IS NOT REQUIRED. HOWEVER THE CONTRACTOR SHALL FOLLOW THE NPDES GENERAL PERMIT AS RECOMMENDED PRACTICE. 2. ANY DISTURBED AREAS SHALL BE KEPT TO A PRACTICAL MINIMUM. 3. ALL INLETS, STRUCTURES, PIPES, SWALES, AND ROADS SHALL BE KEPT CLEAN AND FREE OF DIRT AND SILT. 4. STOCKPILES SHALL HAVE A BERM OR TRENCH AROUND THE CIRCUMFERENCE TO CONTROL SILT IF NEEDED. IF THE STOCKPILE IS TO REMAIN FOR MORE THAN TWO WEEKS, IT MUST HAVE SUFFICIENT VEGETATION TO CONTROL BOTH WATER AND WIND EROSION. 5. STRAW BALES FIRMLY ANCHORED SHALL BE PLACED AROUND ALL INLETS, CATCH BASINS, AND SWALES THAT RECEIVE SILTY RUNOFF DURING THE COURSE OF CONSTRUCTION. 6. ALL SOIL EROSION CONTROL MEASURES MUST MEET THE REQUIREMENTS OF THE CITY OF DEKALB. 7. THE CONTRACTOR SHALL PLACE STRAW WADDLES AROUND ALL PROPOSED DRAINAGE STRUCTURES WITH OPEN LIDS (CATCH BASINS AND INLETS) THROUGHOUT THE ENTIRE SITE. 8. ALL EARTH STOCKPILES SHALL BE SURROUNDED WITH SILT FENCE. 9. EXCESS TOPSOIL AND EXCAVATED MATERIAL SHALL BE REMOVED FROM THE SITE. CONSTRUCT TEMPORARY SITE ACCESS POINTS USING AN AGGREGATE SURFACE. DO NOT TRACK OR SPILL DIRT CLUMPS ONTO ADJACENT ROADWAYS. IF DIRT IS DEPOSITED ON PUBLIC PAVEMENTS, REMOVE THE DIRT AND CLEAN THE PAVEMENT. 10. ALL DISTURBED AREAS SHALL BE RESTORED PER THE SOD RESTORATION DETAIL SHOWN ON THE PLANS. RESTORATION 1. UNLESS SPECIFIED ON THE DRAWINGS, ALL DISTURBED AREAS SHALL BE RESTORED WITH A MINIMUM 4" IMPORTED TOPSOIL, SEEDING IDOT CLASS 1 LAWN MIX, AND COVERED WITH EROSION CONTROL BLANKET NAG S150 OR BETTER. 2.
