3 UNDERWATER INSPECTION REPORT SUMMARY The substructure unit inspected at Bridge No , Pier 2, was found to be generally in satisfactory condition below water, although with significant foundation exposure/undermining. A scour depression approximately 3 feet deep was detected at the upstream nose of Pier 2. The pier footing was exposed around the entire perimeter with undermining and nine exposed timber piles noted across the upstream and east sides of the pier. Moderate to heavy timber debris accumulation was observed along the west side of Pier 2 extending from the pier to the shore. The channel bottom across most of the waterway appeared stable with no appreciable changes since the previous inspection except at the pier. The extent of foundation exposure has increased at Pier 2 from what was noted during the 2012 inspection and height of undermining has significantly increased from a maximum of 4 inches in 2012 to a maximum of 2 feet currently. It should be noted that Pier 3 is normally in the dry and part of the above water inspection, and was only partially located in the water during this inspection due to temporarily elevated water levels. INSPECTION FINDINGS 1. A light accumulation of timber debris consisting of 2 inch diameter and smaller branches was observed at the upstream nose of Pier 2 at waterline. 2. The channel bottom material around most of Pier 2 consisted of fine sandy gravel allowing up to 2 inches of maximum probe rod penetration. 3. Two horizontal impact related voids were observed on the upstream nose of Pier 2 above the waterline, each measuring 2- foot-long by 1-inch-wide with 1/2 inch of penetration and exposed aggregate. 4. Light scaling of the pier concrete was observed from the top of the footing to 4 feet above the waterline, with 1/4 inch maximum penetration. 5. The footing was exposed along the entire perimeter of Pier 2. Undermining was observed along the east face of the pier from the center of the upstream nose to the midpoint of the east face, with a maximum height of undermining of 2 feet located at the southeast corner of the footing. Horizontal penetration of the undermining (generally up to 5 feet) encountered two rows (9 total) of exposed timber piles. Full footing height of 3.5 feet was exposed at the midpoint of the east face of the pier with no undermining observed. 6. A 5-foot-radius, 3-foot-deep scour depression was observed at the upstream end of Pier The channel bottom at the downstream end of Pier 2 consisted of sandy infilling allowing 3 inch probe rod penetration. 8. A moderate accumulation of timber debris, with pieces up to 1.5 feet in diameter, was observed within the scour depression at the upstream end and along the east face of the pier, extending from the channel bottom up 3 feet. 9. A moderate to heavy accumulation of timber debris, with pieces 2 foot diameter and smaller and with a large tree and rootball, extended from the channel bottom to 6 feet above the waterline along the west face of Pier 2 from the downstream nose to the upstream quarter-point and approximately 25 feet out from the face of the pier to the shoreline. 10. Minor accumulation of timber debris, 6 inches diameter and smaller, at the upstream end of Pier 3 extended from the channel bottom to 2 feet above the waterline. RECOMMENDATIONS (A) Monitor the scour depression and footing exposure at Pier 2 during future inspections. If these conditions progress, scour countermeasures may need to be considered. The piers are numbered 1 through 3 starting from the west. 2. 6 UNDERWATER INSPECTION INSPECTION PROCEDURES The routine underwater inspection of Bridge (CSAH 25 over Root River) was completed on October 21, The underwater inspection was conducted from the shore. The inspection was conducted by a team consisting of a Professional Engineer Diver with a valid MnDOT Team Leader certification, a backup diver and dive tender. The inspection utilized commercial dive equipment and techniques (SSA and/or SCUBA) in accordance with OSHA regulations. Profiles were taken along the upstream and downstream faces of the bridge and around the periphery of substructure units to determine the presence, location, and area of scour. The bridge element inspected consisted of one reinforced concrete pier (Pier 2). According to design drawings, the inspected substructure unit is founded on a rectangular reinforced concrete footing supported by timber piles. Inspection procedures followed FHWA guidance and the MnDOT Bridge and Structure Inspection Program Manual with channel bottom probing to search for foundations. The maximum routine underwater inspection frequency is recommended to remain at 60 months based on those findings and risk factors. Also, inspection procedures should continue to follow the above approach and standard guidance with 100% Level I and 10% Level II intensity efforts. Page 6 of 19
