3 UNDERWATER INSPECTION REPORT SUMMARY The substructure units inspected at Bridge No , Piers 1 and 2, were found to be in good to satisfactory condition with no defects of structural significance below water. The concrete surfaces of the piers exhibited light scaling around the waterline and some minor vertical cracks on the pier shafts. A moderate to heavy accumulation of 2 feet diameter and smaller timber debris was observed at the upstream nose and along the east face of Pier 1. A moderate accumulation of timber debris was observed at the upstream nose of Pier 2 and extended approximately 40 feet upstream. The footing was exposed at Pier 2 along the east face and at the upstream and downstream noses. Along the east face the maximum vertical exposure was 1.5 feet with undermining and five exposed steel H-piles. The exposed piles were in good condition with minimal deterioration. At Pier 1 the footing was exposed on the west face from the upstream nose to the downstream nose with a maximum vertical exposure of 6 inches and no undermining. The channel bottom configuration has experienced some notable changes (mostly degradation) since the last inspection, but there has been only a minor increase in the extent of undermining at Pier 2. INSPECTION FINDINGS 1. The channel bottom at upstream end of Pier 1 consisted of gravel and silt allowing up to 2 inches of probe rod penetration. 2. The channel bottom consisted of silty sand allowing up to 1 foot of probe rod penetration at the downstream end of both piers. 3. The channel bottom at the upstream end of Pier 2 consisted of soft silt allowing up to 1.5 feet of probe rod penetration. 4. A vertical hairline crack was observed at the midpoint of Pier 2 that extended from the top of the pier cap to 2 feet below the waterline. 5. A vertical hairline crack was observed at the midpoint of Pier 2 that extended from the top of the pier cap to the top of the footing. 6. Light scaling was observed along all pier faces from 1 foot above to 1 foot below the waterline with a maximum penetration of 1/16 inch. 7. Moderate to heavy accumulation of timber debris, with pieces 2 foot diameter and smaller, from upstream quarter-point along the east face of Pier 1 to the upstream nose and extending from the channel bottom to 3 feet above the waterline and 10 feet out from the pier face and nose. 8. Moderate accumulation of timber debris consisting of a 2 foot diameter tree trunk forked around the upstream nose of Pier 2 and extending approximately 40 feet upstream with associated 1 foot diameter and smaller branches surrounding the trunk. 9. The footing was exposed along the east face of Pier 2 with undermining and exposed steel H-piles observed from the midpoint of the downstream nose, up the east side and around the upstream southeast corner, and extending to the upstream southwest corner, with a maximum height of approximately 1.5 feet along most of the undermined area on the east side of the pier, and 4 to 5 feet of maximum horizontal penetration. The exposed steel H-piles (5 total) within the undermining cavity were sound without any notable deterioration (just spotty surface corrosion). 10. The footing was exposed along the west face of Pier 1 between the upstream and the downstream noses with maximum vertical exposure of 6 inches along the upstream quarter of the footing. No vertical exposure was observed from the midpoint to the downstream nose of the pier and no undermining of the footing was present. RECOMMENDATIONS (A) Remove timber debris accumulations at Piers 1 and 2. Removal of the timber debris will reduce excessive lateral loads on the pier, limit further debris accumulation, and reduce the likelihood of channel bottom degradation resulting from obstructed flow. Until the timber debris can be removed, monitor for any significant increase in debris accumulation and/or scour at the bridge. The superstructure is supported by two reinforced concrete abutments and two reinforced concrete piers. Both the abutments and piers are supported by footings on steel H-piles. The piers are numbered 1 and 2 starting from the south end of the bridge. 2. 6 UNDERWATER INSPECTION INSPECTION PROCEDURES The routine underwater inspection of Bridge 23501(CSAH 5 over Root River) was completed on October 22, The underwater inspection was conducted from the shore. The inspection was conducted by a team consisting of a Professional Engineer Diver with a valid MnDOT Team Leader certification, a backup diver and dive tender. The inspection utilized commercial dive equipment and techniques (SSA and/or SCUBA) in accordance with OSHA regulations. Profiles were taken along the upstream and downstream faces of the bridge and around the periphery of substructure units to determine the presence, location, and area of scour. The bridge elements inspected consisted of two reinforced concrete piers. According to design drawings, the inspected substructure units are founded on rectangular concrete footings supported by steel H-piles and are designated as Piers 1 and 2 starting from the south. Inspection procedures followed FHWA guidance and the MnDOT Bridge and Structure Inspection Program Manual with channel bottom probing to search for foundations. The maximum routine underwater inspection frequency is recommended to remain at 60 months based on those findings and risk factors. Also, inspection procedures should continue to follow the above approach and standard guidance with 100% Level I and 10% Level II intensity efforts. Page 6 of 19
