2 SECTION GENERAL ADDENDUM The following sets forth the format for issued Addenda. ADDENDUM NO. 1, dated June 8, 2017 RE: FROM: LINCOLN MIDDLE SCHOOL ATHLETIC FIELD PROJECT AT LINCOLN MIDDLE SCHOOL located at 1501 CALIFORNIA AVENUE, SANTA MONICA, CA DSA App. No.: OWNER: SANTA MONICA_MALIBU UNIFIED SCHOOL DISTRICT 1651 Sixteenth Street Santa Monica, California TO: ALL PROSPECTIVE BIDDERS: This Addendum forms a part of the Contract Documents and modifies the Bidding Documents dated as noted below. Acknowledge receipt of the Addendum in the space provided on Document Bid Proposal. Failure to do so may result in the bid being deemed non-responsive. 3 E. CHANGES TO SPECIFICATIONS 1. Replace entire section Phasing of the Work with the attached Phasing of the Work. 2. Replace entire section Allowances with the attached Allowances. 3. Delete section Unit Prices (2pp). This section does not apply. 4. Delete section Stormwater Pollution Control Measures for Construction Activities. F. CHANGES TO DRAWINGS None G. OTHER CHANGES AS SET FORTH a. The Contractor will not be permitted to place cranes or other heavy equipment on installed track and field surfaces. b. The existing driveway on the north side of Building F is a fire lane. The Contractor must maintain this fire lane access at all times. c. The Contractor s work area will be restricted as follows (per attached staging exhibits): i. Staging Exhibit 1 - From the NTP to August 18, ii. Staging Exhibit 2 From August 19, 2017 to the completion of the project. d. The contactor shall be responsible to coordinate delivery, receive, unload, store, protect and secure Musco poles and equipment, and complete the work per contract documents. H. Clarification or any other notice of a change in the Bidding Documents will be issued only by the OWNER office of Facilities Construction Contracts and only in the form of a written Addendum, transmitted by fax or to all who are known by the issuing office to have received a complete set of Bidding Documents. Any other purported Addenda are void and unenforceable. 1. Question: Per the new bid documents, the district has added an allowance of $100,000. The spec section (Allowances) It states other allowances not provided in the bid documents. Does the $100,000 cover these items as stated in spec section (Allowances)? If not, how should we include this in our bid? Answer: "Allowances" in the Bid documents shall be replaced with a new "Allowances" section in an Addendum. 2. Spec Section Unit Prices require that the General Contractor quote unit prices for : 1) Unit Price No. 1:Removal of unsatisfactory soil and replacement with satisfactory soil material; 2) Unit Price No. 2: Rock excavation and replacement with satisfactory soil material; 3) Unit Price No. 3: Cutting and LINCOLN MIDDLE SCHOOL / ATHLETIC FIELD PROJECT ADDENDUM NO /08/2017

4 patching of concrete floor slabs; 4) Unit Price No. 5: Pole caisson hole rock. There does not appear to be any location for these unit prices to be quoted or published on the current Bid Form. Is the District issuing a revised bid form with unit price requirements? If not, how are these unit prices to be qouted? There also is an allowance requirement for Testing and Inspections, but only the Contingency Allowance is shown on the Bid Form. Is the District going to add the Testing Allowance to the Bid Form? Answer: Delete section Unit Prices (2pp). This section does not apply. Bid form shall remain as-is. 5 SECTION PHASING OF THE WORK PART 1 - GENERAL 1.1 SECTION INCLUDES A. Requirements for phasing of Work include logistics, phasing, and completion of designated phases prior to commencement of subsequent phases. 1.2 RELATED SECTIONS A. Section Summary of Work B. Section Closeout Procedures PART 2 - PRODUCTS (Not used) PART 3 - EXECUTION 3.1 SUBMITTALS A. Contractor shall submit a Project site logistics plans in accordance with and as required by this Section. * Milestone date to be determined by CONTRACTOR during development of the Construction Schedule and submitted to the District Representative for approval. 6 3.2 LOGISTICS A. Prior to commencement of Work, Contractor shall prepare and submit to District Representative, a detailed Project site logistics plan, at the same size and scale of Drawings, setting forth Contractor s plan of Work relative to following, but not limited to, items: 1 Hauling route in accordance with local ordinances for truck access routes to and from Project site. 2. The identification of any overhead wire restrictions for power, street lighting, signal or cable. 3. Local sidewalk access and street closure requirements. 4. Protection of sidewalk pedestrians and vehicular traffic. 5. Project site fencing and access gate locations. 6. Construction parking. 7. Material staging or delivery areas. 8. Material storage areas. 9. Temporary trailer locations. 10. Temporary service location and proposed routing of all temporary utilities. 11. Location of temporary or accessible fire protection 12. Trash removal and location of dumpsters. 13. Concrete pumping locations. 14. Crane locations. 15. Location of portable sanitary facilities. 16. Mixer truck wash out locations. 17. Traffic control signage. 18. Perimeter and site lighting. 19. Storm Water Pollution Prevention Plan SWPPP * Milestone date to be determined by CONTRACTOR during development of the Construction Schedule and submitted to the District Representative for approval. 7 20. Stockpile or lay down areas. 21. Security lighting 22. The Contractor shall prepare a detailed plan addressing access to school facilities as a part of mobilization activities. Accessible paths shall be maintained to all of school facilities during regular school hours. The District is especially concerned about how students will access Building F and Building J and how students will access safe disbursement areas. These items shall be specifically addressed on the logistics plan during mobilization. B. Revised Project site logistic plan may be required by District Representative for separately identified phases of Work as set forth in this Section. C. Contractor is responsible for securing and/or obtaining all approvals and permits from authorities having jurisdiction relative to any activities set forth in Section 3.2, A D. Contractor shall review the Phasing, Lay Down, and Temporary Control Plans provided in the Drawings 3.3 PHASING OF THE WORK A. Project will be constructed in separate Milestone increments, as identified or as described in this Section or Contract Documents. Phasing will also delineate Work to be completed in each designated phase. Unless otherwise approved or directed by District, each phase shall be completed according to approved Baseline Schedule prior to commencement of next subsequent phase. Contractor shall incorporate and coordinate Work of Separate Work Contracts relative to this Project into the Phasing and Construction Schedule. B. Contractor shall install all necessary Work for, but not limited to, power, lighting, signal, HVAC, drainage, and plumbing systems in phased Work before completion of designated phase. All valves, pull boxes, stub outs, temporary capping, and other Work necessary for phased completion and operation of all necessary systems shall be provided whether or not such Work is specifically identified in Contract Documents. 3.4 PHASING OF THE WORK GENERAL A. Contractor shall prepare Construction Schedule in order to complete Work and related activities in accordance with phasing plan as established in section 3.6 Milestone schedule. Contractor shall include all costs to complete all Work within Milestones or Contract Time. B. District will be seriously damaged by not having all Work completed within Milestones or Contract Time. It is mandatory Work be complete within Milestones or Contract Time. * Milestone date to be determined by CONTRACTOR during development of the Construction Schedule and submitted to the District Representative for approval. 8 3.5 PHASING OF THE WORK SPECIFIC A. Contractor shall prepare Construction Schedule, and shall complete following, but not limited to Milestones, as shown in Section Milestones and within designated phases in accordance with following: 1. Phase 1 Mobilization 12 calendar days from NTP: Milestones 2 & Phase 2 Construction Substantial Completion 144 calendar days from NTP: Milestone, 5, 6, 7, & Phase 3 Closeout 30 calendar days from Substantial Completion: Milestone 9. B. The Contract Time shall be a total of 174 calendar days from date of commencement of Contract Time. 3.6 MILESTONES A. CONTRACTOR shall commence performance of the Contract upon the date specified in the Notice to Proceed and shall furnish sufficient forces, facilities and materials, work such hours, including extra shifts and overtime operations, so as to fully perform the Work in accordance with the following Milestones. Milestone Description Schedule No. 1 No. 2 Notice to Proceed: Is established in accordance with Articles 1.18 and 2.2 of the General Conditions. Completion of Mobilization activities: Is defined as CONTRACTOR mobilization completed, logistics plan submittal and approval (access, gates, parking, trailer locations, signage), site fencing complete, temporary offices complete (trailers- fully functional with furniture, office equipment, network connection and office supplies., storage bins, dumpsters), temporary Utilities complete (water, electric, phone, sanitation, fire protection), Project signage complete, obtain all required permits, As determined in the NTP. 12 calendar days after NTP. Site ready for field turf and track surface installing contractor work: Shall be separately listed for each area and is defined as all underground utilities (mechanical, electrical, and plumbing) installed from the point of supply to five feet outside the building line, inspected by the IOR, earthwork and grading completed and surface facilities as required. Musco Field Light System: System complete and operational under its permanent controls and power supply. Substantial Completion: Is established in accordance with Article of the General Conditions. 12 SECTION ALLOWANCES PART 1 - GENERAL 1.1 RELATED DOCUMENTS A. Drawings and general provisions of the Contract, including General and Supplementary Conditions and other Division 01 Specification Sections, apply to this Section. 1.2 SUMMARY A. Section includes administrative and procedural requirements governing allowances. 1. Certain items are specified in the Contract Documents by allowances. Allowances have been established in lieu of additional requirements and to defer selection of actual materials and equipment to a later date when direction will be provided to the Contractor. If necessary, additional requirements will be issued by Change Order. B. Types of allowances include the following: 1. Contingency allowances. 1.3 SUBMITTALS A. Submit proposals for labor and equipment or purchase of products or systems included in allowances, in the form specified for Change Order requests. B. Submit invoices or delivery slips to show actual quantities of materials delivered to the site for use in fulfillment of each allowance. C. Submit time sheets and other documentation to show labor time and cost for installation of allowance items that include installation as part of the allowance. D. Coordinate and process submittals for allowance items in same manner as for other portions of the Work. 1.4 COORDINATION A. Coordinate allowance items with other portions of the Work. Furnish templates as required to coordinate installation. 1.5 CONTINGENCY ALLOWANCES A. Use the contingency allowance only as directed by Architect for Owner's purposes and only by approved allowance requests that indicate amounts to be charged to the allowance. 13 B. Contractor s overhead, profit, and related costs for products and equipment ordered by Owner under the contingency allowance are included in the allowance and are not part of the Contract Sum. These costs include delivery, installation, taxes, insurance, equipment rental, and similar costs. C. Authorization of use of funds from the contingency allowance will include Contractor's related costs and reasonable overhead and profit margins. D. At Project closeout, credit unused amounts remaining in the contingency allowance to Owner by Change Order. 1.6 ADJUSTMENT OF ALLOWANCES A. Allowance Adjustment: To adjust allowance amounts, prepare a Change Order proposal based on the difference between purchase amount and the allowance, multiplied by final measurement of work-in-place where applicable. If applicable, include reasonable allowances for cutting losses, tolerances, mixing wastes, normal product imperfections, and similar margins. 1. Owner reserves the right to establish the quantity of work-in-place by independent quantity survey, measure, or count. PART 2 - PRODUCTS (Not Used) PART 3 - EXECUTION 3.1 EXAMINATION A. Examine products covered by an allowance promptly on delivery for damage or defects. Return damaged or defective products to manufacturer for replacement. 3.2 PREPARATION A. Coordinate materials and their installation for each allowance with related materials and installations to ensure that each allowance item is completely integrated and interfaced with related work. 3.3 SCHEDULE OF ALLOWANCES A.
