3 EXECUTIVE SUMMARY A limited asbestos inspection and assessment was conducted of the Church of Scientology building located at 709 E. South Temple, Salt Lake City, Utah on April 28, 2015, by Joelle Dickson and Ken Larsen of SAGE Environmental, L.L.C. (SAGE). The building is an approximately 41,000 square foot, two story plus basement office building built in 1976 and remodeled in Based on the architect s description of the planned renovations, the inspection was limited to floor coverings, mastics and leveling compounds; cove base; pipe insulation; and ceiling finishes on all three floors of the building. One sample of gypsum board was also collected on the main floor where a wall will be removed. The purpose of the inspection was to identify potential asbestos containing materials (ACM) prior to renovation of the building. Samples of suspect ACM were collected and submitted for laboratory analysis using polarized light microscopy (PLM). ACM was identified in several materials in the building. These materials are included in Table ES-1 below and in the following checklists for regulated and non-regulated materials. Note that floor tile samples CS-FT-01 and CS-FT-02 are the same tile; the FT-02 sample had two types of mastic and so was sampled separately. Since all of the black mastic sampled contains asbestos, if black mastic is encountered during tile removal in any area of the building, it should be assumed to contain asbestos even if it wasn t sampled as part of this inspection. Removal of the floor tile and mastic may be performed by a flooring contractor if the recommended work practices published by the Resilient Floor Covering Institute are followed ( In order to prevent release of asbestos fibers, these practices dictate that tile must be removed whole, and no mechanical means may be used for removal of tile or mastic. If these methods cannot be followed, the tile and mastic should be abated by a certified asbestos abatement contractor. These materials are non-friable and may be disposed of as construction debris if they would not be rendered friable during removal. Asbestos was detected in joint compound that was adhered to a sample of cove base collected in the basement. This material contained less than 1% asbestos and therefore is not regulated by the EPA and may be disposed of as construction debris. Similarly, the 12x12 ceiling panels located in the first and second floor offices contains less than 1% tremolite cleavage fragments, and may be disposed of as construction debris. Be advised that other materials in the building that were not sampled as part of this inspection may contain asbestos. If renovations to the building are planned which include materials that were not sampled as part of this inspection, or if the building is planned for demolition, an asbestos inspection of these materials must be conducted. Any potential renovation or demolition contractor should be advised of the findings of this report. This advice meets the property owner s OSHA Communication of Hazards requirements. Asbestos materials classified as NON-REGULATED at the time of the inspection may become REGULATED due to ongoing or planned maintenance, renovation or demolition actions which can transform a material containing greater than 1% asbestos from a nonfriable and NON-REGULATED to a friable and REGULATED condition. Classification of ACM in this section and in the executive summary of this report is therefore based on the observations of the inspector at the time of the inspection and may or may not be appropriate at later dates. 2. Maintenance, renovation, demolition, weathering, normal wear, water or other damage can alter the NON-REGULATED status of materials, and necessitate precautions required for handling them as REGULATED asbestos materials. 3. 7 ASBESTOS INSPECTION AND ASSESSMENT 1.0 INTRODUCTION AND BACKGROUND On April 28, 2015, Joelle Dickson and Ken Larsen of SAGE Environmental, L.L.C. (SAGE) conducted an asbestos inspection and assessment of the Church of Scientology building located at 709 E. South Temple, Salt Lake City, Utah. The building is an approximately 41,000 square foot, two story plus basement office building built in 1976 and remodeled in The purpose of the inspection was to identify potential asbestos containing materials (ACM) prior to renovation of the building. Samples of suspect ACM were collected and submitted for laboratory analysis using polarized light microscopy (PLM). 2.0 MATERIALS AND METHODS Based on the architect s description of the planned renovations, a limited inspection of the facility was conducted to observe, identify and locate floor coverings, mastics and leveling compounds; cove base; pipe insulation; and ceiling finishes on all three floors of the building. One sample of gypsum board was also collected on the main floor where a wall will be removed. All samples of pipe insulation were collected in the boiler room and may not be representative of this type of insulation throughout the building. Bulk samples of suspect materials were collected by SAGE and microscopically analyzed for asbestos content by Dixon Information in Salt Lake City, Utah. This laboratory participates in the U.S. Environmental Protection Agency Bulk Asbestos Sample Quality Assurance Program and the National Institute for Standards and Technology's National Voluntary Laboratory Accreditation Program (NVLAP). All suspected ACM was either sampled or assumed to be ACM. 5.0 REGULATORY STANDARDS The United States Environmental Protection Agency (EPA) National Emission Standards for Hazardous Air Pollutants (NESHAP) includes standards for asbestos removal, transportation and disposal, and building demolition. These standards are enforced by the State of Utah Department of Environmental Quality (DEQ), Division of Air Quality (DAQ). The EPA requires that friable ACM be removed from buildings prior to renovation or demolition. Friable materials are those that can be crumbled, pulverized, or otherwise broken up by using hand or finger pressure when dry. The EPA regulates any friable material containing more than one percent (1%) asbestos. However, the Occupational Health and Safety Administration (OSHA) regulates asbestos exposure to employees working with asbestos regardless of the percentage of asbestos contained in the material. The EPA does not regulate non-friable materials and allows them to be disposed of as ordinary demolition waste or construction debris. However, non-friable ACM can become friable over time through deterioration or when disturbed, such as during maintenance or removal operations, which can present a potential health hazard. Accordingly, SAGE recommends that non-friable ACM be removed only as part of scheduled demolition or renovation projects. DAQ regulation R requires that asbestos inspections older than three years be reviewed and updated, as necessary, by a certified inspector prior to an asbestos abatement, renovation or demolition project. If the inspection report is still accurate, the inspector shall provide documentation to that effect. Asbestos Inspection & Assessment Page 5 Church of Scientology Building 709 E. 12 6.0 RECOMMENDATIONS Several types of floor tile and all black mastic in various areas of the basement as well as a few areas on the main and second floors were found to contain asbestos, as indicated in Table 1. Note that floor tile samples CS-FT-01 and CS-FT-02 are the same tile; the FT-02 sample had two types of mastic and so was sampled separately. Since all of the black mastic sampled contains asbestos, if black mastic is encountered during tile removal in any area of the building, it should be assumed to contain asbestos even if it wasn t sampled as part of this inspection. This advice meets the property owner s OSHA Communication of Hazards requirements. 7.0 DISCLAIMER SAGE conducted this asbestos inspection using reasonable efforts according to industry standards, and in accordance with the agreed scope of services. This report is not definitive and should not be assumed to be an exhaustive inspection of all asbestos containing materials that exist at the project site. Information in this report is not intended to be used as a construction document and should not be used for demolition, renovation or other construction purposes without field verification by the construction/demolition contractor. Report findings, conclusions and recommendations of this report are based, in part, on information and/or documents provided by the client or the project site representative. SAGE relies on such information and/or documents, and assumes that information to be true and correct. Regardless of the findings stated in this report, SAGE is not responsible for consequences or conditions arising from facts that were concealed, withheld or not fully disclosed. 15 DEFINITIONS Asbestos-containing material (ACM) is material containing more than 1% asbestos as determined by Polarized Light Microscopy (PLM). Friable asbestos-containing material is any ACM that, when dry, can be crumbled, pulverized, or reduced to powder by hand pressure. Non-friable asbestos-containing material is any ACM that, when dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure. Non-friable ACM is further divided into two categories: Category I Non-friable ACM means asbestos-containing packings, gaskets, resilient floor covering and asphalt roofing products. Category II Non-friable ACM means all other non-friable ACM not included in Category I. Regulated asbestos-containing material (RACM) is material that is regulated under the National Emissions Standards for Hazardous Air Pollutants (NESHAP) and includes the following: a. Friable ACM b. Category I non-friable ACM that has become friable c. Category I non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading d. Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized or reduced to powder by forces expected to act on the material during regulated renovation or demolition activities. Non-regulated asbestos-containing material (N-RACM) is all non-friable ACM that has not become friable and will not become friable during regulated renovation or demolition activities.
