1 TO ALL BIDDERS: OFFICE OF THE UNIVERSITY ARCHITECT June 8, 2011 ADDENDUM NO. 3 To Project Manual and Drawings For: Memorial Athletic and Convocation Center Roof Replacement Phase 2 KSU-11L120 This Addendum, hereby modifies, supplements, and amends the Bidding Documents, including Bidding Requirements, Contracting Requirements, Specifications, and Drawings for all labor, material, and equipment of every nature and required for the execution of the entire work for: Memorial Athletic and Convocation Center Roof Replacement Phase 2. Documents being modified are those dated May 16, 2011, Issued For Bid, as prepared by Kent State University. Wherein this Addendum varies or is in conflict with the Bidding Documents, the requirements of this Addendum shall govern. In all other particulars, the requirements of the original Bidding Documents, including any previous Addenda, shall govern. Bidder must indicate receipt of ALL Addenda in writing with his Bid. PROJECT MANUAL: ITEM 1 Modifies: Document Solicitation Notice to Bidders Revision: Revise the Bid Due Date from Tuesday, June 14, at 02:00 p.m. to Tuesday, June 21, at 02:00 p.m. Note: Due to issues resulting from the Asbestos Content of the Kaylo Roof Decking at Roof Areas 11, 12, & 13, the Bid Due Date will be extended to allow time to further investigate the roofing systems on these three roof areas. KSU will be coordinating providing new sample areas in these areas to observe the content and attachment method of roofing systems. Contact Phil Steinberg at Braun & Steidl Architects for time and date if you wish to be present during test cuts. ITEM 2 Modifies: Specification Section Existing Hazardous Material Information Attachment: Specification Section Existing Hazardous Material Information Note: CTL Engineering of Ohio, Inc. and American Analytical Laboratories, Inc. reports were not altered, and both remain as attachments to Specification Section (copies not included as part of Addendum). Revision: Revise to add new paragraph C as follows:. C. A hazardous material investigation was conducted at the site for Asbestos Containing Materials for the Rackle Slab Roof Decking. Investigation results are documented in a report prepared by the EA Group, for the Kent State University MAC Center Roof Project, dated June 6, Revision: Added report from EA Group to end of Section. Report obtained for the University s use only. 3. Copy of reports follows this Section. B. Related Sections: 1. Document General Conditions 1.02 HAZARDOUS MATERIALS INVESTIGATION DATA A. A hazardous material investigation was conducted at the site for Asbestos Containing Materials. Investigation results are documented in a report prepared by CTL Engineering of Ohio, Inc. for the Kent State University MACC Building Envelope Restoration Project, dated May 5, B. A hazardous material investigation was conducted at the site for Asbestos Containing Materials for the Kaylo Roof Decking. Investigation results are documented in a report prepared by American Analytical Laboratories, Inc. for the Kent State University MACC Center Project, dated June 3, C. A hazardous material investigation was conducted at the site for Asbestos Containing Materials for the Rackle Slab Roof Decking. Investigation results are documented in a report prepared by the EA Group, for the Kent State University MAC Center Roof Project, dated June 6, D. Interpretation: 1. Hazardous material data is available for information and convenience of Bidders, and is not warranted to indicate actual conditions. 2. Kent State University and the A/E do not assume responsibility for variations in amount and level of hazardous material contamination. The University and A/E disclaim responsibility for accuracy, true levels, and extent of hazardous materials investigation prepared by others; and further disclaim responsibility for interpretation of hazardous material data by Bidders and presence, level, and extent of hazardous materials. 3. Hazardous material report and data is not part of the Contract Documents. 4. Report any variances of hazardous Materials in writing to the University s Representative BIDDER INVESTIGATION A. Contractor: Visit site and become familiar with site conditions. B. Contractor may, at the Contractor s own expense and prior to bidding, make any hazardous material analysis tests the Bidder considers necessary, with authorization by the University in advance.
