1 ADDENDUM No. 2 February 17, 2015 Zeeb Road Pathway Scio Township The following changes, additions, and/or clarifications to the Contract Documents shall be incorporated in said documents and shall be allowed for in the unit prices bid by the Contractor such that the unit prices indicated in the Bid Form shall represent the conditions as set forth in the Contract Documents and this addendum. The bidder shall acknowledge the receipt of this addendum on page BF-1 of the Bid Form and shall staple this addendum into the Contract Book. The Bidder shall also return, via facsimile or , the acknowledgment sheet located on page 13 of this addendum. This Addendum contains 13 pages. ADDITIONS AND CHANGES TO THE SPECIFICATIONS SECTION BID FORM: Remove pages BF-3A and BF-4A and replace with pages BF-3A2 & BF-4A2 included in this addendum. The new pages reflect the following changes: 1. Add new pay item #52 Helical Pier, Additional 7 section, 116 Ea SECTION SPECIAL PROVISIONS: Acceptable boardwalk material substitutions in Helical Pier and Pressure Treated Wood Boardwalk Special Provisions: Hot-dipped galvanized steel is an acceptable substitution to anywhere stainless steel materials are specified in the boardwalk appurtenances SECTION SPECIAL PROVISIONS: Replace existing Helical Pier Special Provision with the Special Provision included in this addendum. The new Special Provision reflects the following changes: 1. Additional language is underlined and in bold SECTION APPENDIX B: Remove existing MDEQ Permit Application with the approved and signed MDEQ Permit. ADDITIONS AND CHANGES TO THE PLANS: Remove plan sheet 8 and replace with sheet 8 included in this addendum. The new sheet reflects the following: 1. 4 SCIO TOWNSHIP SPECIAL PROVISION FOR HELICAL PIERS OHM:AJR 1 of 2 02/16/2015 a. Description. The work of Helical Piers shall consist of furnishing all materials, equipment and labor required to install Helical Piers at the location shown on the plans. b. Materials. The helical pier manufacturer must provide shop drawings certifying that the piers can support a 15 kip vertical load upon completion of installation and are capable of handling all installation forces without overstress. Helices are to have a 12 diameter top helix and 10 diameter bottom helix. Materials as required in accordance to 2012 MDOT Standard Specifications for Construction: 1. Pier Shafts (lead sections and extensions): The inch diameter round tubular.260 minimum wall thickness with connection bolts of 0.75 diameter SAE J429 Gr 5 steel (Fy=120 KSI) or equivalent. Structural tubing per ASTM A500 Gr C. Ultimate mechanical capacity 70,000 lbs compression, 60,000 lbs tension minimum. 2. Helices: Carbon steel sheet, strip, or plate formed on matching metal dies to true helical shape, 0.5 inch thick, and shall conform to the following ASTM specifications. 11,000 ft.-lbs.1.75 inch piers: ASTM A656 Grade Bracing: shall be similar material to pier shaft material. Material shall minimize lateral movement to no greater than 1. Diagonal bracing between piers or a single additional diagonal helix may be used. Bracing shall occur a minimum of every other pier location unless necessity is proven otherwise. 4. Pier header brackets: 1/2 structural steel U shaped and 12 long to snugly accommodate four 2 x12 boards as shown on the plans with bolt holes. Sides of U are to be 6 high minimum. Weld angle to threaded rods and provide gusset plate stiffeners as required. Provide coupler and structural tube attached to threaded rods. Coupler as compatible with pier shaft fastened to shaft with weld, bolt or epoxy. A36 angle, plate and structural steel tubing. 35 ksi min for all pipe. Ultimate capacity 200,000 lbs. Further subject to manufacturers specifications. 5. Couplings: Cold-forged welded to the shaft. 6. Finish: All material shall be galvanized per ASTM B Submittals: Product data conforming to the materials listed above. Helical pier submittal must include full shop drawings signed and sealed by a licensed professional engineer in the state of Michigan. An installation plan of action is required and at minimum shall include schedule, installation procedure, plan of action if installation requires additional sections to be installed beyond 20 below existing grade. 5 OHM:AJR 2 of 2 02/16/2015 All materials shall be delivered and appropriately stockpiled at the site for the Engineer s inspection and approval prior to use. c. Construction. The work shall be done in accordance with the manufacturer s installation recommendations. Heavy equipment will not be allowed in the wetland during construction and comply with all provisions of the MDEQ permit. Helical piers must be placed within 6 lateral tolerance of the dimensions show on the plans. The Contractor is not to install any substructure units without the Owner s representative present. The Contractor must give at least 72 hour notice to the owner in order to schedule a representative to be on site. The Contractor must receive approval from the Owner in order to install helical piers beyond the depth of 20 feet below existing grade. d. Measurement and Payment. The completed work as measured shall be paid for at the contract unit price for the following contract item (pay item): Contract Item (Pay Item) Pay Unit Steel Helical Pier... Each Helical Pier, Additional 7 sectionddddddddddddddddd.dddd.each The completed work for Helical Pier shall include payment for furnishing all equipment, labor, and materials necessary to complete the work as described. Helical Piers will be measured in place for each completed vertical pier installed up to a depth of 20 below the existing grade. Helical Pier, Additional 7 section will be measured in place for each 7 foot section used beyond the initial 20 used for the Helical Pier item. The Contractor is required to receive authorization to continue installation from the Owner prior to using the item Helical Pier, Additional 7 section. Diagonal bracing will not be paid for separately and will be considered incidental to this pay item regardless of type. Page 5 of 13
