2 339 ELMWOOD DRIVE PASADENA /29/17 Page! 2 of! 6 STATE LAW REQUIRES THAT YOU BE GIVEN THE FOLLOWING INFORMATION CAUTION - PESTICIDES ARE TOXIC CHEMICALS Structural pest control companies are registered and regulated by the Structural Pest Control Board and apply pesticides which are registered and approved for use by the California Environmental Agency, Department of Pesticide Regulation and the United States Environmental Protection Agency. Registration is granted when the state finds that based on existing scientific evidence there are no appreciable risks if proper use conditions are followed or that the risks are outweighed by the benefits. The degree of risk depends upon the degree of exposure, so exposure should be minimized. If within 24 hours following application you experience symptoms similar to common seasonal illness comparable to the flu, contact your physician or poison control center at and your pest control company immediately. NOTICE: This is a Wood Destroying Pests and Organisms Report and does not include mold or mold-like conditions and was not inspected for the presence or absence of health related molds or fungi. No reference will be made to these conditions. According to California State Law, we are not qualified, authorized or licensed to inspect for health related molds or fungi. Mold is not a wood destroying organism and is outside the scope of this report as defined by the Structural Pest Control Act. If you wish to have your property inspected for mold or mold-like conditions, please contact the appropriate professional. NOTICE: The exterior surface of the roof will not be inspected. If you want the water tightness of the roof determined, you should contact a roofing contractor who is licensed by the Contractors State License Board. Foss Termite Control, Inc. will assume no responsibility for any damage done to the roof due to the fumigation process, unless otherwise stated in this report. NOTICE: The following areas may be considered inaccessible for inspection: furnished interiors; inaccessible attics or portions thereof; the interior of hollow walls; spaces between a floor or porch deck and the ceiling or soffit below; insulated areas; areas covered by plant growth; stall showers over finished ceilings; such structural segments as porte cocheres, enclosed bay windows, buttresses, and similar areas to which there is no access without defacing or tearing out lumber, masonry or finished work; built-in cabinet work; floors beneath coverings, areas where storage conditions or locks make inspection impracticable. We are not responsible for conditions that may exist in these inaccessible areas. NOTICE: This is a separated report which is defined as Section I/Section II conditions evident on the date of the inspection. Section I contains items where there is visible evidence of active infestation, infection or conditions that have resulted in or from infestation of infection. Section II items are conditions deemed likely to lead to infestation or infection but where no visible evidence of such was found. Further inspection items are defined as recommendations to inspect area(s) which during the original inspection did not allow the inspector access to complete the inspection and cannot be defined as Section I or Section II. RECOMMENDATION: Fumigate entire structure(s) with Sulfuryl Fluoride (Vikane or Zythor) with Chloropicrin as a warning agent for the control of drywood termites. Cover or remove accessible evidence. See fact sheet for chemical precautions and information. Some roofs may sustain damage during the fumigation process. It is the owner's responsibility for repair of the same. FINDING 2B: Drywood termite damage, fungus infection and/or dryrot damage noted at the deck flooring, subframing, joist, stairs and rail. FINDING 2C: Evidence of drywood termites noted at the detached deck. RECOMMENDATION: Locally treat infested area with registered termiticide (Cy-kick, active ingredient Cyfluthrin). Cover or remove accessible evidence. FINDING 2D: Drywood termite damage, fungus infection and/or dryrot damage noted at the detached deck flooring, rail and sub-framing FINDING 2E: Drywood termite damage, fungus infection and/or dryrot damage noted at the fascia board.

FINDING 3B: Fungus infection and/or dryrot damage noted at the detached deck sub-framing. FINDING 3C: Fungus infection and/or dryrot damage noted at the basement door jamb. FINDING 3D: Fungus infection and/or dryrot damage noted at the house trim. FINDING 3E: Fungus infection and/or dryrot damage noted at the fascia board. SECTION II FINDINGS FINDING 4A: Cellulose debris noted in the subarea. RECOMMENDATION: Remove cellulose debris. FINDING 4B: Water stains noted at the sub-framing. RECOMMENDATION: Contact proper tradesman to evaluate condition. FURTHER INSPECTION FURTHER INSPECTIONS FINDING 5A: The basement was partially inaccessible for inspection due to storage.

We agree to it, and authorize Foss Termite Control, Inc. to proceed with the work. If this agreement indicates that a title or escrow company is to be billed for the work, we hereby instruct that title or escrow company to hold funds sufficient to pay the bill and to pay the bill upon receipt of Notice of Completion or close of escrow. We understand, however that we are responsible for payment. Customer is aware that chemicals used to effect the intent of this structure may be harmful. Customer holds Foss Termite Control, Inc. harmless from any unusual damage and all damages that may result therefrom. This is the agreement. Absent inclusion, there is no representation. This means that after a court hearing, your property could be sold by a court officer and the proceeds of the sale used to satisfy the indebtedness. This can happen even if you have paid your contractor in full if the subcontractor, laborers or suppliers remain unpaid. To preserve their right to file a claim or lien against your property, certain claimants such as subcontractors or material suppliers are required to provide you with a document entitled Preliminary Notice. General contractors and laborers for wages do not have to provide this notice. a preliminary notice is not a lien against your property. Its purpose is to notify you of persons who may have a right to file a lien against your property if they are not paid. General Information -All work performed by this company is guaranteed for one year, unless otherwise stated in this report. -Fumigations are guaranteed for two years. -Finish painting is not included, unless otherwise stated in this report. -If requested by the person ordering this report, a reinspection of the structure will be performed.
