1 PORTLA STATE UNIVERSITY ADDEUM #3 Science, Research and Teaching Center Cooling Tower Replacement August 16, 2012 This Addendum is issued to provide answers to questions from potential bidders and to inform you of the following revisions to the above-referenced ITB and the Contact Documents for the Project. Where this Addendum is inconsistent with the ITB or any previous addenda, this Addendum shall control. Proposals shall conform to this Addendum. Unless specifically changed by this Addendum, all other requirements, terms and conditions of the ITB, and any previous addenda, remain unchanged and can be modified only in writing by PSU. A. SPECIFICATIONS: PSU Asbestos Specifications B. DRAWINGS: ME7.14 Rev 1 Exterior Elevations requested by city MP7.10 Rev 1 Piping plan changes at cooling tower C. ATTACHMENTS: 1) Basis of design sound data (Contractor Requested). 2) 2008 Asbestos Report. Please note this was conducted prior to the recent SRTC major infrastructure remodel. Much of the asbestos in the sub-basement was removed as a part of that project. D. CLARIFICATIONS: 1) Per the mandatory job walk there will be a concurrent electrical infrastructure project occurring between the dates of 12/22/12 through 01/01/13, contractor to provide all temporary electrical generators and lighting as required. 2) PSU has confirmed that all non-louvered panels of the existing cooling tower contain transite asbestos. Contractor to provide demolition and disposal of asbestos per all state, local and federal codes, see the attached PSU specifications.

2 3) Contractor to provide a logistics plan as a part of the submittal package. The adjacent green space can be used as a staging area but it must be re-planted, as required, to match existing condition. The adjacent driveway can be used for a crane pick area and material drop off point but specific times for use of that driveway must be coordinated with PSU staff. 4) Provide 3-D engineered drawings of the project piping; only new piping is required to be shown up to the tie in points. This will be required in the submittal package and included in the as-built documentation. 5) Contractor is to provide a chemical treatment plan as a part of the submittal process. 6) Per the job walk, the cooling tower VFD is to be ABB or approved equal and provided with the cooling tower and to be located within the cooling tower control panel. Substitution Requests: BAC Cooling Tower This substitution request has been rejected for the following reasons: 1. BAC unit has a higher pressure drop 7.85 psi vs. 2.4 psi 2. Not clear what materials are being submitted; EVAPCO is stainless on lower half including the basin. 3. Options of ladder/platform, vibration switch, flume plate, control panels, VFD, louver access panel, seismic construction are not included 4. Dimensional constraints of this existing cooling tower yard limit the width that can be used; BAC is 21-6 wide vs Top connection for condenser water is a problem for hiding the piping. Project has strict visual campus/ city of Portland constraints. 6. Documentation provided did not adhere to Division section 3.01 of the project specification. Puroflex Sump Sweeper This substitution request has been rejected for the following reasons: 1. The model submitted does not match the pressure requirements of the basis of design product. 2. Documentation provided did not adhere to Division section 3.01 of the project specification.

7 PORTLA STATE UNIVERSITY ASBESTOS SURVEY SCIENCE BUILDING SW 10 TH AVE., PORTLA, OR REVISED JULY 21, 2008 FORENSIC ANALYTICAL PROJECT #PJ4385 PAGE INTRODUCTION Forensic Analytical Specialties, Inc. performed an inspection to identify asbestos-containing building materials at the property located at 1719 SW 10 th Avenue, Portland, Oregon. Noal Kraft, Dan Rouse, and Robin Sharpe conducted the field investigation on April 1 through April 3, 2008, at the direction of Kate Vance, of Portland State University Facilities and Planning. The purpose of the inspection is to determine whether ACM is present in the structures onsite. The subject property consists of a four story building with three basement levels equaling approximately 215,860 square feet. The building was constructed in This report presents the results of the asbestos inspection conducted by Forensic Analytical Consulting Services, Inc. for the above referenced project. Results of the inspection are summarized in Appendix A. Drawings depicting ACM and sample locations are presented in Appendix B. A detailed laboratory report and chain of custody forms are contained in Appendix C. 2.0 FIINGS The results of the samples indicate that asbestos was detected at the site. A complete table of the samples taken and materials assumed to contain asbestos can be found in Appendix A. 3.0 RECOMMEATIONS Materials for which sample analysis by PLM results in greater than one percent asbestos (for any one sample collected from a homogeneous material) are classified as ACM under regulations promulgated by, but not limited to, the following agencies: federal EPA and federal OSHA. The agencies use the following definitions: Federal EPA (Oregon DEQ): materials containing greater than one percent asbestos are ACM Federal OSHA (OR-OSHA): materials containing greater than one percent asbestos are ACM For detailed regulatory requirements in specific situations, Forensic Analytical should be consulted, or the applicable regulations should be examined. All materials identified were classified by condition. Materials in "good" condition should be maintained in place following the OR-OSHA operations and maintenance requirements. Materials in "Fair" condition should have patch and repair activities performed to address any damaged areas. Materials in "Poor" condition should be addressed through removal, repair and/or encapsulation.

During the inspection, delaminated fireproofing material was observed on the top of the ceiling tile where the fireproofing is installed. The pipe fitting insulation on the sprinkler risers on the north and south end of the roof is deteriorated. 4.0 METHODS Materials suspected of containing asbestos were sampled in accordance with the federal EPA AHERA protocols. Materials determined by the inspector to be non-suspect, such as wood, metal, glass, and fiberglass insulation, were not sampled. Destructive sample techniques were not used during the inspection. Additional suspect building materials may be present in areas that were inaccessible, unsafe to inspect, or obscured from view during the inspection process. Suspect materials were grouped and classified as homogeneous materials based on their appearance, usage, and age of the building. Representative samples of each homogeneous material were collected for laboratory analysis. Where previous sample data exists, one confirming sample was collected of materials that previously tested positive. Additionally, where multiple samples of a given homogeneous material were collected, the set was analyzed to first positive. The unique sample description ID was developed specifically for PSU. The sample ID includes; the PSU building ID; the homogeneous material number; followed by a unique material code (FT = Floor Tile); and lastly the sample number. Samples were collected in such a manner as to minimize release of the material into the surroundings. Material type, sample number, sample location and other pertinent information were recorded at the time of sampling. Each sample was placed in a sample container labeled with a unique sample number and submitted to Forensic Analytical s NVLAP-accredited laboratory for analysis under chain of custody documentation. Samples were analyzed in accordance with EPA Method 600/R , using PLM with dispersion staining and using visual area estimation to determine percent asbestos content. This method allows for the identification of the primary types of asbestos used in building materials. The lower limit of detection for this method is one percent. Samples containing less than one percent asbestos by PLM with visual area estimation are reported as Trace. 5.0 LIMITATIONS Forensic Analytical did not inspect subsurface areas for asbestos. ACM such as underground waterproofing coatings, asbestos-cement water pipe, electrical ducts, or other subsurface materials or equipment may be present beneath the site. Forensic Analytical did not disassemble building equipment; such as fans, ducts, elevator equipment, and electrical equipment. Consequently, equipment may contain untested gaskets, internal components, overspray of building materials and the like. It is also common for carpet to be installed over previously installed floor materials. If the aforementioned materials or any other untested suspect materials are encountered during

Forensic Analytical has performed this asbestos sampling in accordance with generally accepted methods and practices of the profession, and consistent with that level of care and skill ordinarily exercised by reputable environmental consultants under similar conditions and circumstances. 69 GLOSSARY OF TERMS ACM - Asbestos-containing material: Any material containing more than one percent asbestos. This includes suspect and/or presumed ACM. AHERA: Asbestos Hazard Emergency Response Act of AHERA Building Inspector: A person who has successfully completed the training requirements for a building inspector established by EPA Asbestos Model Accreditation Plan; Interim Final Rule (40 CFR Part 763, Appendix C to Subpart E, I.B.3) and whose certification is current. AHERA Project Designer: A person who has successfully completed the training requirements for an asbestos abatement project designer established by EPA regulations (40 CFR (g)) and whose certification is current. Asbestos: Chrysotile, amosite, crocidolite, tremolite, anthophyllite, actinolite and any of these minerals that have been chemically treated and/or altered. Asbestos Building Inspection: A written report describing an inspection using the procedures contained in EPA regulations (40 CFR 763,86) to determine whether materials or structures to be worked on, renovated, removed, or demolished (including materials on the outside of structures) contain asbestos. Authorized Person: Any person authorized by the employer and required by work duties to be present in regulated areas. Chain of Custody Record: Legal documentation that follows samples from collection to the laboratory indicating who has been in possession of the samples. Competent Person: A person capable of identifying asbestos hazards, selecting appropriate control strategies and having the authority to take prompt corrective measures.
