Bidders are hereby informed that the construction Plans and/or Specifications are modified as follows: This addendum shall supersede the contract documents in so far as it is disagreement with the same. This addendum is hereby incorporated as part of the construction drawings. SPECIFICATION CHANGES: 1. Add a missing Section Title < Special Project Procedures for DOC > to TABLE OF CONTENTS, under DIVISION 1 - GENERAL REQUIREMENTS, between Section & Section Change the page number from 8 to 9 for Section Thermoplastic Polyolefin Roofing as shown in TABLE OF CONTENTS. 3. Delete <DIVISION 26 - ELECTRICAL and Section Facility Lightning Protection> from TABLE OF CONTENTS. 4. The dates of List of Drawings are 05/17/13 instead of 04/08/13 as shown on Section List of Drawings, Part 3.1 List of Drawings. 5. Change Contract completion time from 40 to 55 working days as shown in Section REVISED BID FORM dated included. 6. Correction: In Section Site Security and Health Requirements (Generic), Part 3 - EXECUTION, 3.1, B - Change normal working hours from "8:00 AM to 5:00 PM" to "8:00 AM to 4:00 PM". 7. Add the following two paragraphs to Section Site Security and Health Requirements (Generic), part 3.2 Health and Traffic Controls: G. Contractor shall remove construction/demolition waste materials from Project site at the end of each day and legally dispose of them in an EPAapproved landfill acceptable to authorities having jurisdiction. No trash or

2 Page 2 of 4 waste materials are allowed to be stored on site or in dumpster more than one day. H. Delivery trucks are allowed to park only in the designated area as permitted by facility's staff or officer. The truck(s) shall be secured with doors locked & windows closed, and engine disabled with key removed at all times in the parking area at Project site. 8. Section Thermoplastic Polyolefin Roofing, part 2.3 Roof Insulation Materials, A., 3. Nominal insulation thickness shall be changed and read as the following: a. The average rigid insulation thickness is 4" as indicated on drawings. DRAWING CHANGES: 1. On sheet A201, note #21 shall be changed and read as the following: REMOVE EXISTING WOOD BLOCKING. INSTALL NEW CONDUIT SUPPORTS AFTER NEW ROOFING IS IN PLACE. See detail 2/A502. GENERAL: 1. A core-drilled field investigation was performed on existing roof of Bldg. #28 on June 18, The results of the core-drill showing existing roof assemblies above existing concrete roof slab are shown as the followings (starting from top to bottom): a. 0.06" thick EPDM roofing membrane. b. 2" rigid insulation. c. 1" thick of three layers of built-roofing with fine gravel. d. 1 1/2" thick gypsum board. e. Existing concrete roof slab. In terms of demolition, existing items from a to d shall be demolished for new roof construction. 2. A copy of Pre-Bid Attendance Record is attached for information only. The Bidder agrees to complete the work within 55 working days from the date the Notice of Intent to Award is issued as modified by additional days added by the Owner s acceptance of alternates, if applicable. This includes 12 working days for document mailing and processing. The Bidder further agrees to pay to, or allow the State as liquidated damages the sum of $500 for each working day thereafter that the entire work is not substantially complete.
