1 RANCHO ESTATES MUTUAL WATER COMPANY Request for Quotations for Public Works Project Reservoir Lining and Covering Introduction: Rancho Estates Mutual Water Company (REMWC) is accepting proposals to line and cover two potable water reservoirs. Please be advised that you are being asked to provide a quote for services on a State-funded Public Works Project. The following requirements apply to this RFQ: No contractor or subcontractor may be listed on a bid proposal for a public works project unless registered with the Department of Industrial Relations (DIR). No contractor or subcontractor may be awarded a contract for public work on a public works project unless registered with the DIR pursuant to Labor Code section No contractor or subcontractor may work on a public works project unless registered with the DIR. Perimeter fencing at two reservoirs shall be removed and replaced with new fencing; new fencing shall be tan in color. 3. Reinforced concrete anchor beam to be installed around two existing concrete reservoirs. Liners and covers to be attached to anchor beams at two reservoirs. 4. Rain removal system and associated electrical system to be installed at two reservoirs. 5. Liners and covers shall be fabricated from a minimum 45 mil thickness CSPE a. Cover shall be tan in color. 6. The underlayment for the liner systems shall be 16-ounce per square yard geotextile. 7. Allow for two conduit sleeves, 2 diameter, for chlorination system. 8. Prevailing wages must apply Project Line Item Add Ons: Contractor to determine if items listed below are necessary. If necessary please state so in your proposal. 1. Deconstruct, and rebuild overflow structures to run flush with curvature of existing reservoir. Provide, and Install grate for overflow pipe. 2. Deconstruct, and rebuild silt dam at bottom of reservoirs. Duties performed by REMWC: 1. Remove sediment and solidified chlorine from reservoirs. 2. Clean concrete walls of both reservoirs. 3. Dispose of removed material. Award Basis: 2

3 Award will be made to the lowest responsible bidder; qualified bidder with the lowest or best bid price, and whose business and financial capabilities, past performance, and reputation meet the required standards. 4 Exhibit A Rancho Estates Mutual Water Company Price Sheet This Price Sheet must be submitted with your quote, whether the submission is manual or electronic. Name of Contractor: Provide your TOTAL COST using unit pricing/fully burdened hourly rates/etc. as appropriate to the service for meeting the Scope of work/specifications outlined above. Your TOTAL COST must be fully burdened to include all costs associated with providing the services and meeting the required minimum specifications in Scope of Work. 5 Exhibit B Prevailing Wage Special Conditions PUBLIC WORKS AND PAYMENT OF PREVAILING WAGE A. Recipient/General Requirements 1. Recipient shall comply with state prevailing wage law, Chapter 1 of Part 7 of Division 2 of the Labor Code, commencing with Section 1720 and Title 8, California Code of Regulations, Chapter 8, Subchapter 3, commencing with Section 16000, for any "public works" (as that term is defined in the statutes) performed on the Project funded by this Agreement. For purpose of compliance with prevailing wage law, the Recipient shall comply with provisions applicable to an awarding body. Compliance with state prevailing wage law includes without limitation: payment of at least prevailing wage as applicable; overtime and working hour requirements; apprenticeship obligations; payroll recordkeeping requirements; and other obligations as required by law. 2. Recipient shall certify to Rancho Estates Mutual Water Company on each Payment Request Form, that prevailing wages were paid to eligible workers who provided labor for work covered by the payment request and that the Recipient and all contractors complied with prevailing wage laws. 3. Prior to the release of any retained funds under this Agreement, the Recipient shall submit to the Rancho Estates Mutual Water Company a certificate signed by the Recipient and all contractors performing public works activities stating that prevailing wages were paid as required by law. The required certificate follows these special conditions. B. Flowdown Requirements Recipient shall ensure that all agreements with its contractors to perform work related to this Project contain the following provisions: 1. Contractor shall comply with state prevailing wage law, Chapter 1 of Part 7 of Division 2 of the Labor Code, commencing with Section 1720; and Title 8, California Code of Regulations, Chapter 8, Subchapter 3, commencing with Section 16000, for all construction, alteration, demolition, installation, repair or maintenance work over $1,000 performed under the contract. Contractor's obligations under prevailing wage laws include without limitation: pay at least the applicable prevailing wage for public works activities performed on the Project; comply with overtime and working hour requirements; comply with apprenticeship obligations; comply with payroll recordkeeping requirements; and comply with other obligations as required by law. 2. Contractor shall ensure that the above requirements are included in all its contracts and any layer of subcontracts for activities for the Project. 5
