May 17, 2012 Asbestos Site Inspection Report AIP INSPECTION SUMMARY Eight vacant single family homes located at 13, 57 & 72 Dumont Avenue, 23 Maryland Street, 2 & 6 Patrick Street, 392 White Street and 6 Elizabeth Street, South Burlington, Vermont were inspected for the presence of asbestos containing materials (ACMs) between January 19 and April 13, The inspections were predemolition inspections conducted to fulfill US EPA NESHAPS requirements in accordance with 40 CFR Part 61 Subpart M and State of Vermont asbestos requirements in accordance with V.S.A. Title 18, Chapter 26. The inspections/sampling included obtaining bulk samples from suspect materials inside and out side the buildings. The inspections are believed to have reasonably determined the ACMs that exist in the buildings. 13 Dumont Avenue 15 different homogeneous suspect materials were identified and sampled in this house. Asphalt shingle roofing (2% Chrysotile asbestos, non friable), window glaze in the basement (5% Chrysotile asbestos, friable), and sink dampener in the kitchen (3% Chrysotile asbestos, non friable) were determined to be ACM. 57 Dumont Avenue 12 different homogeneous suspect materials were identified and sampled in this house. 9 x9 floor tile in the basement (6% Chrysotile asbestos, non friable) was determined to be ACM. In addition the basement chimney packing was <1% asbestos containing which is not an asbestos containing material but VOSHA worker protection rules have to be followed when disturbing it. 72 Dumont Avenue 14 different homogeneous suspect materials were identified and sampled in this house. Chimney patching in the basement (15% Chrysotile asbestos, friable), 12 x12 floor tile in the basement (6% Chrysotile asbestos, non friable), sheet flooring in the kitchen (12% Chrysotile, non friable), and sheet flooring in the bathroom (10% Chrysotile asbestos, non friable) were determined to be ACM. In addition the sink dampener in the kitchen and the vermiculite insulation in the attic were also assumed ACM. 23 Maryland Street - 8 different homogeneous suspect materials were identified and sampled in this house. 12 x12 red/white floor tile mastic in the basement (2% Chrysotile asbestos, non friable), the joint compound on the walls (2% Chrysotile asbestos, friable), and 12 x12 brown floor tile in the basement (10% Chrysotile asbestos, non friable) were determined to be ACM. In addition the red/white 12 x12 floor tile was <1% asbestos containing which is not an asbestos containing material but VOSHA worker protection rules have to be followed when disturbing it. 2 Patrick Street - 7 different homogeneous suspect materials were identified and sampled in this house. No ACMs were identified. 1

Floor tile in various locations (10% Chrysotile asbestos, non friable) was determined to be ACM. In addition the transite board in the garage and the vermiculite insulation in the attic were also assumed ACM. 392 White Street - 16 different homogeneous suspect materials were identified and sampled in this house. Vinyl flooring in the kitchen (20% Chrysotile asbestos, non friable), joint compound on the walls (2% Chrysotile, friable), kitchen sink dampener (15% Chrysotile, non friable), linoleum on the basement countertop (25% Chrysotile, non friable), apartment kitchen sink dampener (6% Chrysotile, non friable), porch roofing material (2% Chrysotile, non friable), and shed roofing material (10% Chrysotile, non friable) were determined to be ACM. 6 Elizabeth Street - 12 different homogeneous suspect materials were identified and sampled in this house. Joint compound on the walls (2% Chrysotile, friable), sheet flooring and mastic in the kitchen (15% Chrysotile, non friable), and basement window caulk (2% Chrysotile, non friable) were determined to be ACM. In addition the white chimney packing was <1% asbestos containing which is not an asbestos containing material but VOSHA worker protection rules have to be followed when disturbing it. A copy of this inspection report should be kept on site during demolitions. Table 1 summarizes the inspection results. 2. INSPECTION METHODS This asbestos site inspection was conducted by an EPA accredited, State of Vermont certified, Asbestos Inspector/ Management Planner and EPA accredited, State of Vermont certified, Asbestos Inspectors generally in accordance with AHERA inspection guidelines and generally accepted procedures. Bulk sample analysis was conducted by a State of Vermont licensed laboratory, EMSL Analytical, Inc. of Cinnaminson, NJ. The asbestos inspection consisted of a visual and physical inspection. The entire interior and exterior of the buildings were inspected including opening cavities. Destructive demolition inspections were performed. In accordance with the workscope for this project, bulk samples were collected and analyzed from each suspect material found except for Vermiculite insulation, when present, which has been assumed ACM in accordance with the latest EPA guidance. Bulk samples of suspect ACM were analyzed for asbestos content using PLM methods. 2

Asbestos bulk samples have been analyzed by EMSL Analytical, Inc. of Cinnaminson, NJ. 5. INSPECTION RESULTS Building material/sample locations and specific results of the sampling and analysis and visual inspection are contained in Table 1 and Appendices 2 and 3. Several asbestos containing materials were identified and are highlighted in Table OBSERVATIONS AND RECOMMENDATIONS Building Comments: The locations inspected consisted of houses greater than 30 years old. All were vacant and were to be demolished. In general the houses were in good condition. The houses were wood frame with some finished basements. The houses at 13, 57 & 72 Dumont Avenue, 2 & 6 Patrick Street, and 392 White Street were one story while the houses located at 23 Maryland Street and 6 Elizabeth Street were two story. All houses contained basements in various degrees of finish. The houses exhibited general renovations and additions. Heating systems and utilities had been either removed or shut off. Heating was generally with forced hot air or hot water. Recommendations: The structures located at 2 Patrick Street did not contain any asbestos containing materials. Regardless an EPA/ state notification of the demolition needs to be submitted to both Region 1 EPA and The State of Vermont Asbestos Control program at least 10 working days before any demolition efforts commence. Prior to demolition the identified asbestos containing materials in the houses in AIP 87 inspected on January 19 & 25, February 22, and April 13, 2012 (13, 57 & 72 Dumont Avenue, 23 Maryland Street, 2 & 6 Patrick Street, 392 White Street, and 6 Elizabeth Street) need to be removed in full compliance with the Vermont Regulations for Asbestos Control. If the houses are to be moved to another location it should be verified with the Vermont Health Department Asbestos Program that it is acceptable to move them with asbestos in place if it is not abated. Proper notifications in accordance with US EPA NESHAPS 40 CFR Part 61 Subpart M, asbestos, and State of Vermont asbestos requirements in 4
