1 ATTACHMENT A- SCOPE OF WORK SCOPE AND SPECIFICATIONS Resurface/ Repair Gym Floor at Central Middle and Riverview High Schools. Walk through available by appointment. Please contact ext to schedule an appointment 1. All work on site is to be performed safely in accordance with all OSHA standards. It is the responsibility of this contractor to enforce its safety program. 2. No contractor will be allowed to operate in an unsafe manner. Any designated Riverview Gardens School (RGSD) District of Facilities and/or Representative shall stop any work activity by contractor employees that presents a serious safety hazard. Any costs incurred as a result of non-compliance will be borne by the contractor. Repeated non-compliance may result in the removal of employees from the project. Continual non-compliance may result in contract termination. 3. The school district has a ZERO tolerance of sexual harassment of its contractors in any form and requires that all contractors avoid all offensive or inappropriate sexually harassing behavior. The school district prohibits the following towards, students, staff and visitors; unwelcome sexual advances; request for sexual favors, verbal or physical conduct of a sexual nature, unwelcomed verbal or physical conduct that creates an intimidating, hostile or offensive environment for the school districts staff, students and visitors. Any person in violation will be immediately removed from the jobsite. 4. This contractor shall submit pre-construction photos to the Riverview Gardens School District Director of Facilities and/or prior to any demolition or construction work. The photos should show existing conditions of adjoining construction that might be misconstrued as damage caused by current construction operation. 5. All contractor employees will be required to provide the School District s, a background check that identifies at a minimum, persons under suspicion of, charged with, or convicted of child abuse, or sex offender relate offenses and fingerprinting. Employees whose background

2 reports indicate such activity shall not be allowed on school premises. The cost of securing criminal background checks shall be borne by each respective contractor or subcontractor affected by this specification. The contractor must furnish this background report to the RGSD Director of Facilities prior to being admitted to the school premises. 6. This contractor will be required to submit drug testing results for all employees and subcontractors working onsite within the past six months. This project prohibits the unlawful possession, use or distribution of controlled substances and alcohol and prohibits employers and employees of subcontractors from being under the influence of such controlled substances or alcohol on the jobsite. This contractor shall provide drug testing of its employees and employees of its subcontractors on a pre-employment, post-accident or for cause basis. All drug negative testing results will be required before an employee or an employee of a subcontractor can perform any work onsite. The minimum screenings are as follows; Drug Description ; Amphetamines, Barbiturates, Benzodiazepines, Cocaine Metabolite, Marijuana Metabolites, Methadone, Methaqualone, Opiates, Phencyclidine (PCP), Propoxyphene, Alcohol. 7. This contractor must abide by the Riverview Gardens School District s discrimination policy for the duration of this project. 8. This contractor will be allowed to place the following maximum percentages of mark-up for profit and overhead for extra work performed; Labor (10%), Material (10%), Subcontract (5%) and Equipment (5%). Unit rates and hourly rates, if requested, should include the noted percentages as part of the hourly or unit rate. 9. This contractor must submit for approval a site logistics plan that outline exit paths from the building in case of an emergency, material storage and laydown area, dumpster location, proposed contractor parking, etc. 10. ALL WORK WILL GENERALLY BE SCHEDULED AFTER SCHOOL HOURS. CONTRACTORS SHOULD CONFIRM THIS INFORMATION PRIOR TO SUBMITTING THEIR BID. 11. During construction, each contractor must direct all correspondence to the RGSD Director of Facilities for distribution to the school district.

3 12. Each Contractor is responsible for reviewing the Project Schedule for timing of delivery of equipment and materials to the jobsites. This includes accounting for material escalation costs associated with material to be installed as well as potential labor rate increases. 13. Consumption of alcohol on this site will not be permitted. The RGSD reserves the right to remove anyone from this site for consumption of alcohol. 14. No smoking or tobacco products will be allowed on school property. The RGSD reserves the right to remove anyone from this site for smoking on school property. 15. Firearms are not permitted on the school district s property. 16. The Contractor shall be represented at the site by a competent full-time superintendent/foreman or other person empowered to act on behalf of the contractor from beginning of the work until acceptance unless otherwise instructed by the Riverview Gardens School District. 17. This contractor shall submit for approval a method for identifying their workers working onsite that have been approved by the school district to work onsite. Approved workers include workers that have submitted a complete and approved background check and a negative drug test. 18. This contractor shall provide temporary floor protection of all existing flooring and surrounding surfaces. All dust and debris created by this scope of work shall be cleaned to its original condition by this contractor prior to the school districts acceptance. Specific Requirements 1. Specifications included but are not limited to the following sections; a. See Below 2. This contractor shall include in its bid all costs for material, labor, and equipment necessary to repair or replace gym floor to school district specifications and standards. Contractor must identify area(s) that need repairing and/or replacing prior to doing the work and must show the Riverview Gardens School District Director of Facilities or designee. Riverview Gardens School Building Director of Facilities or designee must follow up with a written approval.

4 3. This contractor is responsible for any and all costs and procurement of permits and city licenses required for this work. This contractor shall coordinate all applicable permit inspections as required for the activities included in this work package. 4. This contractor shall include in its bid all costs for material, labor, and material necessary to provide a list of steps necessary to perform this work. 5. This contractor shall provide final cleaning of all rooms prior to owner acceptance. 6. This contractor shall include in its bid all costs for clean-up, dumpsters and/or containers, haul off, and proper disposal of debris created by this scope of work. All areas should be kept clean on a daily basis. If required, this contractor is responsible for any permits required with the city to provide dumpsters on surrounding streets or school property. 7. All painted surfaces in buildings constructed prior to 1980 should be assumed to contain lead within the paint. In areas where children under the age of 7 congregate. Work within these areas will be handled by specialized contractors in strict compliance with regulatory standards designed to limit exposure to lead dust. In other areas of the buildings, tradesmen are expected to conduct their craft in accordance with applicable standards to include, but not necessarily limited to, The Occupational Safety and Health Administration (29 CFR ), The United States Environmental Protection Agency Renovation, Repair and Painting (RRP) Rule, and the Missouri Department of Natural Resources. RGSD will not consider change orders for compliance with these rules. If contractors encounter a potential jobsite hazard, they immediately will stop work and notify RGSD Director of Facilities or designee.

5 DETAILED SCOPE OF WORK Note: Work to be completed on 1st shift and/or 2nd shift if necessary. SECTION WOOD ATHLETIC FLOORING PART 1 - GENERAL 1.1 RELATED DOCUMENTS A. Drawings and general provisions of the Contract, including General and Supplementary Conditions and Division 01 Specification Sections, apply to this Section. 1.2 SUMMARY A. This Section includes wood athletic flooring and refinishing of wood athletic flooring. B. The school included in this scope are indicated below Substitutions and/or exclusions will not be accepted. 1. Riverview Gardens High School C. Prior to start of work and removal or disturbance of any existing material, Contractor is to survey the space and notify Riverview Gardens School District (RGSD) of areas that will be affected and await confirmation that surfaces are safe for removal. Necessary testing will be provided by RGSD. 1. Any required abatement/removal of hazardous materials in any form at the project site, including but not limited to asbestos, asbestos products, polychlorinated biphenyl (PCB) or other toxic substances will be performed by RGSD Hazmat Vendor. 1.3 REFERENCE STANDARDS

6 A. MFMA (Spec) Guide Specifications for Maple Flooring Systems; Maple Flooring Manufacturers Association; current edition. B. NF Court and Field Diagram Guide; National Federation of State High School Associations; current edition. 1.4 ACTION SUBMITTALS A. Product Data: Submit three copies of manufacturers product data. B. Shop Drawings: For each type of floor assembly and accessory. Include the following: 1. Layout, colors, widths, and dimensions of game lines and markers. C. Samples for Initial Selection: Manufacturer's color charts showing colors and glosses available for the following: 1. Gym Floor Urethane Solvent Base Oil Modified. 2. Game-line and marker paint. 1.5 INFORMATIONAL SUBMITTALS A. Qualification Data: For qualified Installer and manufacturer. 1.6 CLOSEOUT SUBMITTALS A. Maintenance Data: For wood athletic flooring and finish systems to include in maintenance manuals. 1.7 QUALITY ASSURANCE

7 A. Installer Qualifications: An experienced Installer who has completed wood athletic flooring installations similar in material, design, and extent to that indicated for this Project and whose work has resulted in installations with a record of successful in-service performance. 1. Installer responsibilities include installation and field finishing of wood athletic flooring components and accessories, and application of game lines and markers. B. Mockups: Build mockups to verify selections made under Sample submittals and to demonstrate aesthetic effects and set quality standards for fabrication and installation. 1. To set quality standards for installation, install mockup of floor area. 2. Prepare finished mockup of floor area to set quality standards for sanding and application of field finishes and game lines and markers. 3. Approved mockups may become part of the completed Work if undisturbed at time of Substantial Completion. 1.8 DELIVERY, STORAGE, AND HANDLING A. Deliver assembly materials in unopened cartons or bundles. B. Protect wood from exposure to moisture. Do not deliver wood components until wet work is complete and dry. C. Store wood components in a dry, warm, well-ventilated, weather tight location and in a horizontal position. 1.9 GYM CONDITIONS

8 A. Conditioning period begins not less than seven days before wood athletic flooring installation, is continuous through installation, and continues not less than seven days after installation. 1. Environmental Conditioning: Maintain an ambient temperature between 65 and 75 deg F (18 and 24 deg C) and relative humidity planned for building occupants, but not less than 35 percent or more than 50 percent, in spaces to receive wood athletic flooring during the conditioning period. 2. Wood Conditioning: Move wood components into spaces where they will be installed, no later than beginning of the conditioning period. a. Do not install wood athletic flooring until wood components adjust to relative humidity of, and are at same temperature as, spaces where they are to be installed. b. Open sealed packages to allow wood components to acclimatize immediately on moving wood components into spaces in which they will be installed. B. After conditioning period, maintain relative humidity and ambient temperature planned for building occupants. C. Install wood athletic flooring after other finishing operations, including painting, have been completed SPECIAL PROJECT WARRANTY A. Provide contractors extended warranty against failure due to defective materials and installation of the flooring system. B. Correct defective work within a two-year period beginning with the date of final acceptance of the work by the Owner.

9 PART 2 - PRODUCTS 2.1 GYMNASIUM FLOOR A. System Description: Match existing style and pattern. 2.2 FLOORING MATERIALS A. Random-Length Strip Flooring: Match existing style and pattern. 2.3 FINISHES A. Floor-Finish System: System of compatible components and MFMA approved. 1. Floor-Sealer Formulation: The following manufacturers products have been used to establish minimum standards for materials, workmanship and function. a. Bona Systems: Bona Sport Seal, Oil-modified high build sealer. 2. Finish-Coat Formulation: Formulated for gloss finish and multi-coat application. a. Bona Systems: Bona Sport Poly Oil-modified, gloss finish. 3. Game-Line and Marker Paint: Industrial enamel compatible with finish coats. a. Bona Systems: CourtLines Sport Floor Paint High-solids, fast drying, and solvent- based sport floor paint. PART 3 - EXECUTION

10 3.1 EXAMINATION A. Examine substrates, areas, and conditions, with Installer present, for compliance with requirements for maximum moisture content, installation tolerances, and other conditions affecting performance of wood athletic flooring. B. Proceed with installation only after unsatisfactory conditions have been corrected. 3.2 PREPARATION - GENERAL A. Broom and vacuum clean substrates to be covered immediately before product installation. After cleaning, examine substrates for moisture, alkaline salts, carbonation, or dust. Proceed with installation only after unsatisfactory conditions have been corrected. 3.3 INSTALLATION A. General: Comply with wood athletic flooring manufacturer's written instructions, but not less than written recommendations of MFMA applicable to flooring type indicated. B. Pattern: Lay flooring to match existing style and pattern. 1. Remove any damaged flooring and damaged sub-floor. Replace sub-floor and lace in new floor. Hardwood must match the proper thickness and width to remain visually consistent. Replaced boards must seamlessly fit and look like the original. Flooring integrity to

11 be preserved by not only removing damaged parts, but a portion of the healthy parts surrounding the damage. Locate breaks and replace entire area. 2. Remove angle iron, perimeter trim and metal inserts in floor prior to sanding and finishing. Reinstall when work is complete. 3. All floor repairs must be performed and complete prior to refinishing. a. Area under existing bleachers is not required to be refurbished, unless area is readily accessible with minimal disruption. 3.4 SANDING AND FINISHING A. Allow installed flooring to acclimate to ambient conditions before sanding. B. Mask off adjacent surfaces before beginning sanding. C. Follow applicable recommendations in MFMA s Industry Recommendations for Sanding, Sealing, Court Lining, Finishing, and Resurfacing of Maple Gym Floors." D. Machine sand with coarse, medium, and fine grades of sandpaper to achieve a level, smooth, uniform surface without ridges or cups. Remove sanding dust by tack or vacuum. E. Finish: Apply seal and finish coats of finish system according to finish manufacturer's written instructions. Provide no fewer than four coats total and no fewer than two finish coats. 1. Finishes: Use finishing methods recommended by finish manufacturer to reduce grain raise and side bonding effect. 2. Game-Line and Marker Paint: Apply game-line and marker paint between final seal coat and first finish coat according to paint manufacturer's written instructions. a. Mask flooring at game lines and markers, and apply paint to produce lines and markers with sharp edges. b. Where game lines cross, break minor game line at intersection; do not overlap lines.

12 c. Apply game lines and markers in widths and colors according to NF Court and Field Diagram Guide; National Federation of State High School Associations; current edition. d. Apply finish coats after game-line and marker paint is fully cured. 3.5 PROTECTION A. Prohibit traffic on floor finish for 48 hours after installation. B. Protect wood athletic flooring during remainder of construction period to allow finish to cure and to ensure that flooring and finish are without damage or deterioration at time of Substantial Completion. 1. Do not cover flooring after finishing until finish reaches full cure and not before seven days after applying last finish coat. 2. Do not move heavy and sharp objects directly over flooring. Protect fully cured floor finishes and surfaces with plywood or hardboard panels to prevent damage from storing or moving objects over flooring. END OF SECTION
