Amylon, City Manager April 12, 2016 Ketchikan Medical Center Expansion Project - Owner/Executive Committee Meeting Summary Minutes of March 18, 2016 At its meeting of February 6, 2014, the City Council adopted a motion approving Contract No , Pre-Construction Services for the Ketchikan Medical Center Expansion Project, in an amount not to exceed $86,000 between the City of Ketchikan and Layton Construction Co. I Dawson Joint Venture. At its meeting of April 3, 2014, the City Council adopted a motion authorizing Amendment and Limited Notice to Proceed No. 1 in the amount of $1,196,060. At its meeting of May 15, 2014, the City Council adopted a motion authorizing Amendment and Limited Notice to Proceed No. 2 in the amount of $6,603,763. At its meeting of June 19, 2014, the City Council adopted a motion authorizing Amendment and Limited Notice to Proceed No. 3 in the amount of $320,140. At its meeting of July 17, 2014, the City Council adopted a motion authorizing Amendment and Limited Notice to Proceed No. 4 in the amount of $427,836. At its meeting of September 4, 2014, the City Council adopted a motion approving Contract No A, Construction Services for the Ketchikan Medical Center Expansion Project, in an amount not to exceed $45,207,269 between the City of Ketchikan and Layton Dawson Joint Venture Constructors. The $45,207,269 cost incorporates the initial contract and four amendments and is a Guaranteed Maximum Price (GMP). An Owner/Executive Committee meeting was held on Friday, March 18, A copy of the meeting minutes is attached for City Council review. AGENDA- 04/21/16- GGMR7

2 MEETING NOTES PROJECT: Ketchikan Hospital Expansion BY: Randy Romenesko ~ DATE: March 18, 2016 SUBJECT: Executive Team Coordination Meeting Summary PARTICIPANTS: David M., Matt B., Michele B., Ryan M., Randy R. DATE OF MEETING: 18 March, 2016 The following topics and issues discussed during the meeting warrant documentation and/or require follow up. 1. Work Progress. Interior: Work continuing with the finish trades (paint, flooring, casework, ceiling tiles, low voltage systems) on Main and Level 2. Exterior: Work on fuel system starting with tank install next week. It will be several weeks to install piping, systems and commissioning. Heli lift of elevator equipment the past Sunday went well. LDC expressed appreciation for all the support and assistance provided by PeaceHealth. Systems start-up is lagging a bit, but hope to get all water and air running by end of next week. Commissioning meeting next week will help establish a firm start date for full system testing. LDC anticipates Substantial Completion by June loth. 2. Master Budget dated 7 March 2016 attached. Costs are through Pay Application #19 showing 89% of GMP funds expended. Owner Change Order #7 was accepted by the City Council last evening. 3. ASI-077 under development for minor changes to pedestrian access at the ER I new drive to parking garage. ASI will also provide placeholder provisions for any PeaceHealth signage and parking garage height indicator that may be selected as part of the signage package. 4. PeaceHealth has requested the wall mounted cabinets in Shared Providers Offices on the 2"d floor (rooms 2112, 2211, 2213, 2404, 2406, 2491, and 2493) be removed. Work does not need to be completed until just prior to furniture delivery and install on May 23. DOWL will investigate options to accomplish this work and report back to the Exec. Team. This is new scope work. 5. NAC met with Peace Health Infection Control and Sterile Processing staff today to review the decontamination room, Equipment is to remain per design; a new hand sink is required at the entry of the room to comply with Facility Guideline Institute Page 1 of 2

3 Executive Team Coordination Meeting Summary 18 March, 2016 recommendations. PeaceHealth is looking at other possible changes, but these would be after substantial completion. 6. NAC is preparing contract drawings for the transition corridor. Team (City, LDC, PeaceHealth, DOWL) wishes to perform a 'page turn' review of the documents before releasing for pricing by LDC and HMS. It does not appear we will be able to present a firm price to the city at their 4/21 meeting. Regardless, all believe a change order can be issued in a timely manner to allow work may begin upon successful PeaceHealth transition from the old to new ORs, no later than 8/1. 7. PeaceHealth has requested the GMP Allowance #14- Landscaping be assigned to them for implementation (similar to the arrangement made for Signage). LDC has no objection to the deletion of this work from their scope; the City has no objection to PeaceHealth undertaking the work. PeaceHealth currently has a vendor who performs landscaping work at the facility. The allowance has a value of $50,000. There is a $12,000 allowance in the Patient Terrace; Ryan to confirm if that is all plants or includes cost of the 'green' screen on the north wall. If there are any corrections to these notes, please notify the author within seven (7) days of receipt. The next meeting is 3/25/2016 at 1:30pm Alaska time. Page 2 of 2
