1 CITY OF PANAMA CITY BEACH WWTF INFLUENT SCREENS REPLACEMENT April 14, 2017 ADDENDUM NO. 1 (This Form Must Be Signed and Submitted With Bid Form) Addendum No. 1 addresses the following issues: QUESTIONS 1. Will the reject pond be empty for cutting in the new gate valve? Response: Yes, the reject pond is expected to be empty for cutting in the gate valve. Bidders shall make provisions in their bid for an inflatable plug for installation in the pond side of the pipe segment (either at the valve location or penetration into the pond wall at the contractor s option) to allow the wastewater treatment facility the ability to discharge sub-standard effluent in the event of treatment process upset/malfunction. This plug shall be removed when the valve installation is complete and released for service. 2. What is the Q of the flow that will need to be bypassed and to where? Response: There are provisions in the existing wastewater treatment facility yard piping such that the influent structure can be bypassed completely for the time frames indicated in the bid documents. Although no provisions for bypass pumping/piping are required in the bid, monthly average flows from the May through July 2016 timeframe to the wastewater treatment facility were 6.7, 8.7 and 8.9 MGD respectively. Again, influent flows can be directed by existing yard piping around the influent structure and sent directly to the influent pumping station wetwell. No supplemental by-pass piping or pumping will be required. 3. Is the existing opening for the screen 1 location the correct size and are there any modifications required to it? Response: No modifications of the opening for Screen 1 are proposed. It is to remain as it exists as indicated on the Drawings. 4. Is there any heat tracing required for the small piping? Response: As indicated in Note 1 on Drawing M-201, the washwater piping and valves are to be insulated but heat tracing is not required. Pipe insulation shall be self-sealing, flexible EPDM closed foam type Aerocel SSPT or approved equal. 5. Which of the two (2) stop gates labeled as #3 is actually #3, the one left or right of #2? Response: The stop gate between Screens 1 and 2 is mislabeled and should be designated Stop Gate 1 (SG-1). See Drawing M-201 revision in this Addendum. 6. Are any structural modification required to support the new plank grating at the L? 1 of 2

Shaeffer, PE PCB PANAMA CITY BEACH WWTF INFLUENT SCREENS REPLACEMENT SECTION BID PROPOSAL FORM This proposal of (hereinafter called "BIDDER"), organized and existing under the laws of the State of, doing business as (a corporation, a partnership or an individual), whose Florida contractor s license number is is hereby submitted to the CITY OF PANAMA CITY BEACH (hereinafter called "OWNER"). In compliance with the requirements of the Advertisement for Bids, BIDDER hereby proposes to perform all WORK for the WWTF INFLUENT SCREENS REPLACEMENT project in strict accordance with the CONTRACT DOCUMENTS, within the time set forth therein, and at the prices stated below. By submission of this BID, each BIDDER certifies, and in the case of a joint BID, each party thereto certifies as to its own organization, that this BID has been arrived at independently, without consultation, communication, or agreement as to any matter relating to this BID with any other BIDDER or with any competitor. BIDDER hereby agrees to commence WORK under the CONTRACT DOCUMENTS within ten (10) calendar days after the NOTICE TO PROCEED to be issued by Owner in writing and achieve Substantial Completion of the WORK within the following completion schedule in consecutive calendar days thereafter. Liquidated damages can accrue concurrently. BIDS shall be on the basis of a lump sum price except for Items 3 and 6 as noted above, and shall be the total compensation to be paid by OWNER for the complete WORK. Final contract value will be adjusted based upon actual cost of unit price items. 3. Bid unit prices and quantities, shall be applicable for any revisions to the WORK (either additions or omissions). In addition, these unit prices and quantities shall be reflected in the Schedule of Values as specified in the General Conditions. All unit prices are understood to include all associated charges for layout, insurance, taxes, field office and supervision, overhead and profit, bonds and miscellaneous items. 4. The OWNER reserves the right to reject any and all bids received. 5. Failure to insert a bid amount for any item in the Bid Schedule will be considered grounds for the OWNER to determine the BID is nonresponsive. 6. By submitting this BID, the BIDDER and the BID BOND surety, are deemed to have stipulated and agreed that any and all claims, demands, actions or suits whatsoever, arising under this BID and/or BID BONDS, shall be subjected to the BID PROPOSAL FORM

8 sole and exclusive jurisdiction and venue of the Circuit Court of Bay County, Florida. The BIDDER and BID BOND surety do agree, by submittal of this BID, that the sole and exclusive jurisdiction and venue in said forum is proper and appropriate since performance of the underlying contract to be awarded is to be accomplished within Bay County, Florida. Bidder s Certification BIDDER certifies that it has thoroughly familiarized itself with and inspected the site and has read and is thoroughly familiar with the CONTRACT DOCUMENTS. Additional site investigation, if deemed necessary by the BIDDER, shall be performed prior to BID submittal at the BIDDER s sole expense. Bidder certifies that the BID submitted is complete and is sufficient for the Bidder to provide a fully operational and working system in accordance with the CONTRACT DOCUMENTS. Furthermore, BIDDER certifies its understanding that neither the OWNER, PROJECT REPRESENTATIVE, nor ENGINEER shall provide any labor, equipment or materials of any kind, which may be required for the performance of the WORK, unless otherwise specifically directed by OWNER. Likewise, BIDDER certifies that it shall provide all equipment, materials, labor and services necessary to complete the WORK in accordance with the CONTRACT DOCUMENTS whether or not such equipment, material, labor, or service is expressly identified. Such occurrences are deemed subsidiary obligations of the contract for which complete compensation is made under the Lump Sum. The failure or omission of any BIDDER to do any of the foregoing shall in no way relieve any BIDDER from any obligation in respect to its BID. 9 contractors or sub-subcontractors, if any, nor any of their respective employees or personnel, shall be deemed servants, employees, or agents of OWNER. 2.
