1 Page 1 of 5 Request for Proposal Maintenance 30X50 Storage Building Located West of Maintenance Office 902 West Trimble Berryville School District 27 Notice to prospective venders: The Berryville Public School District will be accepting proposals to provide and install a metal storage red iron type building, 30 X 50, with a 6 concrete slab located west of the 902 West Trimble Maintenance Office at the West Campus. The minimum requirements are listed below. Each bidder should include in the proposal a brand and specifications list. All bidders will adhere to Arkansas Annotated Code (Ethics Bill of 2001) and (Ethics in Public School Procurement Process). All bidders knowing furnishing false information for this bid will be in violation of State Law. Bid form is required. Each bidder should include in the proposal a SPECIFIC ALL-INCLUSIVE WARRANTY, MINIMUM of 1 YEAR unconditional warranty, and assurance of any warranty work resolution within 24 hours of request call. GENERAL SPECIFICATIONS: 1. Excavation/Site Work for Building: Bidder will include in bid proposal for any site work on the existing clay/gravel pad. All disturbed earth will be compacted and finished exterior exposed grade will be covered with at least 5 of top soil and sown with contractor s mix grass seed. 2. Provide metal building, 30 X 50 on concrete slab with 16 sidewalls, clear spanning the 30 direction. 3. Roofing and Siding: 26 Gauge Minimum. Color will be at owner s discretion from samples providers by the bidder. Any rake trim will match the roof color. 4. Insulation: 3 all around, walls and roof, type and specs provided by bidder. 5. Walk Door: One (1), Provided by bidder in location specified by owner, ADA approved, metal, painted to match siding color, 3 commercial with Yale locksets, bidder will spec. Owner will provide key codes, 4 keys provided by bidder with each lockset. 6. Overhead Doors: Two (2), Provided by bidder in location specified by owner, metal, painted to match siding, commercial grade, 12 X 14, with chain-driven manual opener, bidder will spec. 7. Snow guards located 2 feet above roof edge, color to match roof (bidder spec) 8. Floor: Concrete will meet the following specifications: A. Nominal 6 thickness minimum on all pours. B. 6 minimum gravel base compacted. C. Reinforcing Mesh Wire, contractor must specify. D. Concrete 3,000 PSI after 28 days. E. Concrete must have no fly ash. F. Contraction joints of 3/16 locations specified in bidder submission. G. The surface of the concrete shall be trowel led to a dense and closed finish The edges of the concrete shall be tooled with an approved edging tool in a manner that leaves a neat and smooth border.

All bidders are required to submit ONE drawing with professional-licensed engineer or other design professional stamp for the building and truss/support system that meets all Arkansas code requirements. Successful bidder will provide FIVE (5) drawings with professional-licensed engineer or other design professional stamp for the building and truss/support system that meets all Arkansas code requirements (A.C.A ). 10. All bidders are required to submit ONE detailed foundation/footing drawings with professional-licensed engineer or other design professional stamp for all post/let/truss footings that meets all Arkansas code requirements. SPECIAL TERMS AND CONDITIONS: 1. Bond Requirement A 5% bid bond or a certified check made out to Berryville School District in the amount of 5% of the bid shall accompany bidder s submission. (A.C.A ) 2. Insurance A. Contractor shall carry Workmen s Compensation Insurance as required by Arkansas Law and have a current AR license. B. Contractor shall furnish Comprehensive Manufacturer s and Contractor s Liability Insurance including Automobile and General Liability. The Contractor s Bodily Injury Insurance will be in the amount of not less than $300,000 and Property Damage Insurance in an amount not less than $100,000. C. The contractor shall indemnify and save harmless the owner from and against all losses and claims, demands, payments, suits, actions, recoveries and judgments of every nature and description brought or recovered against him by reason of any omission or act of the contractor, his agents or employees in the execution of work or in the guarding of it. The contractor shall obtain in the name of the owner and shall maintain and pay the premium for such insurance in an amount not less than $300,000 for bodily injury and $100,000 for property damage, and will include such provisions as will protect the owner from contingent liability under this contract. Full compliance by the owner with the terms and provisions of such insurance policy or policies shall be a condition precedent to the owner s right to enforce against the contractor any provisions of the article.

3 Page 3 of 5 3. Contractor A. Contractor must hold a current valid Arkansas State Contractor s License (A.C.A ). Any sub-trades contractors hired by the bidder must hold a current valid Arkansas License in the trade area (A.C.A ). All work shall be executed in strict conformity with all laws, regulations of the city and state in which the building is located. None of the terms or provisions of this specification shall be construed as waiving or canceling any of said laws and regulations. B. Each contractor shall pay all State and Federal Social Security Unemployment Compensation and for all other fees, taxes and permits which may be incurred in connection with this work. 4. The bidder will use only products that are asbestos free. If it is discovered that asbestos products are used, the bidder will pay all administrative, design, abatement and other costs associated with its removal and the installation of a new product where this abatement occurred. 5. The Facilities Office has a list of hazard chemicals in the buildings available for inspection by the bidder. Any hazard chemical used by the vendor must have a MSDS available on the job-site for inspection by District employees. If any hazard chemicals are part of the system, then the District must receive a MSDS for our permanent record. 6. The District reserves the right to stop work with the contractor at any time the District feels that production or workmanship is less than deemed satisfactory by the District. If the problem cannot be resolved, the District will contact the next contractor. 7. The District will pay 95% of earned progress payments and retain 5% to assure faithful performance. All payments will be paid to the contractor within 30 days of contract completion assuming the contractor has faithfully performed the contract in full (A.C.A & 604).

Prospective bidders are responsible for site verification of dimensions. Proposals will be evaluated on conformity to specifications, past history with the district, proximity of the company to the district, and price. The successful bidder will be notified as soon as possible after bids are opened and approved by the Board of Education. The Berryville Public School District reserves the right to reject any and/or all bids. Conditional award will be announced by the board after consideration of all items mentioned above. The conditional award will become final after the Arkansas Division of Facilities located in Little Rock, Arkansas, reviews the successful bidder s submitted drawings (Item 9 and Item 10, General Specifications) and notifies the district of approval to proceed with construction. THE AWARD IS NOT FINAL UNTIL DIVISION APPROVAL IS RECEIVED BY THE BERRYVILLE SCHOOL DISTRICT.
