1 Columbia County Health Care Center Added Amenities Columbia County Board Meeting on January 16th, 2013 Building Exterior Added Amenities 1. Upgraded nail-base roofing insulation to 3 thickness on Center Core area roof. 2. Revamped attic venting to accommodate thicker roof insulation. 3. Installed Ice & Water-shield Roofing underlayment on the entire roof, including the existing Canopy in lieu of only at the eves and valleys with 15 lb. felt on the remainder of the roof. 4. Removed and replaced shingles on the existing Canopy. 5. Replaced 281 sheets of roofing plywood that was deteriorated and structurally unsound. 6. Added ductwork enclosures to Core area structure to accommodate revised ductwork changes. 7. Installed Gutters and Downspouts to the roof eaves around the new porch areas. 8. Reinstalled the cast stone masonry cap at area well and repaired caulking as needed. Center Core Area/Entire Facility Added Amenities 1. Replaced the 1 x 1 adhered tile ceilings in 7 additional rooms. 2. Replaced the flooring in all four connecting corridors. 3. Replaced the flooring in the entire Center Core Lobby/Multi-Purpose room in lieu of only a portion of the room. 4. Added decorative floor design in Center Core Lobby/Multi-Purpose room floor. 5. Added a door closer to the storage room door. 6. Remediated damaged asbestos pipe insulation and replaced the pipe insulation with fiberglass 7. Replaced numerous HVAC grills and diffusers that were outdated, dirty and rusted. 8. Replaced scheduled electric heating coils with new hot water heating coils. 9. Upgrade the UV System from what was previously scheduled to accommodate existing conditions. 10. Upgraded flooring in entrance vestibule to Walk-Off Carpet entrance mat carpeting. 11. Painted existing supply diffusers that were in good condition but needed re-painting. 12. Repaired malfunctioning heating control value in Administrators office. 13. Reworked Wing Ventilation system so that the scheduled exposed ductwork was concealed. 14. Replaced existing 1 x 1 ceiling tile with 1 hr. fire rated drywall ceilings in connecting corridors. 15. Upgraded the sprinkler heads throughout the Center Core area. 16. Upgraded exterior vinyl windows to aluminum storefront windows. 17. Added additional smoke detectors in between wood beams per revised state submittal review. 18. Added new Plastic Laminate Wall Panels to the interior of the Elevator. 19. Added new hardwood maple handrails to the entrance areas at Almond and Birch wings. 20. Added wall protection systems to all the drywall wall locations throughout the Multi-Purpose Room walls and connecting corridors.

Removed, repaired and painted the walls where the out dated ceramic tile was in the existing 17. Added additional TV blocking inside new walls in Resident Rooms for owner wall mounted flat screen TV's. 18. Added Self-Locking raised panel wood medicine cabinets in Toilet rooms in lieu of originally 19. Upgrade Toilet Room trim casing from scheduled architectural vinyl composite casings to solid hardwood maple trim. 20. Replaced several locksets (4) that were not working. 21. Replaced window in Tub Room with aluminum storefront window system. 22. Painted every room in the wing, including all the storage and equipment rooms not originally scheduled for painting. 23. Replaced all the attic access panels with 1 hour fire rated access panels. 24. Installed 1 hour fire rated access doors in attic smoke separations. 25. Installed additional power in several locations in the Nurses Station for more work stations. 26. Removed existing Nurse Call systems thru-out entire wing. 27. Installed new junction boxes throughout all the units for the new Nurse Call system. 28. Installed new drinking fountain in corridor.

Installed new vinyl sheet flooring and vinyl base in the Utility Room. 36. Installed Modine Unit Heater supplied by the County as a backup emergency heat source in the 37. Added corner guards throughout the entire wing in the corridors and high traffic corners. 38. Install added ductwork to basement air handling unit ductwork to extend heating and cooling capabilities down to the Training Room at the end of the wing because there wasn't any supply air in that area. This allowed the unit to better utilize excess capacity available from the unit and to resolve a climate control issue that was preexisting. Work included relocation of existing plumbing lines and new diffusers as required. Birch Boulevard Wing Added Amenities in the Shower room, masonry work only. 5. Replaced 2 Existing light fixtures in the two entrance corridors with new fixtures to match the rest of the building. 40. Added receptacles and power for Ice machine and coffee maker. 41. Added a receptacle and power for a garbage disposal. 42. Revised several receptacles to GFCI receptacles per local building inspector in revised servicing area. 43. Removed the existing air exchanger in the Mechanical Room in Willow Court Wing. 44. Repaired the ductwork insulation in the Kitchen Hood which included removing the insulation board that had slid down and blocked the make-up air intake pathway. This resolved a majority of the lack of make-up air issues in the Kitchen area and helped eliminated a negative air pressure problem in that area. 45. Added a 1 hour rated Attic Access panel in the Kitchen Ceiling. 46. Install new dampers in the ductwork above the Kitchen Roof for better airflow. 47. Added a masonry opening in the exterior wall and modified a doorway at the loading dock area for the installation of the trash compactor. 48. Repair 8 existing leaking plumbing pipes throughout the basement area.
