1 Portage Trail 232/223(f) Refinance and 4% LIHTC Project 45 Cathedral Lane, Cuyahoga Falls, Ohio Scope of work January 19, 2016 Project Summary: Portage Trail is a 13 story 183 unit circular apartment building with mixed use of Assisted and Independent Living for Seniors. The building recently underwent renovations of floors one thru six converting those levels into Assisted Living and providing a first floor addition for dining with commercial kitchen. This project entails select renovations to floors seven (7) thru thirteen (13). Units are comprised of 21 one bedroom and 91 efficiency units. Renovations include; corridors, ADA upgrades to seven one-bedroom living units, major electrical and hot water systems replacement. Restructuring of seven (7) stacked one bedroom unit bathrooms along with minimal restructuring of the kitchen is required to comply with current ANSI type A / UFAS standards for accessibility. R/R = remove and replace Asbestos Abatement: See Asbestos Survey and Specification. Abatement for seven (7) onebedroom units floors R/R VCT flooring and mastic. 2. Remove all existing texture from ceilings. 3. Remove kitchen sink. 4. Remove existing insulation at exposed piping. Asbestos Abatement (add-alternate): Abatement for balance of units floors Remove all unit entry doors 2. R/R VCT flooring and mastic. 3. Remove kitchen sink. Site: 1. Repair all damaged and deteriorated asphalt paving and concrete curbing. 2. Mill all asphalt paving 1 ¼. Repair base and install 1 ¼ asphalt wearing course. Restripe parking and drop-off area. Provide provisions to include later seal coating and striping after curing. 3. Reconfigure HC parking and replace walks at rear lot to meet current accessibility standards. 4. Construct 12x12 wood framed gazebo with slab on grade, asphalt shingles and two park benches (match existing) at rear yard green space. Provide accessible walk. 5. Add / enhance landscaping at main entry drive, existing one-story addition and new gazebo. Building exterior:

2 1. Tuck point approximately 25% of the existing mortar joints that are scattered throughout the elevations. Areas to be tuck pointed 100% will be all shelf angle joints at the top 1 foot of

3 brick where it protrudes out under the EIFS at the top of the building at either side of the window bay and rowlock coarse at the second floor area. 2. Replace defective or missing brick at the 2 nd floor rowlock as needed. 3. Install sealant at the top of the building where the EIFS butts the drip edge going vertically and horizontally 100%. 4. Prep and coat 245 window sills. Non-Structural: 1. Repair vertical cracks at interior g.w.b. walls within units 804, 809, 904, 1007, 1008, and New Accessible one Bedroom Units: (7 units) 1. R/R unit entry door and metal frame. Provide closer. 2. Demolish existing bathroom, bedroom closet and segment of kitchen walls to limits shown on drawings. Existing construction consists of wire frame metal studs with gypsum / plaster finish. 3. Infill square opening in floor/ceiling with 2hr rated concrete full depth of floor slab where tub has been removed. 4. Infill floor/ceiling penetration for plumbing supply and waste lines at bathroom unit separation wall. 5. Install new 36 and 42 grab bars in bathroom. Install new recessed medicine cabinet and towel bars. See plumbing below. 6. Install sloped floor skim coat to new shower threshold prior to flooring. 7. Construct new reconfigured accessible bathroom with reduced size bedroom closet. 8. R/R kitchen cabinetry with new countertop 33 3/4. Top of sink rim to be 34 max. Wall mounted cabinets to be lowered to 48 to top of bottom shelf. Countertops to be molded edge with 6 self-edged backsplash. Locate GFCI receptacles in backsplash. 9. R/R single bowl kitchen sink, supply and waste lines. Provide and install anti-abrasion piping insulation. 10. Provide and install Whirlpool refrigerator model W5TXEWFWQ, 20 Amana electric range model AEP200VAW, white grease shield. 11. R/R all flooring: Provide vinyl plank flooring in kitchens and baths. Carpet with ¼ pad in living area and hall to kitchen. All flooring to extend under kitchen cabinets and removable equipment. R/R vinyl wall base. 12. R/R lighting throughout with energy star rated fixtures. 13. R/R all door hardware including entry. Based on Schlage AL series with Neptune lever handle design Balance of units: Efficiency Units: (91 units) 1. R/R flush wood 20 min. rated unit entry door and metal frame. 2. R/R all vinyl resilient flooring and base in kitchen and bathroom with vinyl plank: Carpet / vinyl base to remain. R/R flooring transition strips at kitchen and bath. 3. R/R kitchen cabinetry and counters. 4. R/R single bowl kitchen sink, supply and waste lines. Provide ADA compliant low flow faucet.

4 5. R/R toilet with new ADA compliant low flow fixture. 6. R/R bathroom sink, countertop and vanity base, supply and waste lines. Provide ADA compliant low flow faucet. 7. R/R all unit interior doors. 8. R/R unit lighting fixtures. 9. R/R 24 Energy Start refrigerator. 10. R/R 20 electric range with front controls.

5 Standard one bedroom units: (14 units) 1. R/R flush wood 20 min. rated unit entry door and metal frame. 2. R/R all vinyl resilient flooring and base in kitchen and bathroom with vinyl plank: Carpet / vinyl base to remain. R/R flooring transition strips at kitchen and bath. 3. R/R kitchen cabinetry and counters. 4. R/R single bowl kitchen sink, supply and waste lines. Provide ADA compliant low flow faucet. 5. R/R toilet with new ADA compliant low flow fixture. 6. R/R bathroom sink, countertop and vanity base, supply and waste lines. Provide ADA compliant low flow faucet. 7. R/R all unit interior doors. 8. R/R unit lighting fixtures. 9. R/R 24 Energy Star refrigerator. 10. R/R 20 electric range with front controls. Common areas Corridor, Laundry room, Trash chute room: 1. R/R all lighting with Energy Star fixtures. 2. R/R all flooring: Provide vinyl resilient flooring in laundry and trash rooms. Carpet with no pad. R/R vinyl base. 3. Paint all areas. 4. R/R all emergency lighting and exit signage. Add emergency lighting to elevator lobby. 5. R/R all handrail. Install 5 ½ painted poplar handrail with decorative band. Install 5 ½ chair rail opposite wall. 6. Skim coat with plaster inner corridor core concrete walls for smooth, level 4 or greater finish. 7. Install a continuous metal framed g.w.b. soffit below existing suspended g.w.b. ceiling and inner core corridor wall for new electrical feeders to units. 8. Install faux beams crossing corridor between each two units for new electrical feeders to units. 9. Provide one frontloading washing machine (by owner) Electrical general: 1. Provide new U.G. wiring for lighting at new gazebo. Electrical Switchgear: 1. Remove two transformers within the building. 2. Modify existing openings as required for transformer removal. 3. Install new pad mounted transformer at exterior. 4. Extend new primary from existing location Bore under pavement to new transformer. 5. Provide new 5000 amp switch gear with ground fault protection. 6. The existing self-supporting steel mezzanine shall remain. Remove existing switchgear after new switchgear is operational (existing cabinet assembly at underside of deck may remain in use as a junction box). 7. Work shall include all rigging and demolition of old transformers and switchgear. 8. All work shall be performed per NEC. Electrical distribution panels floors 7, 10 & 12: 1. R/R distribution panels and conductors.

6 2. R/R feeders to units on floors 7 thru The existing feeders to the unit load centers will be abandoned in place. New feeders in conduits will be installed in the corridors below the ceiling. Conduits to be hidden inside premanufactured moulding. 4. Feeders into the units will penetrate the gyp/plaster corridor wall and be run in wire mould along the inside corner of the kitchen ceiling, soffit and then down inside the bathroom wall to the new unit load center. Electrical Corridors: 1. R/R existing light fixtures with new fixtures Energy Star fixtures. 2. R/R receptacles and switches. 3. Remove existing emergency and exit light fixtures. R/R with new fixtures and augment with additional fixtures to meet current codes (1 footcandle. minimum). Add emergency lighting at elevator lobby. Unit electrical (new accessible units): 1. R/R existing light fixtures with new Energy Star fixtures. 2. Install horn / strobe in unit living room, bedroom and bath connected to fire alarm system. 3. R/R existing receptacles and switches. 4. R/R smoke detectors with wired. 5. R/R load center. 6. Install new wireless emergency call system separate from floor 1-6. Provide one call station in sleeping area and one in bathroom. Unit electrical (balance of units): 1. R/R existing light fixtures with new Energy Star fixtures. 2. R/R load center. 3. R/R smoke detectors with wired. 4. Provide GFCI is bathroom. 5. Install new wireless emergency call system separate from floor 1-6. Provide one call station in sleeping area and one in bathroom. Sensory Impaired units (total of three): 1. Efficiency units total of three (3): Install horn / strobe in unit living room, bedroom and bath connected to fire alarm system. Install battery powered doorbell with strobe indicator.

7 Backup Generator: 1. Remove existing diesel generator (100kW, 120/208v, 3phase, 4 wire). 2. Transfer existing loads to newer generator. Loads and capacity has been confirmed by Speelman Electric. Hot Water System: 1. Provide low pressure regulators and gas piping within the facility to new gas fired equipment. 2. Configure two existing 750 gallon electric fired tank type water heaters to operate as storage for new gas fired water heaters. 3. Install two new 199 MBH (kbtu/hr) gas fired water heaters. 4. Provide all related piping and supports (no plastic piping due to high temperature hot water). 5. Provide all related combustion air and exhaust ventilation louvers, ducts and supports. Plumbing: 1. Accessible one bedroom units (7 units) a. R/R existing bathroom sanitary stack floors 7 thru 13. b. Install new single bowl kitchen sink with ADA compliant low flow faucet. c. Install new ADA compliant low flow toilet. d. Install new bathroom sink with low flow ADA compliant faucet. a. Install new 3-piece Clarion model MP6436BFKD34 barrier free shower with integral grab bars. b. Install square floor drain in bathroom 2. Efficiency and standard one bedroom units. (105 units) a. R/R single bowl kitchen sink, bathroom sink, supply and waste lines. Provide ADA compliant low flow faucets. b. R/R toilet with new ADA compliant low flow fixture. Plumbing 1 st floor: 1. Inspect and jet-spray sanitary line and sewer main to remove grease and sludge buildup. HVAC: 1. R/R all unit PTACs. 2. Floors 7 thru 13: R/R corridor makeup air grilles. Install 12x48 adjustable supply grilles. 3. Floors 4 and 6: Install 7 adjustable exhaust grilles per floor. End of Work Scope
