2 SUMMARY The Mahid Apparel Ltd. factory is a ten (G+9) storied building with one basement floor. The building was constructed in 2011, and factory began production in November, The building has been approved for 9 storied mixed use building purpose. During the inspection the factory had approximately 840 workers. The generators and substation are located in a separated building owned, operated and maintained by Jaj Spinning Mills Ltd, which are sister concern of JAJ BHUIYAN Group. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which would be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. The implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design/installation constraints, shall be submitted to the Accord for an approval. MAHID APPAREL LTD. Page 2 of 19

3 FINDINGS AND FINDING NO: E- 1 CATEGORY: DOCUMENTATION Electrical Single Line Diagram (SLD) is unavailable. Have a qualified engineer create an as-built electrical SLD mentioning all the required information, and get it reviewed by Accord. PRIORITY: P1 REMEDIATION TIME FRAME: 12 WEEKS FINDING NO: E- 2 CATEGORY: MAINTENANCE TEST/ DOCUMENTATION Thermographic scanning of the entire electrical system has not been tested and recorded. 4 FINDING NO: E- 3 CATEGORY: MAINTENANCE TEST/ DOCUMENTATION Insulation resistance test of electrical equipment is not performed. Insulation resistant test of all the cables must be performed once every 2 year cycle and recorded (this requires a complete power shut off). REMEDIATION TIME FRAME: 10 WEEKS FINDING NO: E- 4 CATEGORY: EARTHING SYSTEM Earth Pit resistance record is unavailable. Record earth pit resistances for all the earth pits, and do it once a year. Hang this instruction near all electrical equipment (LT panel, MDB, FDB, DB, SDB) on a visible location. PRIORITY: P3 REMEDIATION TIME FRAME: 3 WEEKS FINDING NO: E- 7 CATEGORY: TRANSFORMER ROOM Transformer Oil Test report is unavailable. Check the transformer oil condition by performing oil test, this must be done twice a year and records kept. REMEDIATION TIME FRAME: 10 WEEKS FINDING NO: E- 8 CATEGORY: Lightning Protection & Earth Lightning Protections System (LPS) is inadequate for the factory. 6 FINDING NO: E- 9 CATEGORY: TANSFORMER ROOM Transformer area is not separated from the substation room. The transformer must be installed with barrier walls between transformer and other panels. The walls must be fire resistant and should have height up to the ceiling. The wall should have the provision for necessary ventilation and fire rated door on required side. Transformer area. REMEDIATION TIME FRAME: 12 WEEKS FINDING NO: E- 10 CATEGORY: TANSFORMER ROOM HT cables laid on concrete floor. Provide cable ladder made of noncombustible material to support the HT cable. Excess length of cable may be supported by providing cable tray at safe height raised from the floor. Clearly identify each earth pit, and mark it for periodic maintenance purposes. PRIORITY: P3 Earth Pit not identified. FINDING NO: E- 14 CATEGORY: SUBSTATION ROOM Maintenance persons use substation room as their working station. Substation room must not be used as a working station. The generator room must be installed with barrier walls between generator room and substation room. The walls must be fire resistant and should have height up to the ceiling. Provide covered metallic tray/ladder/riser to support and protect the cables. Ensure at least 1.07 meter (or equal to the width of board, whichever is higher) working clearance in front of each panel. Provide capacity information labels (Maximum current rating, no of circuit breakers etc.) for distribution boards. PRIORITY: P3 No capacity information label. REMEDIATION TIME FRAME: 12 WEEKS FINDING NO: E- 28 Non-graded and inadequate rubber (insulation) mat on the working are of distribution board (Typical). Put electrical insulation (rubber mat) on the working area of each electrical installation (Transformer/LT panel/mdb/db/sdb/ other manual operated machineries). PRIORITY: P3 Non-graded rubber mats are provided in front of distribution boards. FINDING NO: E- 29 Panel doors are not connected with earth (Typical). 14 FINDING NO: E- 30 Distribution Board's top/bottom is left open (typical issue). Seal each distribution board's top/bottom; and use cable glands holding/supporting cables. Panel base plates removed to allow cable entry. FINDING NO: E- 31 Distribution panel is filled with lint/dust (Typical). Ensure inside distribution boards/panels are always kept clean. Put a purposely made phase separator (rubber type) between two phases; also terminate cables by proper sized cable lugs and cover cable lugs by heat shrink. Standard phase separator missing. REMEDIATION TIME FRAME: 6 WEEKS FINDING NO: E- 33 Cables are connected without lugs at MCCB/ MCB terminal (Typical). Cables must be connected to terminals by cable lugs according to the cable size. Cable termination without lugs at MCCB terminal. FINDING NO: E- 34 Multiple cables terminated at MCCB terminals (Typical). Terminate each power cable at single terminal and use proper sized cable lug. Multiple cables termination is allowed if all are soldered properly and powering to a single circuit. 16 FINDING NO: E- 35 Multiple cables are connected without lug at bus-bar terminal (Typical). Each cable must be connected in a single point of bus bar with proper size of cable lugs. PRIORITY: P1 Cable termination without lugs. FINDING NO: E- 36 CATEGORY: SWITCH & PANEL BOARDS Three phases MCCB connected to control one phase. Check and redesign the requirements to control the circuits. If three phase control is not required, then replace with suitable control devices. Three phase circuit breaker used as single phase breaker. REMEDIATION TIME FRAME: 1 WEEK FINDING NO: E- 37 Panel is not provided with earth bus bar. 17 FINDING NO: E- 38 Wires in panel are spliced and insulated with PVC insulating tapes (Typical). Use PVC or porcelain cable connector and PIB tape wound around it for joining cables inside panel. Splicing cable joints found inside panel. REMEDIATION TIME FRAME: 3 WEEKS FINDING NO: E- 39 Un-terminated wire present inside the panel (Typical) Need to remove from the panel. PRIORITY: P1 REMEDIATION TIME FRAME: 1 WEEK Un-terminated wire. FINDING NO: E- 40 CATEGORY: CABLE & CABLE SUPPORTS Cables entering or leaving distribution panel are not supported and protected. Install cable tray or riser (up to the panel base plate) to support the cables entering and leaving the panel boards to reduce cable strain on cable termination point. 18 FINDING NO: E- 41 Panel/Distribution boxes are inaccessible or cannot be opened to perform any maintenance work. Take necessary initiative thus every panel/distribution board can be easily accessible. PRIORITY: P3 Distribution board installed at a height out of normal reach. FINDING NO: E- 42 CATEGORY: WIRINGS Cable duct/channels have are excessive lint/dust. Also, no cover on cable duct (Typical). Clean all the cable channels/ducts and cover them with (checkered/perforated plates may be used for the bottom). Cover is not provided on cable duct.
