Main Street Chamberlain, SD Scope of this Addendum: To all bidders and all others to whom drawings and specifications have been issued by Architecture Incorporated or Puetz Corporation. Acknowledge receipt of this addendum by listing its number and date in the bidders Form of Proposal. Failure to do so may subject bidder to disqualification. This Addendum forms a part of the Contract Documents. It modifies them as follows: GENERAL ITEMS: 1) Table of Contents: a) Add SECTION RESIDENTIAL APPLIANCES. This section is in the project manual; was left off Table of Contents list. 2) Bid Packages: a) Bid Package #3: i) Add Section RESIDENTIAL APPLIANCES. ii) Remove Section COLD FORMED METAL FRAMING. b) Bid Package #4: Remove material, accessories and installation of the following systems from the Scope of Work: i) Section THERMAL INSULATION, 2.1 A. 4.b. ¾ Cavity Wall Insulation. ii) Section WEATHER BARRIERS. c) Bid Package #8: Add material, accessories and installation of the following systems from the Scope of Work: i) Section THERMAL INSULATION, 2.1 A. 4.b. ¾ Cavity Wall Insulation. ii) Section WEATHER BARRIERS. iii) Exterior wall sheathing will be changed from ½ glass mat gypsum to ½ OSB sheathing. iv) Add Section COLD FORMED METAL FRAMING. 2 3) SECTION DECORATIVE METAL RAILINGS: a) Paragraph 2.1. A. 1.: Basis-of-Design Product: Change 9/16 horizontal stainless steel rod system to Cable Connection Ultra-tec - Cable Railing system. Cable to be ¼ diameter 1x19 stainless steel strand. Provide complete cable rail system with components and fittings as required for a complete tensioned system. Balance of vertical posts and hannndrails to be same materials as detailed in the drawings. b) Provide Egress Barrier rope at top of stair run to basement level: ½ diameter 1x19 stainless steel rope, with swivel sleeve on end attached to north side wall with clip plate bracket. The free end of rope is to be finished with a hook sleeve that can attached to an eye bracket mounted on the south top vertical railing post. The design intent is to provide a code-required barrier to the basement stair that is easily unhooked for service access. 4) SECTION SHEATHING: a) Paragraph 2.4 B.: Change Glass-mat Gypsum Wall Sheathing to: APA /16 OSB Sheathing. 5) SECTION GLAZED ALUMINUM CURTAIN WALLS: a) Paragraph 2.2 B. Clarification: Steel reinforcement to be placed in curtain wall framing sections, as determined by manufacturer s Delegated Design engineer, to be provided & installed by curtain wall manufacturer. 6) DRAWING SHEET 5.3 STAIR DETAILS: a) Details 9, 10/5.3: Change horizontal 9/16 SS rods at 4 o.c. to ¼ dia. SS cable at 3 ¼ o.c. b) Details 9, 10/5.3: Delete Powder Coat finish. All plain steel to be painted per Section ) DRAWING SHEETS 5.5, 5.6, & 5.7 SECTION DETAILS: a) Change Fiber-cement vented soffit notes to read Fiber-Cement Non-Vented Soffit with standard dividers and trim pieces. b) Change exterior wall sheathing from 1/2 glass-mat gypsum to ½ OSB Sheathing. 8) DRAWING SHEET 5.8 ROOF PLAN a) Insulation Legend, first note: Sloped structure condition applies only to raised, sloped roof section at northwest corner. The balance of roof is not sloped structure. Slopes are to be achieved with tapered insulation. 9) DRAWING SHEET 6.1 REFLECTED CEILING PLAN a) Ceiling Legend: Wood ATC Ceiling is to be Base Bid. Delete reference to Bid Alternate. STRUCTURAL ITEMS: See attached Addendum #2 from Albertson Engineering for additional Structural Steel submittal & quality assurance requirements. 3 ELECTRICAL ITEMS: SPECIFICATION ITEMS: 1) (Add Alternate) Replace the existing Dukane intercom system serving the adjacent elementary school building with new. Scope to be issued in next addendum. 2) Public Address System - Section a) Clarifications- i) The PA System specified to be installed throughout the first and second levels of the clinic is to serve as a means to broadcast BOTH campus emergency and school intercom all call announcements. These announcements are to originate from two SEPARATE sound systems. ii) A PA System Mixer/ Amplifier shall be installed in a 3-part sound cabinet, to be mounted on the west wall of Mechanical Room 125 (see note 12 on the room detail). The mixer shall accept audio inputs from BOTH the campus emergency announcement system and also the school intercom system. This contractor shall provide audio isolation transformers, lightning/ground protection circuitry and other electronics necessary to accept & condition line level paging from these owner provided existing audio sources for introduction into the new paging headend. iii) Campus Emergency Announcement- audio feed is to appear as a line level input at the telephone demarcation point in the Clinic Building. Owner will make arrangements, outside the scope of this project, for that audio signal to reach the new Clinic building. iv) School Intercom All Call. This contractor shall, under the scope of THIS project, extend audio cabling from the existing School Intercom System (found in office of adjacent school building) over to the new paging headend. This contractor shall locate and connect to the existing School intercom system s all call paging buss and condition that audio signal, as necessary, for introduction into the new Clinic PA Sound System. v) Existing School Intercom is by Dukane, subject to change should the Owner elect to proceed with replacement of the School Intercom. 3) DRAWING SHEET 9.3 a) In reference to the nurse call S Symbol associated with the nurse call device to be provided in each of the exam & single/double rooms, deivce shall be a staff assist pushbutton station with call assurance light and cancel button. b) Add Duty Station on corridor side of south wall outside Lab Rm 133. c) Annunciator Panel i) At main Nurse Station area 112- Eliminate the Master Station shown at desk ii) At main Nurse Station area 112- On east back wall, behind the desk, add a Multipoint Visual/ Tone annunciator panel. Size the panel to annunciate calls by room, with spare capacity to accommodate at least 20% future growth of system. 4) DRAWING SHEET 9.3 a) In reference to the nurse call system: i) Clarification- Toilet Room 228 Nurse Call. Anytime the pull cord station in room 228 is activated, the light in the hallway outside the room shall light, tone shall start on the local duty station and call shall also appear (by light and tone) as a separate point on the main annunciator panel at the first floor Nurse s Station 112. 4 b) Add Duty Station on corridor side of the south wall outside Toilet Room ) DRAWING SHEET 9.6 a) In reference to keynote #6, distance for run length is approximately 350 feet. Field verify exact distance and routing via tunnels. ELECTRICAL SUBSTITUTIONS AND PRODUCT OPTIONS The following material or equipment furnished by the manufacturers listed may be substituted as equal, providing that each item, material and piece of equipment conforms to the design and requirements of the Drawings and Project Manual. This Addendum forms a part of the Contract Documents. It modifies them as follows: SECTION STRUCTURAL STEEL of the specifications shall be modified as provided below. Shop Drawings: Show fabrication of structural-steel components. 1. Include details of cuts, connections, splices, camber, holes, and other pertinent data. 2. Include embedment drawings. 3. Indicate welds by standard AWS symbols, distinguishing between shop and field welds, and show size, length, and type of each weld. 4. Indicate type, size, and length of bolts, distinguishing between shop and field bolts. B. Welding certificates. C. Steel Fabricator: For non AISC certified fabricators, submit a list of projects demonstrating a minimum of 5 years of experience in the fabrication of structural steel for projects of similar size and complication.

6 1.4 QUALITY ASSURANCE A. Fabricator Qualifications: One of the following criteria shall be met by the structural steel fabricator 1. A qualified fabricator with a minimum of 5 years of experience in the fabrication of structural steel for projects of similar size and complication. 2. A qualified fabricator who participates in the AISC Quality Certification Program and is designated an AISC-Certified Plant B.
