3 UNDERWATER INSPECTION REPORT SUMMARY The substructure units inspected at Bridge No , Piers 1 and 2, were found to be in good condition with no defects of structural significance observed. The concrete shafts and exposed footing were typically smooth and sound with light scaling present above the waterline. The channel bottom consisted of sand and gravel around the substructure units and a moderate accumulation of timber debris was observed at the upstream noses of both piers and along the east and west faces of Pier 2. Footing exposure was observed at Pier 1 with up to 1 foot of vertical face exposure. INSPECTION FINDINGS (A) The channel bottom material consisted of sand and gravel with random cobbles throughout. Overall, probe rod penetration was negligible. (B) Scaling was observed around the entire pier shaft of both piers typically extending from the waterline up 1 foot with a maximum penetration of 1/4 inch. The scaling was heaviest at the upstream nose where it extended up to 7 feet above the waterline. (C) Below the waterline the concrete of the pier shafts and exposed footing were typically smooth and sound with random minor areas of poor consolidation with penetration up to 1/4 inch. (D) Footing exposure was observed at the upstream nose, along the upstream quarter of the pier on west side, and along the entire east side of Pier 1 with up to 1 foot of vertical face exposure. (E) A moderate accumulation of timber debris, consisting of logs and branches up to 12 inches in diameter, was observed from the channel bottom up 2 feet around the upstream noses of Piers 1 and 2, and along the east and west faces of Pier 2. RECOMMENDATIONS (A) Monitor timber debris accumulations around Piers 1 and 2 during future inspections, and if found to be progressing, removal may become warranted, to eliminate excessive lateral loads on piers and the potential for scour. (B) Reinspect the submerged substructure units at the normal maximum recommended (NBIS) interval of sixty (60) months. Contractor: Contractor Job Number: Collins Engineers, Inc Page 3 of 17

6 UNDERWATER INSPECTION INSPECTION PROCEDURES The routine underwater inspection of Bridge (College Drive over Mississippi River) was completed on June 8, The underwater inspection was conducted from a 14 ft boat. The inspection was conducted by a team consisting of a Professional Engineer Diver with a valid MnDOT Team Leader certification, a backup diver and dive tender. The inspection utilized commercial dive equipment and techniques (SSA and/or SCUBA) in accordance with OSHA regulations. Channel bottom profiles were taken along the upstream and downstream faces of the bridge and around the periphery of substructure units to determine the presence, location, and area of scour. The bridge elements inspected consisted of two reinforced concrete piers. According to design drawings, Piers 1 and 2 are founded on steel H-piles. Inspection procedures followed FHWA guidance and the MnDOT Bridge and Structure Inspection Program Manual with channel bottom probing to search for foundations. The maximum routine underwater inspection frequency is recommended to remain at 60 months based on those findings and risk factors. Also, inspection procedures should continue to follow the above approach and standard guidance with 100% Level I and 10% Level II intensity efforts. Page 6 of 17
