1 ADDENDUM ONE Partial Reroofing and Built-in Gutter Repairs January 11, 2013 This addendum forms a part of the contract documents and modifies the original specifications and drawings, dated January 2, 2013, as noted below. Acknowledge receipt of the Addendum in the space provided on the proposal form. Failure to do so may subject Bidder to disqualification. ITEM NO. 1 PREBID MEETING AND PREQUALIFIED BIDDERS The prebid meeting was held January 8, 2013 at 10:00a.m. A copy of the agenda, minutes and sign in sheet are attached to this addendum. The following contractors attended the mandatory prebid meeting and walk-through and are the only qualified contractors to bid this project. Add: Additive Alternate #1: In lieu of the 60 mil self adhesive rubberized waterproofing underlayment membrane installation on the steep slope roofs, the roofing contractor to install a 2 ply self-adhering 70 mil fiber glass mat reinforced SBS modified bitumen membrane, nailed along the head laps min. 16 O.C. and heat weld a ceramic coated torch grade modified bitumen cap sheet over the installed base sheet, with head laps nailed. 2 January 8, 2013 PREBID CONFERENCE MINUTES Mr. Joe Adams Facilities Design and Construction Tallahassee, Florida Partial Reroof and Gutter Repairs The prebid conference was held on Tuesday, January 8, 2013, at 10:00 a.m., at the Student Life Building on s campus. The following were present and signed in: Joe Adams, Project Director, FSU Greg Akridge, Oglesby Student Union Roberta Williams, FSU Purchasing Fredrick Ross, FSU Purchasing Hamp Stoutamire, Jenkins Roofing Elliot Jenkins, Jenkins Roofing Leo Stewart, Rowe Roofing Randle Bryant, Harrell Roofing Mark Ganskop, Old World Craftsmen Donny Strickland, Acme Roofing Randy Lewis, Project Architect, MLD Architects, Inc. Roosevelt Bivens, Project Manager, MLD Architects, Inc. The following was noted: 1. The Project Managers, Roosevelt Bivens, introduced himself, the Project Architect, Randy Lewis, and the FSU s Project Director, Joe Adams. The attendees introduced themselves. 2. The prequalification packages were collected and the sign in sheet was passed around. It was emphasized to print contact information clearly and legibly to avoid errors in communications. An addendum covering questions, concerns and clarifications discussed will be issued about one week prior to the bid. These prebid conference minutes are to be included as a part of the addendum. The scope of work was thoroughly explained (see attached agenda). 3. Tuesday 01/15/13 at 10 am has been set as the open time to revisit the building with subcontractors if needed. Mr. Akridge will provide access to the areas below the downleaders above the lay in ceiling. The bid opening time is 2:00 p.m. on January 24 nd, at the FSU Facilities Maintenance Office in the Mendenhall Building on FSU s campus. No late bids will be opened or considered.

3 4. Safety is of the utmost concern. The University has a zero tolerance for inappropriate behavior that will be enforced. It was stated this area is heavily populated and there shall be a ground safety man while working at the building perimeter edges. 5. The contractor is required to have a full time competent supervisor on the jobsite to oversee the work of all the trades, and who can correspond with the designated building s representative and the Architect. The building will be occupied during construction and the contractor s superintendent must closely coordinate around scheduled evening events. The contractor will schedule bi-weekly progress and coordination meetings. 6. All questions are to be directed to the Project Architect or Project Managers by fax or prior to the addendum issuance. The Addendum will note the extension if the time for question to January 17 th. There were no additional questions and the group walking the roof. Introduction of representatives: FSU Project Manager, Joe Adams Project Architect, MLD Architects, Randy Lewis Project Manager, MLD Architects, Roosevelt Bivens Sign-In Sheet: Print name, company name, phone and address. 2. Scope of Work: Discuss scope of work The reroofing and gutter replacement work of the, at Florida State University includes; removing the existing concrete roof tiles to be salvaged for reinstallation, wood battens and felts from the steep sloped roofs indicated down to the plywood deck, and removing the copper gutter liner, parapet wall base flashing, tapered insulation down to the existing corrugated metal deck of the built-in gutter. The work includes installation of new rubberized waterproofing membrane underlayment, new pressure treated wood battens and reinstallation of salvaged concrete roof tiles and new concrete tiles to match. It also includes installing a new tapered gutter system, cast iron roof drains, and mineral surface modified bitumen membrane roofing and flashing system in the areas indicated. If more or less work is required, the following unit prices shall apply: Unit Cost: P.T. Plywood deck replacement (200 sf in base bid) + $ /sf 3 Rigid insulation replacement on steep slope (200 sf in base bid) + $ /sf 4. Special Aspects: Safety and coordination of access and staging/storage areas with Owner/Staff. Full time supervision by Contractor required. 5. Set up a time for the contractors to revisit the site with subcontractors. 6.
