Please see the Habitability Booklet under the General Info setion of this site, for more information on the form that needs ompleted below. Dek and porh plans- must have minimum 3' X 3' landing and be self-supporting. If full basement indiate type of provided means of egress. Provide omplete drawings of any strutural work (suh as a garage) to be performed in the field and was not fatory inspeted. If the Manufaturer s Installation Instrution Manual is not available, then the A Existing Manufatured Home Standard must be followed. (Continued)

5 FOOTER OPTIONS: (Chek one) INSTALLATION REQUIREMENTS AND METHODS Conrete Pier Footings Round Provide 36 deep minimum frost protetion when skirting is installed 48 without skirting. For piers spaed no more than 8' apart, holes should be 28" diameter, unless soil bearing apaity is greater than 1,500 lbs. per square foot. FOUNDATION OPTIONS: (Chek one) Main Support Piers Generally within 2' from eah end, and spaed 8' apart. A single stak onrete blok pier apaity is 8,000 lbs., up to 36". Piers higher than 36' require double bloks, interloked. No mortar required for piers less than 80" high. Cap bloks must be full size (16x16 pier requires 16x16 ap blok). Full Foundation Plan and speifiations required. Cross setion submittals are available for your drawings. Verify that all imposed strutural loads are properly supported. ANCHORING METHODS (Chek one) Auger and Strap Type: Installed within 2' of the end of the home, then generally 11 spaed apart. Install below frost line. Closely follow manufaturer s installation instrutions. Alternative systems - Vetor systems, OTI system. Any stamped engineered or alternative system approved by the manufaturer. 11 Pennsylvania Habitability Guidelines for Reloated Manufatured Homes November 2013 Data Plate Information The data plate is typially loated inside a kithen abinet, in a bedroom loset or on the eletrial panel-board over area. If the data plate is not available, additional inspetions may be required to determine if the home was onstruted for the Pennsylvania limate. TYPICAL DATA PLATE 1. Identify the date of manufature. 2. Identify the serial number. (Cross referene to the front rossmember of the hassis.) 3. Identify Wind Zone (Pa. WIND ZONE 1) 4. Identify Roof Snow Load Zone. (Pa. SOUTH (20) ZONE) NOTE Pennsylvania design riteria: Home manufatured after OCTOBER 1994 CLIMATE ZONE 3. Homes manufatured between June 1976 Otober 1994 CLIMATE ZONE 2. 2

12 Pennsylvania Habitability Guidelines for Reloated Manufatured Homes November 2013 Habitability Cheklist for Reloated Manufatured Homes Use this heklist to aid in onfirming ompliane with the habitability guide for reloated manufatured homes. All items must be orret prior to oupany. A ompleted heklist must be submitted to the loal building ode offiial as part of the building permit proess. Home Address: Data Plate, Serial Number & Certifiation Label Loate the serial number from the front hassis rossmember and the ertifiation labels from the exterior siding. Cross referene to the data plate in the home. See page 2. Verify the home was ertified as a manufatured home and that it was designed and onstruted for the Pennsylvania limate zone. Floors Floor is struturally sound. Floor overings are seure and do not present tripping hazards. Floor insulation is properly installed in floor avity, espeially at aess areas. Bottom Board is pathed and seure. See page 5. Walls Walls are struturally sound, without holes, breaks or protrusions. Doors and windows are operational with proper glazing. See page 3. Doors are equipped with proper hardware. See page 3. Exterior Coverings Roofing material free from obvious defets, holes, et. Roof aps, vents, flashing, et. are properly installed. Fireplae himneys are the proper height (3 above the roof where it passes and 2 higher than any part of the building with 10 of the himney). Siding material is free from obvious defets, holes, et. and is properly hanneled or sealed around penetrations. Sanitary Failities Plumbing system is in good working order and free from defets, leaks and obstrutions. See page 4. Drain piping under the floor is properly sloped and supported every 4. See page 4. Water supply line rossover insulation and overs are properly installed. Hot water appliane is properly installed and in good working order. 6 Ventilation Clothes dryer exhaust duts terminate outside of the skirting rawl spae enlosure. See page 4. Kithen, bath and toilet ompartment fans are operational. For homes built after Ot 25, 1993, see page 4. Heating Heating failities are in working order. Registers and grills are in plae at all outlets and intakes for the heating system. Fuel Burning Applianes All vents, flu pipes, himneys, et. are properly installed, and are free from rust, damage or any ondition that ould result in a leak of ombustion gases into the home. See page 4. Fire-bloking is adequately installed where vents, flu pipes, himneys or other penetrations are present in the eiling or walls inside furnae and water heater ompartments. See page 5. Fuel supply piping is properly installed and supported. Fuel supply piping has been tested for leaks by qualified personnel. Eletrial Systems All eletri reeptales, swithes, juntion boxes, fixtures, et. are properly installed with appropriate over plates. See page 5. All eletrial rossovers are properly assembled and seured. See page 5. Operational test assured that all eletrial devies operate properly. Ground Fault Ciruit Interrupters (GFCI) where required, test and reset properly. See page 5. All exposed metal parts likely to be energized have been bonded. Proper smoke detetors/alarms plaement and operation. See page 3. Proper arbon monoxide alarm plaement and operation, if appliable. See page 4.
