1 ATTACHMENT B STATEMENT OF WORK SCADA STILLING WELL/PLATFORM REPLACEMENT STORMWATER TREATEMENT AREA (STA), PACKAGE INTRODUCTION AND BACKGROUND The South Florida Water Management District (District) has established locations for collecting data at specified locations throughout its 16 county boundary. These locations use stilling wells to collect information on stages, flows, water quality and other parameters. Over time, many of these locations have reached the end of their useful life creating an unsafe condition or have experienced other issues preventing the proper collection of the data. This project will result in the replacement of stilling wells and platforms allowing District staff to continue to collect required data in a safe and continuous manner. 2.0 OBJECTIVE The objective of this project is to utilize a qualified Contractor to replace stilling wells and platforms located at G377B. See attached Exhibit A Site Map, for location and directions to each site. 3.0 SCOPE OF WORK The Contractor shall furnish all equipment, tools, materials, labor and everything known and necessary to complete all required Work as specified in this Statement of Work and the attached Technical Specification Section The Contractor shall determine the method, details and means of performing the services. The District may provide additional guidance and instructions to Contractor s employees, subcontractors or hired workers where necessary or appropriate as determined by the District. 4.0 PRELIMINARY MATTERS 1. Before Starting Construction Following purchase order execution, prior to Construction Notice to Proceed (NTP), Contractor shall submit to District for its timely review on or before the Pre-Construction Conference: 1. An initial cost-loaded construction schedule in accordance with the attached technical specification A preliminary schedule of submittals listing each required submittal and the times for submitting it. 2 2. Pre-Construction Conference Upon purchase order execution, the Contractor shall schedule a pre-construction conference with the District Construction Manager (CM) and Project Manager (PM), and others as appropriate. The Contractor shall prepare an Itinerary for use during the preconstruction conference. The Itinerary shall include: 1. An introduction to the Contractor s employees and/or subcontractors involved with building the Work. 2. An initial cost-loaded construction schedule in accordance with technical specification A preliminary schedule of submittals listing each required submittal and the times for submitting it. 4. A detailed plan of how the Work will be accomplished. 3. Notice-To-Proceed The period of performance of this project shall commence upon the effective date of a Notice to Proceed (NTP) and continue for a period as indicated on the signature page of the purchase order. The Contractor shall not proceed with construction work until a Notice to Proceed is received from the District. 5.0 WORK BREAKDOWN STRUCTURE Contractor shall perform the following tasks: Task 1: Stilling Well and Walkway Replacement 1. Obtain a C key through the Districts online request page: 2. Notify the District Construction Manager five (5) days prior to starting any construction so that they can have the stage parameters set to test. 3. The Contractor shall cut down, and then entomb the old HW and TW stilling wells. The wells shall be filled with a combination of the broken up concrete (no rebar) and new concrete. The mix should be thick enough to prevent it from coming out the transfer tubes. 4. At a pre-determined location agreed upon by the District Project Manager and the Contractor, install a walkway in a northerly direction perpendicular to the canal. The walkway shall extend far enough into the canal to allow a lower measurement of 6.8 (this includes 2 into the canal bottom.) The deck shall be constructed in accordance with the plans and specifications. 5. Provide and install a 12 stilling well at the end of the walkway using current District installation standards. Due to the rapid current expected during high water the stilling well shall be attached at three places (handrail, walkway and waterline), and shall be buried in the ground a minimum of 2. 3 6. At a pre-determined location agreed upon by the District Project Manager and the Contractor, install a walkway in a southerly direction perpendicular to the canal. The walkway shall extend far enough into the canal to allow a lower measurement of 6.26 (this includes 2 into the canal bottom. The deck shall be constructed in accordance with the plans and specifications. 7. Provide and install a 12 stilling well at the end of the walkway using current District installation standards. Due to the rapid current expected during high water the stilling well shall be attached at three places (handrail, walkway and waterline), and shall be buried in the ground a minimum of 2. The well bottom shall be wrapped with well wrap to prevent the well from filling with dirt. 8. Provide and install wellheads on both stilling wells, and then provide and install junction boxes in the back right corner of the wellheads; ensure the junction boxes do not interfere with the wellhead operation. 9. Provide and install a piece of 1 thick Starboard in the wellheads over the wells; the Starboard shall be aligned so that an encoder wheel can be positioned in the center of the wells and not have the Starboard interfere with the encoder tapes. 10. Provide and run new conduit from the old conduit to the front right of the wellheads through a traffic box, to the point where the old conduit starts. 11. Provide and run new Belden 9883 from the RTU to the wellheads. Leave 5 of wire at each end for the SCADA technicians to work with. 12. Provide and install bollards 2 from the end of each walkway at 45 angles. 13. Provide and install a 10 X 5/8 ground rod next to the stilling wells, and then provide and run #6 solid ground wire to the wellhead. The wire shall be connected to the wellhead via a Burndy KC17B1 ground lug, which shall be provided by the Contractor; the ground lug can be used as one of the junction box tie down bolts. 14. For project survey and stilling well installation, refer to the plans and specifications. 15. Properly dispose of all leftover materials. Task 2: Weekly Reports Upon NTP, the Contractor shall prepare and a weekly report due Fridays by close-of-business (COB). The contractor shall the report to the CM and PM. Weekly reports shall include an updated list of: 1. Work Completed. 2. Work in Progress. 3. Work Planned using the approved cost loaded construction schedule showing how the work will be accomplished within the contract time. Issues or delays shall be recorded as they are encountered. 4 6.0 ADDITIONAL CONTRACTOR RESPONSIBILITIES Safety and Protection The Contractor shall be solely responsible for initiating, maintaining and supervising all safety programs and precautions in connection with the Work. The Contractor shall comply with all applicable Federal, State and local laws, regulations, ordinances and standards pertaining to the safety of its employees, subcontractors and the Work while performing the Work pursuant to this Contract. The Contractor shall also be responsible for the compliance of its subcontractors, suppliers and individuals or entities hired directly or indirectly with all of the foregoing laws, regulations and standards pertaining to safety in connection with the Work. Contractor shall take all necessary precautions for the safety of, and shall provide the necessary protection to prevent damage, injury or loss to: 1. All persons on the Site or who may be affected by the works; 2. All the Work and materials and equipment to be incorporated therein, whether in storage on or off the Site; and 3. Other property at the Site or adjacent thereto, including trees, shrubs, lawns, walks, pavements, roadways, structures, utilities, and Underground Facilities not designated for removal, relocation, or replacement in the course of construction. Contractor shall ensure that all Subcontractors, Suppliers, and such other individuals or entities performing or furnishing any of the Work will be prohibited from using firearms, engaging in hunting, fishing, trapping, using illegal drugs or using alcohol either on the work site, on District property, or on any land adjoining the work site. The Contractor shall adhere to Occupational Safety and Health Administration (OSHA) requirements regarding the notification and documentation of injuries and fatalities that occur during the performance of this Work. The Contractor shall verbally notify the District s CM immediately of any fatalities or injuries requiring medical treatment beyond first aid that occur arising out of the Work. The Contractor shall submit a written accident report, signed by the Contractor, describing the details of the accident, fatality or injury and the measures being taken to prevent a recurrence to the District s CM within 5 working days of the event. The Contractor shall verbally notify the District s CM of all accidents that occur which result in property damage during the Work the same day the accident occurs. Emergencies In emergencies affecting the safety or protection of persons or the Work or property at the Site or adjacent thereto, Contractor is obligated to act to prevent threatened damage, injury, or loss. Contractor shall give District prompt written notice if Contractor believes that any significant changes in the Work or variations from the Contract Documents have been caused thereby or are required as a result thereof. Contractor shall be responsible for providing first aid and medical care in accordance with applicable laws and Page 4 of 8, SCADA Stilling Well/Platform Replacement (STA), Statement of Work

5 regulations. The Contractor shall secure or remove from the site, prior to a storm event, any materials or equipment which could cause bodily injury, damage to the Work, the District's installations and/or public or private property. No Contractor equipment may be parked within 100 feet of any District facilities. In the event of the issuance of a Storm Warning, the District will attempt to notify the Contractor, however, the Contractor is responsible for preparing for a storm event. The Contractor shall take the necessary precautions to protect the walking and motoring public from harm due to construction activity. Reporting and Resolving Discrepancies If, during the performance of the Work, Contractor discovers any conflict, error, ambiguity, or discrepancy within the Contract Documents or between the Contract Documents, Contractor shall report it to District CM and PM in writing at once but in no event later than thirty-six (36) hours. Contractor shall not proceed with the Work affected until obtaining a written clarification from the District. After Contractor's discovery of such a conflict, error or discrepancy, or after the date when Contractor should have reasonably known thereof, until the interpretation or clarification is obtained from the District, any Work done by Contractor which is directly or indirectly affected by same, will be at Contractor's own risk and Contractor shall bear all costs and delay arising therefrom. Access, Tests and Inspections District, its representatives, testing agencies and governmental entities with jurisdiction shall be permitted access to the Work for its observation, inspection and testing. Contractor shall provide proper and safe conditions for such access. Contractor shall give District timely notice of readiness of the Work for all required inspections, tests or approvals. Inspections, tests or observations by CM, District or its agents may be performed at its discretion to provide information to the District on the progress of the Work. However, such information is not intended to fulfill the Contractor's obligations in accordance with the Contract Documents. If any testing, inspection or approval under this paragraph reveals defective Work, Contractor shall not be allowed to receive any associated costs and District shall be entitled to deduct from the Contract Price, by issuing a Change Order, District's costs arising out of the defective Work, including costs of repeated procedures, compensation for District's services and other related costs. If any Work that is to be inspected, tested or approved is covered without written concurrence of District, it must, if requested by District, be uncovered for observation. Such uncovering shall be at Contractor's expense and will exclude the right to an increase in the Contract Price or Contract Times unless Contractor has given District timely written notice of Contractor's intention to cover such Work and District has not acted with reasonable promptness in response to such notice. 6 Correction, Removal or Acceptance of Defective Work Contractor shall immediately, without cost to District and as specified by District, either correct any defective Work, whether or not fabricated, installed or completed, or, if the Work has been rejected by District, remove it from the site and replace it with conforming Work. The Contractor shall bear the cost of repairing or replacing all Work and property of the District or others destroyed or damaged or in any way impacted by such correction or removal. If, instead of requiring correction or removal and replacement of defective Work, District prefers to accept it, District may do so. In such case, if acceptance occurs prior to Final Payment, District shall be entitled to issue a Change Order incorporating the necessary revisions in the Contract Documents, including appropriate reduction in the Contract Price. If the acceptance occurs after such Final Payment, an appropriate amount shall be paid by Contractor to District. 7.0 PROJECT MANAGEMENT The CM will be District's authorized representative during the construction period. The duties and responsibilities and the limitations of authority of the CM as District's authorized representative during construction are as follows: The CM may issue written clarifications or interpretations of the requirements of the Contract Documents as the CM may determine necessary, which shall be consistent with the intent of and reasonably inferable from the Contract Documents. The CM may authorize minor variations in the Work from the requirements of the Contract Documents which do not involve an adjustment in the Contract Price or the Contract Times and are compatible with the design concept of the completed Project as a functioning whole as indicated by the Contract Documents. These may be accomplished by a Field Order and will be binding on District and also on Contractor, who shall promptly perform the Work involved. District will have authority to disapprove or reject Work which District believes to be defective, or that District believes will not produce a completed Project that conforms to the Contract Documents or that will prejudice the integrity of the design concept of the completed Project as a functioning whole as indicated by the Contract Documents. District will also have authority to require special inspection or testing of the Work. When Contractor has been notified of disapproval or rejection of defective work, Contractor shall take immediate action to correct or replace same. District will be the interpreter of the requirements of the Contract Documents and judge of the acceptability of the Work. Without invalidating the Contract, District may, at any time or from time to time, order additions, deletions, or revisions in the Work within the general scope of the Contract by a Change Order. Upon receipt of any such document, Contractor shall promptly proceed Page 6 of 8, SCADA Stilling Well/Platform Replacement (STA), Statement of Work

7 with the Work involved which will be performed under the applicable conditions of the Contract Documents (except as otherwise specifically provided). 8.0 DELIVERABLES Within seven (7) calendar days after Final Inspection Approval, the Contractor shall furnish the CM with the following: 1. As-built Site plan showing precise locations of underground pull boxes, conduit runs, stilling wells, gate structures, reference elevations, and bench marks. Refer to the Field Engineering specification for submittal requirements. 2. Digital photographs which shall have a minimum resolution of 1792 x 1200 pixels and shall be named using the format G374B_P02.JPG, where G374B is the applicable site name, 02 is the picture number and JPG is the file extension. The photographs shall have the correct date embedded in the original photograph and shall include but not be limited to: a. The entire site from a distance of 50 approximately setting the lens to an equivalent of normal naked eye b. The site taken from due North looking South c. The site taken either due West or East from a distance of approximately 15 or both if possible d. A composite panoramic view 3. Final Deliverables on a data CD. 8 EXHIBIT A SITE MAP Location: The site located in STA 3/4, which is south of South Bay, Florida along US- 27. Directions: G377B is located along an unnamed rock levee road, adjacent to US-27, approximately 21 miles south of the SR-80/US-27 South Bay intersection.
