Changes to existing contract documents are shown in bold, italic type and are referenced as Addendum No. 1 changes in the footer of the document. The following is a response to a written request for clarification in an dated March 4, In response we offer the following: Q: Do the following materials meet the lockable specification that is called for on this job. East Jordan Iron Works, Inc. Material - Product Number: o Monument Box Assembly o Revolution Assembly R01 o V2327 Assembly o V4275FR V4275GR Assembly B01 o Revolution Access Assembly A: Deviations from the specifications or plans will not be permitted for this material. Per the specifications, the castings shall be heavy duty and meet the general dimensions shown in the plans. All the fastening members appear acceptable except for the Revolution Access Assembly. Materials and shop drawings will be reviewed by the Engineer after the contract is awarded, and before the work. This addendum does not constitute Engineer s approval or denial of the aforementioned materials. B. The following is a response to a written request for clarification in an dated March 4, In response we offer the following: Q: Please let me know where and when we are able to obtain the boring log for the Covington Airport Project. 3 A: Boring information is not available for this project. C. The following is a response to a written request for clarification in an dated March 5, In response we offer the following: Q: 1. Please confirm wall types. a. Wall A concrete per the plans. Can it be MSE/modular block? b. Wall B specs call out Panel Walls, Plans call out modular block but wall profile shows contraction joints like a concrete wall. A. Wall A shall be a mechanically stabilized embankment retaining wall with panel walls. Wall B shall be concrete. 2. Is stone backfill required for MSE walls? A: The MSE wall backfill material shall meet the specification for MSE backfill in GDOT s 2013 Standard Specifications Construction of Transportation Systems A: No 3. Can stone foundations be used for the MSE walls? 4. Can GDOT approved alternates to the Keystone Modular Block be used (Mesa Retaining Wall Systems)? A: GDOT approved alternates would be considered for this project provided that the contractor can show that it is structurally equivalent to an MSE wall. If the proposed alternate is not approved, the contractor will be expected to install a mechanically stabilized embankment retaining wall. D. The following is a response to a written request for clarification in an dated March 9, In response we offer the following: Q: Will there be a construction easement along City Pond Rd. for sanitary sewer installation to allow for lay back of trench slopes and area for excavation of soil? Are you planning to have any of the existing City Pond Rd. removed to allow for sanitary sewer installation form SSMH 8 to SSMH 10? A: In general, the contractor will be responsible for providing any shoring needed to build the project within the right of way limits. Besides utility crossings shown on the plans, City Pond Road will remain in place and in service during construction. S.E. 4 E. The following is a response to a written request for clarification in an dated March 10, In response we offer the following: Q: 1. Where are the drainage pipe profiles for line C2 to C1? A: The profiles will be added on the next plan revision and distributed via an addendum. 2. On the grading plan from Sta 0+00 to 4+25, it appears that the grading extends past the fence line onto Nisshinbo property. Will there be a temporary easement for this work? Also, I didn t see any temporary fence as a pay item for this area that would have to be installed for security reasons. A: Easements and additional fence will be addressed on the next plan revision and distributed via an addendum. F. The following is a response to a written request for clarification in an dated March 5, In response we offer the following: Q: Will cadd files of the survey and finished grades be available for the project? A: While not included in the bid documents, cadd files can be made available following award of the contract at the selected contractor s request. 5 COVINGTON MUNICIPAL AIRPORT FEBRUARY 2015 ADVERTISEMENT FOR PUBLIC BIDS COVINGTON MUNICIPAL AIRPORT COVINGTON, GEORGIA Sealed bids will be received by the Covington Municipal Airport, Covington, Georgia at the City of Covington, City Hall, 2194 Emory Street, Covington, Georgia on April 17 th, 2015 until 10:00 AM Local Time and at that hour opened and publicly read aloud for the improvements to the Airport as listed herein. Please note that bids will only be considered by those bidders and subcontractors currently pre-qualified with the Georgia Department of Transportation. A Pre-Bid Conference will be held on Wednesday, March 4, 2015 at 10:00 A.M. at the Covington Municipal Airport, Airport Road, Oxford, Georgia, Interested contractors are encouraged to attend this conference. PROJECT DESCRIPTION The work consists of furnishing all labor, equipment, and materials and performing all work in strict accordance with the plans and specifications for: SE APRON - PHASE II (SITE PREPARATION & UTILITIES) The location of the work is at the Covington Municipal Airport, Covington, Georgia. Prospective bidders should read the following instructions carefully before submitting their bids. For each item on the bid form there is a space provided for the price to be shown in numerals and words. All notations must be in ink. Totals read at the opening of bids are not guaranteed to be correct and no final award of contract will be made until the bid and extensions have been verified. A Bidder's bond must be executed on the form furnished by the Owner, and the required bond, cash, cashier's check, or certified check must accompany each proposal, in the amount of 5% of the total amount of the proposal. A 100% performance bond and a 100% payment bond will be required of the Contractor at time of contract execution. A Georgia Resident Agent must countersign all bonds from a surety company authorized by law to do business in this State pursuant to a current certificate of authority to transact surety business by the Commissioner of Insurance; no bond shall be approved unless the surety is on the United States Department of Treasury s list of approved bond sureties. The successful bidder will be required to provide the Owner with the affidavit required by OCGA (e) Competitive Award Requirements. All work under the contract shall be completed within One Hundred Eighty (180) calendar days from the issuance of the notice to proceed. Each BID shall be enclosed in a sealed envelope and marked and addressed as required in the below and in the Advertisement and shall be accompanied by the Bid Security and other required documents. If the BID is sent through the mail or other delivery system, the sealed envelope shall be enclosed in a separate envelope with the notation "BID ENCLOSED for (Project Name)" on the face thereof. Submit original and one copy of the Bid Form, Schedules and other required documents. Indicate the following information on the outside of the sealed envelope containing the bid: a. Project Name as stated on page one of the Bid Forms b. Project Number c. Location of Airport d. Bidder's Name and Address Submit Bids to: City of Covington Attn: Scott Crommer 2194 Emory Street Covington, Georgia The Submittal Checklist must be reviewed and the bidder is to comply with the order of the submittal of documents. This document is to be included with the bid. Bids may be submitted by mail, common carrier or delivered in person. Fax or electronic bids are not acceptable. It shall be the duty of each Bidder to ensure that their bid is delivered within the time and at the place prescribed in this document. Bids received prior to the time fixed in this bid document will be securely kept unopened. Any bid received at the office designated in this document after the exact time and date specified, will not be considered. If a late bid is received via carrier, it will be marked late bid and will not be opened. If a late bid is hand delivered, it will be returned unopened to the presenter. At the date and time specified for the opening of the bid, the bid shall be publicly opened and read aloud for the information of Bidders and others present. If descriptive literature is attached to the bid, your firm s name must be on all sheets submitted. Each bid submitted shall be deemed to have been made with full knowledge of all terms, conditions, and requirements contained in this Bid request. The failure or omission of any Bidder to examine any form, instrument or document shall in no way relieve any Bidder from obligations in respect to the bid submittal or the compliance of the terms, conditions and requirements of the bid. Individual contractors shall provide their Social Security number and proprietorships; partnerships and corporations shall provide their Federal Employer Identification number and provide a completed W9 form to be submitted with the bid. The undersigned bidder hereby agrees to be bound by the award of the contract and, if awarded the contract on this Proposal, to execute within fifteen calendar days after notice of award, the required Contract and the Performance Bond and Payment Bond, of which Contract this Proposal, the Plans for the work, and the Standard Specifications, with subsequent revisions shall be a part.
