1 ELECTRICAL SAFETY INSPECTION REPORT. SREERAMPUR, DHAKA, BANGLADESH Factory List: Graphics Textiles Limited Inspected on May 20, 2014

2 SUMMARY Graphics Textiles Limited is located in Sreerampur, Dhaka. Reportedly, there are three sections of building constructed in 2008 with common basement. The factory was established and owned by themselves. They started production in the year of The building was approved for industrial purpose and the factory currently has around 3,620 permanent staffs in total. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. Page 2 of 7

3 FINDINGS AND RECOMMENDATIONS E- 1 Category: SWITCH BOARD & PANELS Cables terminating at panel, not supported. Cables should enter the panel using cable glad of proper size while supported on cable trays to minimize the stress on cable bends. Cable entering panel without support and fixture at gland hold. Disconnected cable exposed. E- 2 Category: SWITCH BOARD & PANELS Barrier/separators between different phases are not installed. Install separators between different phases of MCCB. Standard separators provided by the MCCB manufacturer must be used. Typical phase barrier being used. Proper coordination of load sharing required. E- 3 Category: SWITCH BOARD & PANELS Three phase MCCB connected to control two phases or less. Check and redesign the requirements to control the circuits. If three phase control is not required, the control devices suitable for the purpose may be selected (replaced). MCCB is used to control single phase supply by looping in another terminal. 4 E- 4 Category: SWITCH BOARD & PANELS Panel doors not connected with earth bond. Panel door(s) must be connected with earth bond connecting frame and door. Panel without base plate & glands to support incoming and outgoing cables. Earthing not provided to body and door. E- 5 Category: SERVICE LINE Service cable not supported nor protected. Service cables should be properly supported in cable trays for protection and safety reasons. Service cable not protected, arranged and supported. E- 6 Category: CABLE & CABLE SUPPORTS Cables joined in junction box without proper join kit and no junction box cover Cables should be properly joined using join kit within the junction box and appropriate cover should be placed. 5 E- 7 Category: WIRINGS Cables are drawn inside the PVC conduit. The use of PVC conduit must be minimized only to a limited application. PVC conduit should be replaced with rigid type steel conduits. PVC protected wire drawn through wall and clamped along the wall. E- 8 Category: BOILER & COMPRESSOR ROOM Messy control cables are in contact with sharp edges. Control cables should be properly rearranged and firmly fixed with cable ties and brackets. Cables entering the panel should be properly protected with cable glands for protection and safety reasons. MCB mounted on transformer, messy control cables in tray and sharp edges. E- 9 Category: EQUIPMENT & MACHINE Motor unfixed and cables left unprotected on floor. Standing motor must be firmly fixed to the foundation and the cable must be properly protected. Standing motor not bolted to the foundation. Supply cable laid freely on compressor and floor. Page 5 of 7

6 E- 10 Category: LIGHTNING PROTECTION & EARTH Earth cable is drawn without any protection. Earth cable must be properly protected with rigid metal conduits and firmly connected to the earth bus. Down conductor directly dropped for connection to the earth bus. E- 11 Category: CABLE & CABLE SUPPORTS Cables laid on concrete floor The cable should be supported on covered tray or ladder throughout its length up to the panel baseplate. Excess cable may be supported and laid outside the room. Cables behind the panel not properly layed and arranged in trench. E- 12 Category: CABLE & CABLE SUPPORTS Flexible PVC conduit wiring not supported. Flexible PVC conduit wiring must be additionally supported on cable tray and risers. 7 E- 13 Category: GENERATOR ROOM Generator frame earth looped. Reconnect generator earth loop to establish direct connection. Earth looping done with broken terminal just attached to the nuts & bolt. E- 14 Category: CABLE & CABLE SUPPORTS Wiring in flexible PVC pipes laid inside cable trench. Wiring laid in cable trench must be protected in rigid conduits with bends and accessories. If flexible conduits are used for some specific reasons, it must be of industrial grade and should bear ample strength to within stand physical impacts.
