CTS No. CITY OF VAUGHAN RESIDENTIAL PLANTING OF A CITY BOULEVARD TREE APPLICATION PACKAGE Considering the magnitude of losses throughout the city during the 2013 Ice Storm and the Emerald Ash Borer infestation, replacement of lost city-owned boulevard trees will be spread out over several years. Trees will be replanted starting in 2015 pending budget approval. The department will try to replace as many trees as budget will permit. This guideline was created to allow residents the option to plant their own City Boulevard tree instead of waiting for funding to be available. 2 Instructions for Completion of Application: 1. This application is applicable to those who wish to plant their own City Boulevard tree. 2. The APPLICANT INFORMATION form is to be completed by the applicant. Print CLEARLY. Incomplete applications will not be processed. 3. Municipal address of the tree plant location must include street name and number. (911 numbers for rural location) 4. A map of tree location and available planting space (in meters) of Boulevard tree. Also provide distance (in meters) of proposed site in relation to Personal Driveway Service Utilities box Hydro Box Streetlight Neighbouring driveway Man holes Stop Signs This will be verified through a site visit with a Forestry Inspector in consultation with the resident. Curb marking of planting location must be done by a City of Vaughan Forestry Inspector. 5. Provide written authorization from the owner if this application is signed by an Applicant other than the owner, or by an agent. 6. Submit the APPLICANT INFORMATION form and supporting documentation to Parks & Forestry Operations, Joint Operations Centre located at 2800 Rutherford Road, Vaughan ON L4K 2N9. Application Administrative Process: 7. Once the APPLICANT INFORMATION form and supporting documentation has been received, the Forestry Department has 30 days to set up a site inspection to review the application. 8. A site visit will be arranged with the Applicant present and at that time the application will be reviewed and options will be determined in consultation with the homeowner. Trees suitable for that location from the preapproved list attached will be provided to the resident by the Forestry Inspector at the time of inspection. 9. If the Applicant chooses to move forward in the planting process they need to provide the following documents in order to receive a permit: Provide the information of the nursery where the tree was purchased as well as a copy of the one year warranty. (PLANTING CONTRACTOR AND NURSERY INFORMATION FORM) Provide the information of the insured contractor that will be planting the tree as well as a current copy of their Insurance Certificate(s) and Workplace Safety and Insurance Certificate of Clearance. (PLANTING CONTRACTOR AND NURSERY INFORMATION FORM) Provide a copy of the locates for underground utilities found by the planting contractor. 3 10. The Forestry Department then has 14 days to produce a permit, which is valid for twelve (12) months from the date of issuance with planting restricted to Spring (April to June) and Fall (September to November) seasons. 11. The Forestry Department will grind the stump below grade within two (2) weeks of permit issued. 12. Twelve (12) months after stumping is completed, the Forestry Department will inspect the newly planted tree for assumption. If the tree passes for assumption, the resident will contact the Forestry Department for any further maintenance required. The Corporation of the City of Vaughan 3

THAT the Applicant has carefully examined the locality and site of the proposed works, as well as all the Application Documents (including any Health & Safety Regulations) relating thereto, prepared, submitted and rendered available by The Corporation of the City of Vaughan, and hereby acknowledge the same to be part and parcel of any contract to be let for the Work therein described or defined. 2. THAT this Application Form has not been changed or altered in any way from the original wording. 3. THAT all representations made in the said Application are in all respects true, accurate and complete. All documents required are provided in the application and are in all respects true, accurate and complete. 4. THAT the Applicant hereby applies and offers to enter into a contract to do all the work and to provide all of the labour and to furnish, deliver, place and erect all materials mentioned and described or implied therein including in every case freight, duty, currency exchange, taxes, in effect on the date of the acceptance of application, and all other charges on the provisions therein set forth. 5.
