1 Purchasing Department 40 est 11 th Street, Suite 200 Panama City, FL Telephone: (0) Fax: (0) Board of County Commissioners March 29, 201 Prospective Bidders RE: Addendum No CR229 (Cooks Bayou) Bridge Replacement Please accept this as Addendum No. 4 for the above referenced project. The following questions have been received. The County s answers follow in bold. 1. Sheet B1-11, Under Pile Installation Notes, the 2 nd note states that Minimum Tip Elevation is required for lateral stability, yet the ** note states Lateral Stability does not control. Please clarify this contradiction. The pile installation note stating minimum tip elevations is required for lateral stability should be disregarded. 2. Please provide the utility relocation schedules. The water main that was in conflict was previously relocated by the City of Callaway. The existing force main does not appear to be in conflict with the proposed construction. The design team has coordinated with the power company throughout the design phase of the project and will notify them of the proposed project schedule. 3. Please provide any information (what are they, location & depth) of the two existing 1 HDPE directional bores shown on sheet 4 of the roadway drawings. Are they to remain in service? Does the one conflict with the bridge piling and are both below the limits of final excavation? Attached is the as built survey for the water relocation project that was performed by the City of Callaway. e do not have vertical information for the force main and the horizontal information was obtained from the Contractor who installed the utility and should be considered approximate. Both utilities are to remain in service throughout the project duration. It appears that both utilities will not be in conflict with the bridge piling. 4. Is the detail Typical Section of Retention Site shown on Sheet 32 for the temporary spoil storage site, the final sediment storage site, or both? The referenced detail is that of the temporary spoil storage site associated with the dredging activities, if necessary.

2 . Please define Standard or Normal working hours. ill Contractors be allowed to work night shift? No night work will be allowed and normal/standard working hours shall be defined as daylight hours. 6. Sheet 40 states that lane closures are not allowed from 6 AM to 9 AM CST. The specifications state that lane closures are not allowed from 6 AM to AM & from 4:30 to 6:30 PM. Please clarify. There shall be no lane closures between 6AM to 9AM CST.. Specification Section 102 states that Contractor personnel are to distribute notifications to each business & resident 24 hours prior to start of construction & prior to starting any operations that could prevent access to any resident or business. How many businesses & residents will be effected? Variable message boards may be used to notify business and residents. All proposers shall acknowledge receipt of this addendum by electronically signing and submitting the addendum acknowledgement form included in the electronic ITB package. Sincerely, endi Sellers Purchasing Director 1-16 CR229 (Cooks Bayou) Bridge Replacement Addendum #4 Page 2 of 2
