GOVERNMENT OF WEST BENGAL Office of the Superintendent, Asansol District Hospital ASANSOL Dated- Page1 of T E N D E R N O T I C E Sealed tenders are invited by the Superintendent,Asansol District Hospital, Asansol, Dist-Burdwan West Bengal for procurement TENDER FOR ORTHOPAEDIC IMPLANTS, DENNIS BROWN SPLINTS&ORTHOPEDICAL SHOES for this hospital from experienced suppliers,distributors, Co-Operative Societies, resourceful agents etc. Tender must be submitted in a sealed cover and superscribed with capital letters as TENDER FOR ORTHOPAEDIC IMPLANTS, DENNIS BROWN SPLINTS&ORTHOPEDICAL SHOES to the office of the undersigned (Tender Box). Tender must be accompanied by the attested copy of the followings:- 1. Current trade license 2. Recent Professional Tax paying Challan 3. Income Tax return for the last three consecutive Financial Year 4. Trading Profit Loss Sheet of the last three consecutive Financial Year 5. current dated Bank Draft of any Nationalised Bank in favour of the Rogi Kalyan Samity S.D.Hospital, Asansol as Earnest Money of Rs =00 as Earnest Money, in case of exemption the certificate of MSME//SSI unit,entrepreneur memorandum or Udyog adhar Memorandum are required to be furnished which will be returned in due time to the unselected bidder. 6. GST Registration Certificate with GST returns for the period of last December For the partnership firm the legitimated Deeds 8. For the Company the certificate of Registration for incorporation 9. ESI Registration certificate with last payment certificate if applicable, and in the event of ESI Registered the Last EPF Payment certificate. Has the bidder not under ESI Registration the Self declaration to be furnished. 10. Bank solvent certificate from the bank of the bidder 11. Credential certificate on supply to any Government institution/organisation kind of the material amidst this tender. Treasury Challan of Rs (Fifteen thousand) as an earnest money duly pledged to the Superintendent Asansol District Hospital, Asansol under the Head of Account, Rogi Kalyan Samity of Asansol District Hospital, through Nationalized Bank Draft. Money order will not be accepted. The last date of receipt / submission of sealed tenders are on 31/03/ Noon from this end and no tender will be accepted after the stipulated date and time. Rate should be quoted both in figure and in words on the supplied list by the undersigned against each item. Vat will be applicable as per Govt. Norms and that please be noted in separate coloumn. List of Orthopaedic TENDER FOR ORTHOPAEDIC IMPLANTS, DENNIS BROWN SPLINTS&ORTHOPEDICAL SHOES with terms & conditions relating to the tender are obtainable from the office of the undersigned(equipment Store) at free of cost on any working days from 14/03/2018 to19/03/2018 at Noon to 4.00 P.M. excepting Govt. holidays. Requests for postal dispatch of forms / list will not be entertained. Tender will be opened on 31/03/2018 at 2.00 P.M. in presence of the tenderers or their representatives. The selection committee reserves the right to reject or to accept any tender without assigning any reason thereof. For orthopaedic implants it will be determined by the cumulative total of the items, put to tender against a particular items with various sub items may be attached with it. This is required to avoid mismatching of the ortho implants and other items Vis-a-Vis the accessories within a particular item, however no rate should be quoted for the item/set attached to it which and to be supplied free of cost during operation which can be taken back after operation.

2 TERMS & CONDITIONS RELATING TO THE TENDER Period of Annual Contract: Security Deposit:- The successful tenderers must furnish security deposit of Rs. 15,000.00(Fifteen thousand) for the items of Annexure-A, within 15 days positively from the date of acceptance of the tender or approval letter and same is to be deposited through bank draft in favour of Rogi Kalyan Samity, Asansol District Hospital, Asansol duly pledged to the Superintendent, Asansol District Hospital, Asansol and the Bank draft will be retained in the office of the undersigned. The security money will be liable to be forfeited without prejudice to any other action in the event of failure / refusal to do works according to the contract. The security deposit will be released after successful completion of contract. Risk & penalty :- In the event of failure to do supply the indented devices/implants & water purifier as per contract the local authority will be compelled to procure the same from open market or any other sources in the better interest of hospital patients. The excess expenditure will be recovered from the failure contractor from any money (Account) due to the contractor or any other legal manners. Warranty:- The selected contractor will be required to furnish a warranty form / Deed of agreement as per specifications supplied by the tendering officer on and after finalization of the tender on the Non-Judicial Stamp Paper of Rs. 10/-(Ten) which will be supplied by the selected party.
