1 CITY OF CHICAGO REQUEST FOR QUOTATION INVITATION FOR: RE: Chicago Police Candidate Background Investigative Services and Reporting Small Orders - Target Market Request for Quotation (RFQ) Specification Number: (DPS#B) DPS: James Bracewell DPS Contract Administrator James.Bracewell@cityofchicago.org Telephone: DUE DATE (Bid Open Date): Thursday, December 16, 2010 The City of Chicago Department of Procurement Services invites vendors to submit a quote for the above requirement. If you are interested in doing business with the City of Chicago, you are encouraged to respond. Please direct all questions to the DPS Contract Administrator. This RFQ and resulting Purchase Order (PO) are subject to City of Chicago Request for Quotation and Detailed Specification (both attached) and to the Small Orders Terms and Conditions - Standard, (downloadable from the DPS Small Orders web site), and are hereby incorporated into the RFQ and PO by reference. The Maximum Compensation under the resulting PO cannot exceed $100, This is a Target Market RFQ. Award of PO is restricted to City of Chicago Certified Minority Business Enterprises (MBE) and/or Women Business Enterprises (WBE) that certified at the time of bid submittal (due date). A copy of your current City of Chicago Letter of Certification is to be submitted with your response to the RFQ. In the event that an MBE or WBE cannot be awarded a PO, the RFQ will be re-posted (advertised) open to all bidders. RFQs, EDSs, and blank insurance certificates are available from the DPS Small Orders web site. TO RESPOND: Please complete and sign the City of Chicago Request for Quotation, and along with your current City of Chicago Letter of Certification, fax or them to the above DPS Contract Administrator. All RFQs must be received no later than 11:00 AM Central Time on the attached Please Respond By date on the RFQ. The Please Respond By date is the due date. Please write or stamp your firm s name on al pages of your bid submittal. Following the RFQ due date and time, a bid tabulation will be prepared. Bid tabulations are made available to the public on DPS sweb site: under Contract Administration, Bid Tabulations (Bid Tabs), Get Started Online. Search by the above specification number. Please allow one to three business days for posting of the bid tabulation. The low bidder will then be contacted by the DPS Contract Administrator and will be required to submit original 1) City of Chicago Request for Quotation with original signature; 2) properly executed Economic Disclosure Statement (EDS); 3) completed insurance certificate; and 4) all other required documents (as applicable). The CPO will then move to award a PO. Notice of Award for a Smal Order PO is made by the City s web site when an Award Date is posted. SCOPE CITY OF CHICAGO DETAILED SPECIFICATION CHICAGO POLICE CANDIDATE BACKGROUND INVESTIGATIVE SERVICES AND REPORTING Specification Number The Contractor must furnish and deliver Chicago Police Candidate Background Investigative Services and Reporting, F.O.B, City of Chicago Police Department, Human Resources Division the specified as described herein and in accordance with the terms and conditions of this specification INTENT The intent for this bid solicitation is to provide a background investigation service within the hiring process for Probationary Police Officers that is orderly and fair. The background investigation is to ascertain weather there is any discernable reason to disqualify a candidate from employment with the Department of Police INVOICES All invoices must be forwarded by the Contractor to the Chicago Police Department, Personnel Department, 3510 S. Michigan Ave, Chicago Il Contact is Director Tracey Ladner, , to apply against the contract CANDIDATE BACKGROUND INVESTIGATIVE REPORTS Completed Personal History Questionnaires (PHQ) will be assigned to the Contractor for a candidate in either electronic or hard copy format. Please see the sample Chicago Police Department PHQ that is available on the website, Completed Candidate Background Investigative Reports (CBIR) must be completed and delivered to the CPD representative within 30 calendar days of receipt of each PHQ. Additional time to complete a CBIR must be requested in writing to the Superintendent of Police and must contain supporting information in writing. The Superintendent will review the request and in extraordinary circumstances may grant additional time to complete the CBIR. The Superintendent s approval must be in writing. Verbal approvals are not binding or valid. The Superintendentof Police ( Superintendent ) refers to the chief executive officer of the City of Chicago Police Department, and any representative duly authorized in writing to act on the Superintendent s behalf. CBIRs must be completed with the highest level of professionalism. Completed CBIRs must be submitted to the CPD representative named at the time the PHC is assigned to the Contractor REVIEW AND APPROVAL OF SAMPLE REPORT Bidder must submit a sample CBIR prior to award for the City s review and approval. An electronic version of the sample CBIR must be provided to the DPS Contract Administrator within 72 hours of the Chief Procurement Oficer srequest. 3 CITY OF CHICAGO DETAILED SPECIFICATION The sample CBIR must contain the information and must be organized similar to the sample report Summary of Findings Chicago Police Candidate Background Investigations. Please see the Summary of Findings Chicago Police Candidate Background Investigations website, The City will review the CBIR and may approve, provide comments, or reject. If requested by the Chief Procurement Officer, the Bidder may resubmit one revision within 72 hours of the Chief Procurement Oficer s request for the City sreview. The City will either accept the sample CBIR format or reject the bidder s proposal as being non-responsive to the requirements of this solicitation and the needs of the City. The format and contents of the CBIR may not be changed from the approved sample CBIR without the written approval of the Superintendent PAYMENT Payment will only be made by the City for completed and accepted reports EXCEPTIONS Any deviations from these specifications must be noted on the Proposal Page or Pages attached thereto, with the exact nature of the change outlined in sufficient detail. The reason for which deviations were made must be submitted with the bid if not self-explanatory. Failure of a bidder to comply with the terms of this paragraph may be cause for rejection. The City reserves the right to disqualify bids which do not completely meet outlined specifications.
