Laboratory Analysis conducted by DENR Recognized Laboratory (for Power Plants- PCB Analysis (Aroclors, Congeners), for wastes under K-series TCLP Analysis and Sludge Characterization ); AMOUNT OF FEES [G]

Proof of Compliance with RA 9275 (Clean Water Act) and/or Supreme Court Mandamus to Rehabilitate Manila Bay Proof/Copy of Certificate of siphoning/desludging of septic tanks, hauling, transport and treatment of wastewater/septage by an accredited service provider. 6 9. Upon evaluation of the Section Chief, the application is then forwarded to the Chief of the Division for his final evaluation and signature CPD Chief Engr. Dennis O.
