Box 356 Augusta, AR RE: Quarterly Inspection; AFIN ; Permit Number 0012-STSW-A Dear Judge Dallas: On November 4, 2015, I conducted a routine inspection of your facility pursuant to the Arkansas Solid Waste Management Regulation 22, and the above-referenced permit. A copy of the inspection report is enclosed for your records. Please refer to the attached inspection report to note the specific allegations. A written response for the noted allegations, describing the corrective action taken or to be taken, must be submitted to my attention at the address listed below within thirty (30) days from the date hereof. Failure to submit the written response shall be construed as an admission of any allegations. Any items noted that are neglected and/or persistent may warrant formal enforcement action. 4 Arkansas Department of Environmental Quality Solid Waste Management Division Comments Page Site Name Woodruff County TS AFIN Permit Number 0012-STSW-A Category 2 Comments Reg Operational Requirements (Adequate facility maintenance) (k) Facility Maintenance - Sanitary conditions shall be maintained through the periodic wash-down or other appropriate cleaning method of the transfer station and transfer vehicles. Frequency of cleaning shall be sufficient to prevent odors and other nuisance conditions from developing. All residuals shall be properly disposed of following cleaning operations. Comments: Waste residuals were noted beneath the compactor and along the drain where the truck connects to the compactor. Additionally, wastes (likely removed from the exterior of the trailer) had been deposited on the ground surface. These residuals and wastes should be properly disposed of following cleanup operations by placing them back into the compactor. See photos 1 through 6. Category 3 Comments Reg Operational Requirements (Adequate drainage to sanitary sewers) (i) Drainage - All floors must be free from standing water. All drainage from cleaning, unloading and processing areas must be discharged to sanitary sewers or permitted onsite treatment facilities. Storm water discharges from the site shall conform to Department storm water permitting requirements. Comment: The drain from the pad under the compactor has wastes in it and is causing standing water. This water is allowing for the growth of maggots. See photo 7.
