1 Memo To: Dean Vanderhoff, Emergency Response Section From: Jeff Tyler, District 4 Field Inspector Through: Kerri McCabe, Inspector Supervisor Date: May 13, 2013 Re: Quality Petroleum in Crawford County On April 26, 2013, I conducted a site investigation in response to an oil release at 1818 Main Street in Van Buren. Quality Petroleum was unloading bulk oil at the Shell Rapid Lube facility; site coordinates N W. Problems occurred with the equipment, which allowed approximately 169 gallons to be released to stormwater. Jones Environmental was contacted for containment and clean-up. I arrived at the site at 1600 and met with Brad Dooly who works for Quality Petroleum. He advised that the oil flowed downslope from the facility, it pooled up and some also entered a concrete lined stormwater drainage ditch named the Town Branch. Jones Environmental was able to vacuum up approximately 110 gallons of product. Crusher dust was applied to the parking lot to act as an absorbent and booms were placed in several locations along the water way. During my investigation, I did observe sheen on the water at the location where the oil entered, which was flowing downstream. After tracking downstream to the intersection of South 11 th and Mary Allen, there were booms in place at this location ( ), it was apparent that some of the oil was by-passing the booms. I then went downstream to the bridge on Scott Street ( ), where a very slight sheen was observed in the water. I advised Mr. Dooly that more efforts were needed in regard to containing the oil and preventing further downstream impact. He contacted Jones Environmental at that time. On April 29, 2013, I conducted a follow-up site visit; booms were still in place at several locations along the Town Branch and appeared to be effective in preventing the oil from going further downstream. I contacted Mr. Dooly and requested that he and or Jones Environmental continue to monitor the water way. Mr. Dooly was also advised that a final report summarizing the incident must be submitted to the Department by May 20, Spill Report Page1 of 6

Local Coordinator reported a Quality Petroleum tanker truck pulled in to make a delivery of motor oil and a line came off between the truck and the Rapid Lube storage tank. It was raining which caused some oil to get into Town Branch which is a concrete stream. Town Branch is diked and boomed. Jones Environmental remediated. No evacuations or injuries were reported. 1
