GROUNDWATER MONITORING AND CORRECTIVE ACTION REPORT Under Federal CCR Rule 40 CFR Part Groundwater Monitoring and Corrective Action Muscatine Power and Water (MP&W) as the owner of an existing coal combustion residue (CCR) landfill must prepare, no later than January 31, 2018, and annually thereafter, an Annual Groundwater Monitoring And Corrective Action Report. The report must, for the preceding calendar year, document the status of the groundwater monitoring and corrective action program for the CCR unit, summarize key actions completed, describe any problems encountered, discuss actions to resolve the problems, and project key activities for the upcoming year. The prepared annual report must be placed in the facility s operating record as required under Part (h)(1). The following sections cover the annual report s content requirements under Part (e) for calendar year A. LOCATION AND SITE MAPS (e)(1) MP&W maintains a private CCR landfill that provides for the controlled disposal of CCR originating at its power generating facility located at 1700 Dick Drake Way in Muscatine. The approximate 80 acre landfill site is located 7.5 miles west of the power plant in the SW¼ of Section 16, Township 76 North, Range 3 West in Muscatine County (Figure 1, Appendix A). The landfill has been in continuous operation since The CCR includes a mixture of gypsum, fly ash, bottom ash, and slag. The permitted disposal area is approximately 34 acres which is being developed in four phases, where the cells are constructed and filled in sequential order from east to west. Currently Phases I and II are open and are actively being filled with CCR (Figure 2, Appendix A). The site is regulated by the Iowa Department of Natural Resources (IDNR) under [567] Iowa Administrative Code (IAC) Chapter 103 and by state Sanitary Disposal Project Permit #70-SDP-06-82P, issued August 9, 2010 with an expiration date of August 9, 2020, and subsequent Permit Amendment Nos. 1 through 9. A comprehensive list of references for this facility is provided in Section II. Of primary interest herein is: Groundwater Monitoring System and Sampling and Analysis Program, CCR Landfill (HR Green, revised May 2, 2017, original May 18, 2016). That document addresses the groundwater monitoring and corrective action requirements of the Federal CCR Rule Part and is posted as a reference on MP&W s publicly accessible Internet site at B. IDENTIFICATION OF WELLS (e)(2) Table 1 provides a summary of the existing groundwater monitoring wells (Appendix B). I-1

Project No Annual GMCA Report CCR Landfill Muscatine Power & Water There were no new monitoring wells installed during One new well, MW-22 is proposed for installation in This well will allow monitoring of groundwater near the southwest entrance of the landfill. This well is discussed in Section I.E below. Well MW-17 was decommissioned on December 11, The well was used exclusively for water level measurements. The well was plugged and abandoned under direction of the Iowa DNR due to damaged casing at approximately 4.5 feet deep which prevented access to the well. Proper documentation was submitted to the Iowa DNR on its standard form # C. SUMMARY OF SAMPLE COLLECTION AND ANALYSIS (e)(3) Sample Collection and Results Under 40 CFR Part (a) the Groundwater Monitoring Program (GMP) includes the following groundwater monitoring points: Upgradient wells: MW-8 and MW-10 used to establish background quality; and Downgradient wells: MW- 4A, MW-5B, MW-6A, MW-13, MW-14A, MW-15A, MW-18A, and MW-21 to monitor for downgradient impacts. Table 1 provides a summary of the groundwater monitoring points (Appendix B), including: (1) Location coordinates (see also Figure 2), (2) Construction details, (3) Function as a monitoring well or level measuring point, (4) Hydrogeologic unit monitored, and (5) Recent water level measurement used for the current evaluation of horizontal groundwater flow pattern and vertical gradients. The monitoring wells are sampled for the constituents specified in Appendix III and Appendix IV to Part 257, as follows: Appendix III: boron, calcium, chloride, fluoride, ph, sulfates, and total dissolved solids. Appendix IV: antimony, arsenic, barium, beryllium, cadmium, chromium, cobalt, fluoride, lead, lithium, mercury, molybdenum, selenium, thallium, and radium 226 & 228 combined. Table 2 provides the implementation schedule for the GMP (Appendix B), consisting of: (1) Well function as either a upgradient or downgradient monitoring point (2) Number of samples collected in each monitoring program, (3) Dates of completed sampling events and constituents tested, including: I-2

6 HR Green, Inc. Project No Annual GMCA Report CCR Landfill Muscatine Power & Water a. Establish background quality, b. Detection monitoring, c. Resampling events to verify initial SSI, d. Assessment monitoring, and e. Corrective action monitoring. Samples are collected and handled as described in Procedure for Groundwater and Surface Water Sampling (HR Green, May 2016, updated June 2017). A tabulation of the groundwater sampling data obtained under through is provided. This tabulation covers the period of June 2016 through December 2017, including the initial 8 events used to establish background quality, the first detection (compliance) event, and the first resampling event (Appendix C). The laboratory s analytical reports, and field low-flow sampling forms, and the DNR Sampling Forms are also provided for the two events evaluated herein, including the first detection (compliance) event and the first resampling event (Appendix C). A full copy of all sampling documentation and laboratory analytical reports for the background events is filed in the landfill s operating record and is available for review upon request to MP&W. A full copy is not included herein due to its overall length (~800 pages). Analysis The collected groundwater samples were analyzed by certified testing laboratory TestAmerica Laboratories, Inc. The analyzed data were then used to calculate statistical limits for each well/constituent pair (e.g., MW-21/boron). Statistical calculations were performed by Groundwater Stats Consulting using industry standard SANITAS Statistical Software, an EPA-compliant package (EPA 2009, Unified Guidance). The full procedure is as detailed in the document entitled: Groundwater Monitoring System and Sampling and Analysis Program, CCR Landfill (HR Green, revised May 2, 2017, original May 18, 2016). Event-specific reports by summarizing the statistical analyses completed in 2017 are provided for reference (Appendix D) and discussed below. D. DISCUSSION OF FINDINGS (e)(4) The establishment of baseline water quality, detection monitoring, and the present transition from detection monitoring to assessment monitoring is being conducted in accordance with the statistical analysis methodologies originally presented in Groundwater Monitoring System and Sampling and Analysis Program, CCR Landfill (HR Green, May 2017), and which are also provided herein for quick reference (Tables III-4, III-6, III-8, and III-9 in Appendix D). Project No Annual GMCA Report CCR Landfill Muscatine Power & Water Following the establishment of background water quality during the period of June 2016 through August 2017 (8 sampling events), the detection monitoring program was initiated in October The October 16, 2017 event was the first detection monitoring event, meaning Appendix III parameters were tested (see Table 2). For that event an initial 22 well/constituent pairs were determined to exceed their respective statistical limits, which are called initial SSI or statistically significant increase above background concentration (Table 2, resampling column). The statistical analysis is presented in the report by Groundwater Stats Consulting dated November 15, 2017, where the summary table on page 6 of that report shows the 22 initial SSI results (Appendix D), corresponding to Table 2. Procedure then required a resampling event for those 22 well/constituent pairs, which was conducted on November 28, 2017 to confirm that each is in fact an SSI and not a false positive. Based on re-sampling and statistical analysis there were 3 false positives (calcium, sulfate, and total dissolved solids at MW-13) and 19 confirmed SSI (Table 3, confirmed SSI column). The statistical analysis is presented in full in the report by Groundwater Stats Consulting dated December 19, 2017, where the summary table on page 4 of that report shows the confirmed SSI results (Appendix D), corresponding with Table 3. The 19 confirmed SSI lead the facility into assessment monitoring where Appendix III & IV constituents will be tested during 2018, as shown in Table 3. The first event in 2018 is tentatively scheduled for March. This event is intended to satisfy the requirements of both the routine semi-annual event and the first assessment event. The fall semi-annual event will be September. Other resampling events and/or assessment events may be scheduled based on findings. Table 3 provides a groundwater monitoring program summary (Appendix B), including: (1) The current monitoring program status, (2) Planned change in monitoring program status for the next sampling event, (3) Confirmed statistically significant increases (SSI) over background, (4) Statistically significant trends, (5) Statistically significant level (SSL) over a groundwater protection standard, and (6) Upcoming sampling dates and constituents (as best as can be determined at this point in time). E. SUPPLEMENTAL INFORMATION (e)(5) The following information is provided to fill in context for the MP&W CCR facility. I-4

Project No Annual GMCA Report CCR Landfill Muscatine Power & Water Monitored Hydrogeologic Unit For a full discussion of the GMP reference the document Groundwater Monitoring System and Sampling and Analysis Program, CCR Landfill (HR Green, Revised May 2, 2017, original May 18, 2016). Of particular relevance herein is that the GMP wells monitor (1) water levels to determine horizontal and vertical groundwater flow paths, and (2) for downgradient groundwater quality impacts to the uppermost continuous aquifer beneath the site. The aquifer and gradients are discussed below. Uppermost Continuous Aquifer: As a whole, a deep un-weathered and un-oxidized clay-rich glacial till functions as a lower confining unit with field hydraulic conductivity values of less than 1 x 10-7 cm/sec. Over most of the site, this underlying low permeability glacial till confining unit is overlain by a sequence of weathered oxidized till, sand, and clayey silt (loess) which collectively constitute the uppermost continuous aquifer beneath the site. This aquifer exhibits hydraulic conductivity values as great as 1.7 X 10-4 cm/sec and which are two to three orders of magnitude greater than the underlying confining unit. Therefore, the assemblage of deposits generally at depths of less than 50 feet is interpreted to function as the uppermost continuous aquifer beneath the landfill. This is also the unit, within which the water table fluctuates, which means the uppermost continuous aquifer is one in the same hydrogeologic unit as the shallow water table aquifer. This uppermost continuous aquifer is the unit monitored by the HMSP groundwater monitoring wells. Groundwater Flow: The pre-landfill groundwater flow direction in the uppermost aquifer was dominantly horizontal from the east/southeast toward the west/northwest with natural convergence along an ephemeral stream that formally drained the undeveloped site. Under current conditions the dominant flow direction remains the same except that convergence is now to the runoff control pond located west of the landfill, which has replaced the original stream. In addition, there is an apparent radial flow component caused by the constructed groundwater cutoff drains along the east and south perimeters, and by the landfill bypass ditch north of the fill area, and due to the apparent presence of some liquid within the original CCR (see Figure 2). The water table contours on Figure 2 were generated using April 2017 water level data. The steep gradient in the southeast corner of the landfill is a product of landfill construction, where the cell was cut steeply into the pre-existing topographic ridge thus creating a lateral flow discontinuity between the CCR and native glacial till interface. I-5

Project No Annual GMCA Report CCR Landfill Muscatine Power & Water The observed vertical flow components are recharge (downward) in the upland area of the southeast corner of the site (MW-8/9) and discharge (upward) in the lowland area along the drainage way in the northeast corner of the site (MW-10/11) (see Table 1). State Monitoring Requirements Monitoring is conducted per the DNR-approved Hydrologic Monitoring System Plan (HMSP). An updated HMSP is being proposed in in the January 2018 annual report, where the update is needed because of (1) low flow purging and sampling methodologies that have been implemented in order to test total recoverable metals, (2) the addition of proposed assessment well MW-22, and (3) to establish a statistically reliable background data set. Assessment well MW-22 is being proposed because of continuing detections at MW-21 of boron and sulfate above their respective regulatory standards. MW-22 is proposed to determine if the boron and sulfate are limited to localized bank storage near the edge of the sediment run-off pond, or if migration beyond the by-pass ditch to the property line has occurred. Under Iowa DNR the approved Hydrologic Monitoring System Plan for groundwater monitoring is the same as the Federal CCR Rule in relation to monitoring wells. However, the constituent list is different because the state s requirements are from Iowa Administrative Code Chapter 103.1(2)f, the operating permit, and Permit Amendment #6, which includes arsenic, barium, beryllium, cobalt, copper, iron, lead, magnesium, manganese, selenium, zinc, chloride, sulfate, aluminum, calcium, boron, fluoride, molybdenum, nickel, strontium, and vanadium (as total metals). The state also requires testing of five (5) surface water points for the same parameters. The evaluation for the state is conducted in relation to regulatory standards (i.e., established MCL, or HAL, or Statewide Standards for a protected groundwater source). No statistical evaluation is currently required. The state s monitoring and analysis requirements are not addressed further herein but can be found in the Annual Water Quality Report to Iowa DNR (submitted to DNR annually in January). Regulatory Status The site is regulated by the Iowa Department of Natural Resources (IDNR) under [567] Iowa Administrative Code (IAC) Chapter 103 and by state Sanitary Disposal Project Permit #70-SDP-06-82P, issued August 9, 2010 with an expiration date of August 9, 2020, with subsequent Permit Amendments 1 through 9 incorporated, including: Amendment Date Purpose #1 03/23/11 - Incorporate Plugging Record for MW-16. #2 02/13/12 - Incorporate field tile slope drain extension Authorization to construct Phase II expansion. #3 05/08/12 - Incorporate Plugging Records for MW-15, MW-18, MW-19, and MW-20. Quarterly monitoring of the designated Farm Pond outfall and annual reporting are completed by I-7

Leachate Collection System The landfill s leachate collection system is located in the active Phase I and Phase II areas of the landfill (see Figure 2). The system consists of three northsouth perforated leachate pipes, each over 800 feet long, that tie into an eastwest leachate header line located at the north end of Phase II. Two of the perforated pipes are located under a portion of the Phase I development, while the third is located under the Phase II development. The header line is also paralleled by a perforated leachate line which captures leachate from the northern Phase I fill area. All collected leachate gravity drains into Lift Station A located along the haul road north of the runoff containment pond. The original Phase I area consists of 6 diameter Schedule 80 perforated leachate pipe that was trenched into the native low permeability landfill base (clay till), backfilled with pea gravel, and covered by geotextile fabric and a sand berm (GES, 1991). The more recent Phase II area consists of a 2 percent sloped, low permeability clay liner, with an overlying porous aggregate drainage layer, draining to a 6 diameter HDPE SDR11 perforated leachate pipe (HR Green, August 2012). The leachate collected in Lift Station A is pumped overland via discharge hose to the sediment runoff control pond. The pump operates continuously and, based on the pump s nameplate information, the rate is 434 gallons per hour for an estimated pumped volume of 3,800,000 gallons per year (MP&W, Per. Com. 11/2/17). This corresponds to the visually observed discharge from the end of the hose, which resembles a garden hose running at about 7 gpm. MP&W has a spare leachate pump on hand and has entered the make and model number of the leachate pump into the supply management system, which are steps taken to ensure a new leachate pump is immediately available should a future pump failure occur. The natural (pre-landfill) groundwater flow pattern crossed the site from the eastsoutheast to the west-northwest. Conditions for this original flow pattern remain albeit modified by landfill development, specifically arising from the presence of the groundwater cut-off drains along the east and south perimeters. The cut-off drains appear to capture dominantly groundwater but there is a possibility of intermittent influence of leachate as well. Table 4 provides a summary of the landfill s leachate composition, which was determined proactively by MP&W in April Five existing CCR piezometers and one leachate collection manhole were tested for a comprehensive list of constituents. The list does not exactly match the federal Appendices III and IV but nonetheless provides a useful assessment benchmark. This information will be used more extensively during the evaluation of assessment monitoring data. I-8
