2 EXECUTIVE SUMMARY Terrapex Environmental Ltd. (Terrapex) was retained by the City of Toronto to conduct a Phase I Environmental Site Assessment (ESA) of the property located at 1300 Wilson Avenue, in Toronto, Ontario (the site). It is understood that the study is required by the City of Toronto as part of its due diligence activities prior to a potential real-estate transaction. The objective of the Phase I ESA was to identify actual and potential sources of soil and/or groundwater contamination at the site associated with historical activities at the site and on neighbouring properties. The site is located on the north side of Wilson Avenue approximately 200 m east of Lexfield Avenue, in Toronto, Ontario. The site is roughly rectangular in shape, is approximately 3.03 acres (1.23 ha) in size. The site is developed with one a two-storey building, which is currently occupied by St. Noel Chabanel elementary school. The site is owned and operated by Conseil Scolaire de District Catholique Centre-Sud (South-Central District Catholic School Board). Vehicular access to the site is obtained from Wilson Avenue. The surface at the site is asphalt paved with a grassy area to the north of the building. According to historical documents, the site was first developed prior to 1947 as a residential property. In 1975, construction of the current building was completed and began operating as École Saint-Noel-Chabanel, a French Catholic School. A tributary of Black Creek formerly transected the central portion of the site in an east/west direction. During the late 1970s the stream was channelized into a culvert that extends beneath the property, and which is currently still present at site. Surrounding land uses have historically been predominantly residential with some institutional and commercial land use. No on-site environmental concern with respect to potential soil and groundwater quality at the site was identified at the subject site. A previous report confirmed the presence of asbestos at the subject site in vinyl floor tiles and plaster in several locations in the school. The potential presence of other designated substances such as lead, mercury and PCBs were noted in a previous work program. Other hazards, such as mould located below sink area were also noted. Several off-site sources of potential impact to the environmental quality of soil or groundwater at the site were identified during the study: an electrical sub-station is located on the property immediately west of the site; numerous groundwater monitoring wells were also observed at this property; TERRAPEX ENVIRONMENTAL LTD. 3 Technical Standards and Safety Authority Records relating to the "discovery of product" at 1303 and 1307 Wilson Avenue (apartment buildings located approximately 20 m south of the site, across Wilson Avenue) were identified, although no other details regarding these instances was provided; and, the Apartment building located on the property immediately east of the site has been granted a Certificate of Approval for the operation of a backup generator, indicating a potential for an above ground or underground fuel storage tank to be present on the property. Based on the records review, interviews and site inspection, the Phase I ESA did identify sources of potential sources of environmental concern with respect to soil and groundwater quality at the site. 6 1.0 INTRODUCTION Terrapex Environmental Ltd. (Terrapex) was retained by the City of Toronto to conduct a Phase I Environmental Site Assessment (ESA) of the property located at 1300 Wilson Avenue, in Toronto, Ontario (the site). It is understood that the study is required by the City of Toronto as part of its due diligence activities prior to a potential acquisition of the site for future use as an Emergency Medical Services (EMS) ambulance station. The site is located on the north side of Wilson Avenue approximately 200 m east of Lexfield Avenue, in Toronto, Ontario. The site is roughly rectangular in shape and approximately 3.03 acres (1.23 ha) in size. The site is developed with a one two-storey building, which is currently occupied by St. Noel Chabanel elementary school. The site is owned and operated by Conseil Scolaire de District Catholique Centre-Sud (South-Central District Catholic School Board). Vehicular access to the site is obtained from Wilson Avenue. The site is asphalt paved with grassy areas to the north of the building. The site location is presented in Figure 1, while the general site features are presented on Figure OBJECTIVE The objective of the investigation was to identify actual and potential sources of soil and/or groundwater contamination at the site associated with historical or current activities at the site and on neighbouring properties. 1.2 SCOPE OF WORK The ESA was conducted in general accordance with the principles set out in Canadian Standards Association (CSA) Standard Z768-01, Phase I Environmental Site Assessment. The scope of work comprised: reviewing available historic and current environmental information; inspecting the site, and accessible neighbouring sites for evidence of potential environmental concerns; interviewing persons knowledgeable or potentially knowledgeable of current and historical site conditions; and, preparing this report documenting the findings of the assessment. 8 Requests for file information from: MOE Freedom of Information Office regarding environmental concerns, such as waste generator registration numbers and waste class codes assigned to the site, Certificates of Approval, spill notifications, violations, complaints, or control orders; The City of Toronto regarding environmental issues; and, The Technical Standards and Safety Authority, Fuels Safety Branch (TSSA) for information pertaining to fuel storage tanks. Copies of two reports pertaining to the site that were provided to Terrapex by the City of Toronto. Anecdotal information from the following individuals: Ms. Janice Green, Environmental Project Manager, City of Toronto, Facilities Management Division. Mr. Richard Nsengimana, Agent à la planification (Planning Agent), Conseil Solaire de District Catholique Centre-Sud (South-Central District Catholic School Board). 2.2 SITE INSPECTION The site inspection was conducted by Ms. Sara Sutherland of Terrapex on July 12, 2013 using Terrapex s standard Phase I ESA checklist and field forms. The site and accessible neighbouring sites were inspected for evidence of potential environmental concerns such as regulated substances, former and existing underground and aboveground storage tanks (USTs and ASTs, respectively), miscellaneous wastes and debris, hazardous materials, infilled areas, surface staining and stressed vegetation. 2.3 LIMITATIONS Observations of surrounding properties during this work program were limited to areas visible from the site or from publicly-accessible vantage points. A small concrete shed located on site was locked and not accessible for visual inspection. Areas that could not be inspected are noted in Section It should be noted that although Terrapex has attempted to verify information wherever possible, except where explicitly noted, Terrapex has relied upon the accuracy of the information collected during the records review and interview processes. 9 3.0 RESULTS AND DISCUSSION 3.1 RECORDS REVIEW AERIAL PHOTOGRAPHS In the 1947 aerial photograph, a single family residential structure is visible on the southern portion of the property. The majority of the property remains forested. A stream is visible transecting the northern portion of the site in a northeast to the southwest direction. Surrounding properties appear to be undeveloped (agricultural) or residential houses. In the 1956 aerial photograph the site appears unchanged from the previous photograph. Large residential developments are visible to the west and north of the site. In the 1966 aerial photograph the appears unchanged from the previous photograph, however the surrounding area has increased residential and commercial developments. In the 1976 aerial photograph, the site appears to be nearing completion of construction of the current onsite building. An additional small rectangular structure is visible directly north of the main building, possibly for storage or use as a portable classroom. The stream is visible transecting the through the site. The surrounding properties appear to be well developed with residential and commercial buildings. In the 1981 aerial photograph, the site and surrounding area appears similar to the previous aerial photograph with the exception of the construction of the main building is complete and the stream is no longer visible transecting the site (has likely been channelized below the site surface). The 1987 aerial photograph shows three additional small rectangular structures directly north of the main building of similar size and placement as the previously noted additional structure. The north portion of the property has visibly been landscaped with additional walking paths. The surrounding areas generally appear similar to the immediately previous aerial photographs. In the 1992 aerial photograph the site and surrounding area appears similar to the immediately previous aerial photographs. In the Google Earth 2002 and VuMap 2011 satellite images, no significant changes of the site and surrounding areas were noted. It should be noted that identification of more specific features at the site and surrounding areas was precluded by the scale and resolution of the aerial photographs and satellite imagery. 10 Copies of the aerial photographs and satellite imagery are included in Appendix I PHYSIOGRAPHIC AND HYDROGEOLOGIC RECORDS Topography: Based on topographic mapping, the site is estimated to be at an elevation of approximately 161 metres above sea level and is located in a predominantly residential area of Toronto with occasional commercial and institutional, area of Toronto. The nearest mapped watercourse is Black Creek, approximately 800 m west of the site. Black Creek discharges into the Humber River, which flows in a southerly direction into Lake Ontario. Based on the historical records review and site inspection, it appears that a portion of an intermittent stream (former tributary of Black Creek) has been channelized and buried below the subject site. In the vicinity of the site, topography generally slopes towards the central portion of the site, where it assumed that the former stream was channelized. The regional groundwater flow direction is anticipated to be southwest, towards the Humber River (a regional groundwater discharge zone). The local groundwater flow direction has not been determined at the site, but is expected to be influenced by both the regional flow regime and local topography (e.g., likely west). A copy of the topographic map is included in Appendix II. Geologic Mapping: The site is located in a physiographic region known as the South Slope which is generally described as primarily tills which are more sandy in the east and clayey in the west, with shale content increasing westward. Bedrock geology consists of upper Ordovician shale, limestone, dolostone, and siltstone of the Georgian Bay, Blue Mountain, and Billings Formations, Collingwood and Eastview Members. The quaternary geology consists of predominantly sandy silt to silt matrix, commonly rich in clasts, often high in total matrix carbonate content. Overburden thicknesses in the vicinity of the site are expected to be significant, and bedrock outcrops in this area are considered unlikely DATABASE RECORDS Municipal Directories: Municipal directories for the subject site and surrounding area were searched at approximately five to eight year intervals by Terrapex for the years between 1963 and Listings for the area for years prior to 1963 were not available. 11 1300 Wilson Avenue (the site) The address is listed as a residential address in the 1966 city directory. The school Ecole St. Noel Chabanel is first listed in the 1982 city directory search Wilson Avenue (adjacent to the west) Listed as a hydro (electrical) substation from 1966 to The historical presence of the hydro substation represents a potential environmental concern to soil and groundwater quality at the site. Other listings for the area were predominantly residential listings and some commercial listings (flower shops, restaurants, convenience stores, etc.). The residential and commercial listings are not considered to represent a significant potential environmental concern with respect to the environmental quality of soil and groundwater at the subject site. HEIRS Documents: Terrapex requested Fire Insurance Plans (FIPs), Inspection Reports, and Site Specific Plans and Reports through HEIRS. One FIP from 1955 was found for the site and surrounding areas and is discussed below. HEIRS documentation is presented in Appendix III FIP The subject site appears to be developed with a residential dwelling. The surrounding area to the west, east and south are occupied by residential dwellings. To the north of the site is generally vacant; however an area northeast of the site, at the base of an escarpment, is identified as the Town of Weston Waterworks and illustrated as occupied by a pump house and artesian well. Additional residential dwellings and a "public school" are illustrated north of the waterworks and vacant area DATABASE SEARCHES EcoLog ERIS Search: Ecolog ERIS (Environmental Risk Information Services Ltd.) provides environmental and historical information using various sources of government and private records. ERIS searched the site and surrounding properties within an approximate radius of 250 m from the site. A copy of the EcoLog ERIS report is included in Appendix IV. The EcoLog ERIS report identified a single data base entry for the site: a record pertaining to waste generator registration under Ontario Regulation 347 (R.R.O. 1990, Reg. 347, General - Waste Management). However, no further details were provided by EcoLog other than the registration was for the year The following pertinent information related to neighbouring properties within 250 m was summarized from the EcoLog ERIS report. 1286/1288 Wilson Avenue (adjacent to the east): The site has an approved Certificate of Approval (CofA) for an emergency diesel generator. The certificate was approved in TERRAPEX ENVIRONMENTAL LTD. City of Toronto CT

12 It is typical for such infrastructure to be powered by diesel fuel stored in either an AST or UST. The potential presence of a diesel UST or AST represents a potential environmental concern relating to the environmental quality of soil or groundwater at the site. Borehole: Eighteen boreholes were listed to be drilled within 250 m of the site for geotechnical/geological purposes between 1959 and The depths of the boreholes ranged from 1.5 m to 11.1 m below grade. No particular concerns are attached to these records. Ontario Spills: A total of five spill records were identified by EcoLog. The closest incident occurred adjacent to the site on the roadway of Wilson Avenue and Ridge Road. Approximately 12 L of coolant was spilt on the roadway. Other spills pertain to sewage overflow, a natural gas leak, 3 L of paint thinner to a catch basin and a reported fire in a catch basin. Due to the nature, quantity of spilled substances and/or intervening distance the reported spills are not considered a significant concern with respect to the environmental quality of soil or groundwater at the site. TSSA Incidents: Five incident records were identified by EcoLog. One listing pertained to a carbon monoxide release, one involved a natural gas pipeline hit, and one involved a natural gas leak at a private dwelling. The remaining two records reportedly pertain to the "discovery of product" at 1303 and 1307 Wilson Avenue (apartment buildings located south of the site, across Wilson Avenue). No details pertaining to the type of incident or fuel type involved were provided for the two remaining records, and so the significance of these records is unclear. However, it should also be noted that the Wilson Avenue road allowance is quite wide, and that the 1303 and 1307 Wilson Avenue properties are located more than 20 m south of the site. Waste Generator: In total, there were five waste generators were mapped within the 250 m search radius. The data base entries which were mapped closest to the site are described below: 1305 and 1309 Wilson Avenue (Biddington Property Management) are located approximately 100 m southwest of the site, registered wastes generator of heavy fuels; 1265 Wilson Avenue (Flagship Property Ventures Corp) located approximately 200 m east of the site; registered wastes generated includes light fuels; 1270 Wilson Avenue (Pierre Laport Jr High School) located approximately 200 m west of the site; registered waste generated includes PCBs, inorganic laboratory chemicals and organic laboratory chemicals; and, 1263 Wilson Avenue (Local 183 of L.I.U.N.A, the Laborers International Union of North America) located approximately 200 m east of the site; registered wastes TERRAPEX ENVIRONMENTAL LTD. 13 generated includes misc. wastes and inorganic chemicals, photoprocessing wastes and pathological wastes. Due to the intervening distances and nature of the likely waste generating activities, no particular concerns are attached to these records. National PCB Inventory: Three listings were identified and mapped within the 250 m search radius. Pierre Laporte Junior High School is listed as having PCBs in use on the site. Dates on the records are from Due to the intervening distance, no particular concerns are attached to the records. Water Well Information System: Four entries from this Provincial database were mapped within the 250 m search radius. The wells were drilled between 1938 and 2009 to depths ranging from 4.2 m to 29.1 m below grade. Two wells were completed as observation/monitoring wells and two were listed as unused. No particular concerns are attached to these records. Inventory of Coal Gasification Plant Waste Sites in Ontario : A review of the inventory did not identify any former coal gasification plant waste sites located within 1 km of the site. Industrial Sites Producing or Using Coal Tar and Related Tars Inventory : A review of the inventory did not identify any former industrial sites producing or using coal tar or related tars located within 1 km of the site. Waste Disposal Site Inventory : A review of the inventory did not identify any closed or active waste disposal sites located within 1 km of the site. Brownfields Environmental Site Registry : A review of the MOE registry (available on the MOE website) did not identify any Records of Site Condition (RSCs) pertaining to properties located within 250 m of the site GOVERNMENT AND REGULATORY DOCUMENTATION Ministry of Environment (MOE): Terrapex submitted a Freedom of Information request to the MOE FOI Office requesting information in the Ministry s files with respect to environmental concerns, infractions, complaints or orders related to the site. A response was received dated August 7, Documentation received included the registration for a Hazardous Waste Registration Number (HWIN), however no waste classes were registered. Other file documentation pertained to lead water testing within the school. An exceedance was reported in the water prior to flushing the water lines (pre-water flush test) and was reported as non-detect after the water lines were flushed (post water flush test). No recommended action was required by the MOE as regular water use would correct the issue. It should be noted that testing was conducted during the summer when the school is not TERRAPEX ENVIRONMENTAL LTD. City of Toronto CT

14 operational and that the report could not confirm the source of the lead (on site or off site pipes, lead solder used on copper pipes ect.). A copy of the MOE correspondence is included in Appendix V. The City of Toronto: Requests were sent to various departments of the City of Toronto for documented environmental concerns, including infractions, complaints, notifications or control orders. Maps from the City of Toronto were reviewed on-line for environmentally site sensitive information, land use and zoning. Terrapex received a response from Toronto Public Health on July 23, In their response, Toronto Public Health indicated that there were no records pertaining to the subject site on file. A response was received by Toronto Fire Services on August 7, No records were found pertaining to the site. Terrapex received a response from Toronto Water dated July 3, Records pertained to services call and did not indicate the presence of potential environmental concerns. According to the City of Toronto Official Plan, the subject site is not located within or adjacent to lands that are designated as Environmentally Significant Areas. The site is not designated as parkland, however is adjacent to parkland. The Official Plan does indicate that the site is part of a Natural Area. City of Toronto Zoning By-Law indicates that site is currently under the Former General Zoning By-Law 7625 (North York). A copy of the City of Toronto correspondence is included in Appendix V. The Technical Standards & Safety Authority (TSSA) was contacted regarding records relating to registered underground fuel tanks, incident reports, fuel oil spills or contamination records on the site. The TSSA responded that they did not identify any records pertaining to the site. A copy of the TSSA correspondence is included in Appendix V PREVIOUS REPORTS AND OTHER DOCUMENTATION Two reports were provided to Terrapex by the City of Toronto: Appraisal of Real Property, 1300 Wilson Avenue, Toronto, Ontario. 15 Conseil Scolaire de District Catholique Centre-Sud of Toronto by Maple Environmental Inc. (Maple) October The assessment indicated that the site is zone R5 for residential and is subject to the Ravine and Natural Feature Protection By-Law. The Official Plan indicates that the site is considered a Natural Area. The site is approximately 3.03 acres in size and the school was reportedly built in A reassessment of hazardous materials was conducted in 2012 at the subject site. An initial DSS was conducted in 2007, although a report documenting that work program was not provided to Terrapex. As part of the reassessment, several building material samples, including vinyl floor tiles and plaster were taken for analysis of asbestos. None of the tested building materials contained asbestos. However, it should be recognized that the previous assessment reportedly identified asbestos containing material at the site. This included vinyl floor tiles and plaster in several locations within the school, which are identified in the report. One paint sample was analysed for lead and the paint sample was considered not to be lead-containing. Mercury was identified to be present as vapour within fluorescent light tubes and liquid mercury is present within thermostats in various locations. Minor mould was observed beneath various sinks within the building. Polychlorinated Biphenyls (PCBs) were suspected to be present within fluorescent lamp ballasts. Devices suspected to be radon sources were not identified. Free crystalline silica in the form of common construction sand (a constituent of concrete and masonry products present at the site) was also identified. 3.2 SITE INSPECTION SITE DESCRIPTION General Site Layout: The site is located on the north side of Wilson Avenue approximately 200 m east of Lexfield Avenue, in Toronto, Ontario. The site is roughly rectangular and is approximately 1.23 ha (3.03 acres) in size. The site is developed with one two-storey building, which is currently occupied by St. Noel Chabanel elementary school, which is located in the southern portion of the site. The northern portion of the site is occupied by grass and tree areas, asphalt walking paths, and an asphalt paved area. A play area, dedicated for kindergarten students, is located along the western side of the school. The site is entirely fenced, with retaining walls along portions of the eastern and northern property boundary. The site slopes towards the centre of the site, where a portion of a former stream has been channelized below the site. TERRAPEX ENVIRONMENTAL LTD. City of Toronto CT

16 A hydro transformer is located along the eastern side of the building. Catch basins are located in the asphalt paved areas north and south of the building. A concrete shed and garbage bins are located in the southwest corner of the site. Vehicular access to the site is obtained from Wilson Avenue. cul-du-sac is located north of the site on Calais Avenue. However, a drop-off A general site layout is shown on Figure 2. Building Description: The site is developed with a one two-storey building, which is currently occupied by St. Noel Chabanel elementary school, which is located in the southern portion of the site. The first floor is occupied by classrooms, utility rooms, office area, library, gymnasium with a stage, washrooms and change rooms. A partial second floor is occupied by four classrooms, washrooms and storage rooms. Interior finishes in the school were generally tile floors, painted cinder block walls and ceiling tiles. The building is serviced with municipal water and sewer, as well as hydro, telephone and fibre optic utility services. The building is heated and cooled by electrical baseboards and individual heating and cooling units. A small concrete shed is located in the southwest corner of the site and is used for the storage of gardening and maintenance equipment. A general site layout is shown on Figure 2. Selected site photographs are provided in Appendix XI REGULATED MATERIALS AND DESIGNATED SUBSTANCES A previous DSS was conducted in 2007 and a reassessment in 2012 (see Section 3.1.6). Asbestos-Containing Material (ACM): Any material which contains greater than 0.5% asbestos fibre (by dry weight) is considered to be an asbestos-containing material (ACM) with respect to the requirements of the Designated Substance - Asbestos on Construction Projects and in Buildings and Repair Operations (O. Reg. 278/05). O. Reg. 278/05 defines Afriable material@ as material that when dry can be crumbled, pulverized or powdered by hand pressure, or is crumbled, pulverized or powdered. Disposal of asbestos waste in Ontario is governed by the General - Waste Management regulation (R.R.O. 1990, Reg. 347). TERRAPEX ENVIRONMENTAL LTD. City of Toronto CT

17 Building materials in structures constructed prior to 1980 may contain friable ACM. As noted in section a previous assessment has been conducted at the site. ACM was noted in vinyl floor tiles and plaster in several locations within the school, which are identified in the report. According to O.Reg. 278/05, it is the owner=s responsibility to furnish all prospective contractors who may encounter ACM with information pertaining to asbestos present at the site including any reports or drawings. Lead: Historically, the Federal Hazardous Products Act had limited the amount of lead permissible in new interior paint to 0.5%, or 5,000 parts per million (ppm). While this limit did not apply to paints already applied, for some time it comprised the generally accepted level over which a paint was considered to be "lead-based". However, the current Surface Coating Materials Regulation, a section within the Federal Hazardous Products Act, limits the permissible lead content in many surface coating materials, including interior paints, to 0.06%, or 600 ppm. There are exceptions to this limit, and the limit does not apply to paints already applied. However, the 0.06% limit is now the generally accepted level over which a paint is considered to be Alead-based@. Lead may be present in piping, solder joints of piping, and paint used on walls, pipes and other structures. Lead may also be associated with products such as wall shielding (x-ray rooms, etc.) and may be present in soil or bedrock from natural or anthropogenic sources. One paint sample was analysed in the 2012 reassessment for lead and the paint sample was considered not to be lead-containing. Furthermore, water testing for lead was conducted at the school in 2007 (see Section 3.1.5). Based upon the age of the site buildings, lead may be present on other painted surfaces, and in pipe solder. Care should be taken to avoid worker and public exposure to lead dust through the use of appropriate personal protective equipment and proper dust control measures during any potential renovations or demolition of the building. A Designated Substance Survey would be required to quantify the extent and magnitude of lead-containing paint and other building materials present at the site. Mercury: Mercury is a naturally occurring element that can occur in several forms. Metallic mercury is a shiny, silver-white coloured, odourless liquid. When heated, mercury is a colourless, odourless gas. Mercury may also combine with other elements to form inorganic mercury compounds (Asalts@) which typically resemble white powders or crystals, while some microscopic organisms are known to produce organic mercury compounds (e.g., methylmercury). Metallic mercury is used in thermometers, dental amalgams, batteries, fluorescent lamps, high intensity discharge (HID) lamps and related products, and is also used in the production of chlorine gas and caustic soda. Mercury salts are sometimes used in antiseptic creams and ointments. TERRAPEX ENVIRONMENTAL LTD. City of Toronto CT

18 Mercury may also be present in surface coatings such as paint. The current Surface Coating Materials Regulation limits the permissible mercury content in many surface coating materials, including interior paints, to 0.001%, or 10 ppm. While this limit does not apply to paints already applied, it is generally accepted as the level over which a paint is considered Amercury-containing@. Small volumes of mercury are likely present in thermostat controllers in the buildings, as well as the vapours within fluorescent light bulbs. Care should be taken to avoid worker and public exposure to mercury during any potential renovations and demolition by removing and appropriately disposing of thermostat controls and fluorescent light bulbs prior to removal of building roofs, walls, supports, etc. by heavy equipment. A Designated Substance Survey would be required to quantify the extent and magnitude of mercury-containing paint and other building materials present at the site. Silica: Silica, also called Asilica sand@ or Aquartz sand@, refers to sands, gravels, and other soil and rock products with a high silicon dioxide (SiO 2 ) content. ADesignated substance@ requirements under the Occupational Health and Safety Act only apply to crystalline silica present in a respirable form. Because silica will be naturally present in soil and bedrock as well as many construction materials including cement, concrete, brick, and mortars, appropriate personal protective equipment and dust control measures should be employed during any potential renovations or demolition to mitigate exposure of workers and the public to respirable silica. Other Designated Substances: Other designated substances in addition to asbestos, lead and mercury include acrylonitrile, arsenic, benzene, coke oven emissions, ethylene oxide, isocyanates, silica, and vinyl chloride. None of these substances was observed on the site during our site inspection. PCBs: Historically, PCBs have been used in electrical equipment such as transformers, fluorescent light ballasts and capacitors. The use of PCBs was banned in heat transfer and electrical equipment installed after 1977, and in transformers and capacitors installed after These bans did not initially apply to existing equipment, however, recent changes to the Federal PCB regulations (SOR/ ) have specified deadlines for the removal from service of PCB-containing equipment: $ equipment containing PCBs at a concentration equal to or greater than 500 mg/kg must be removed from service by December 31, 2009; $ equipment containing PCBs at a concentration less than 500 mg/kg must be removed from service by December 31, 2025; and, TERRAPEX ENVIRONMENTAL LTD. 19 $ equipment containing PCBs at a concentration less than 500 mg/kg and equal to or greater than 500 mg/kg, and which is located at, or within, 100 m of a drinking water treatment plant or food or feed processing area, a child care facility, preschool, primary school, secondary school, hospital or senior citizens= care facility, must be removed from service by December 31, 2009 (this deadline does not apply to light ballasts or pole-mounted transformers or their associated auxiliary equipment). In addition, Federal PCB regulations require that PCBs or products containing PCBs (at concentrations equal to or greater than 50 mg/kg) that were in storage as of September 5, 2008, be removed for destruction by December 31, Polychlorinated Biphenyls (PCBs) are suspected to be present within fluorescent lamp ballasts as reported in the 2012 Reassessment Report (see section 3.1.6). A transformer was observed along the eastern side of the school. However, the transformer was surrounded by a fence and could not be inspected. The transformer is owned by Toronto Hydro. During any potential renovations or demolition activities, it should be verified whether PCBs are present. Any PCB-containing equipment must be disposed in accordance with the requirements of the Waste Management - PCBs regulation (R.R.O. 1990, Reg. 362). Urea Formaldehyde Foam Insulation (UFFI): Urea Formaldehyde Foam Insulation (UFFI) is insulating foam plastic typically, but not exclusively, used to insulate wood-framed residential homes. Most installations occurred between 1977 and 1980, after which it was banned in Canada. UFFI is produced by mixing urea formaldehyde resin, a foaming agent, and compressed air, and injecting into void spaces. No foam insulation was observed, or evidence of the placement of UFFI identifiable, at the site during the site inspection. Based on the historical land use and currently site observations UFFI is unlikely to be present at the site. Ozone-Depleting Substances (ODS): ODS include chlorofluorocarbons (CFCs), halons, carbon tetrachloride, and methyl chloroform. Most ODS in industrial/commercial settings are found in refrigeration equipment (including air conditioning units) and in older halon fire suppression systems for areas containing computers or other sensitive electronics. ODS-containing equipment was observed at the site during the site inspection. These included fridges and heating and cooling units. The presence of potential ODS in the building is not considered a significant environmental concern, however disposal and/or servicing of ODS-containing devices must be completed by a licensed technician to ensure that these substances are managed in accordance with Ontario s General Waste Management regulation (R.R.O 1990, Reg. 347) and the Refrigerants regulation (O. 20 Other Hazardous Materials: Hazardous materials that were observed on site included minor quantities of cleaning solvents, varsol (mineral spirits), paint, disinfectant, cleaning agents etc., Mould Mould is a general term for microscopic fungi that are highly adapted to grow and reproduce rapidly, producing spores and mycelia. Mould may grow indoors when provided with moisture and nutrients. Under wet or damp conditions, mould may grow on building materials such as wallpaper, ceiling tiles, carpets, insulation material and drywall. Minor mould was observed beneath various sinks within the building in the 2012 reassessment (see Section 3.1.6). Appropriate personal protective equipment and mould dust control measures should be employed during any renovation or demolition work, to mitigate exposure of workers and the public to mould spores. Wastes: General domestic wastes (paper, refuse ect.) are generated at the site from employees and students are stored in large collection bins located in the southwest corner of the site near the hazardous waste storage building. Wastes are regularly pick-up by a licensed waste hauler STORAGE TANKS No aboveground storage tanks, or evidence of existing or former underground storage tanks or aboveground storage tanks (such as vent or fill pipes) were observed at the site during the site inspection ADJACENT PROPERTIES The site is located in a mixed predominantly residential neighbourhood. Occupants of adjoining and neighbouring properties observed at the time of the site inspection are listed below: North: Northeast: East: Southeast: South: Southwest: West: Northwest: Calais Avenue and residential; Roding Park; Apartment building and Pierre Laporte Middle School beyond; Wilson Avenue, apartment building beyond; Wilson Avenue and apartments/convenience store beyond; Wilson Avenue and apartments beyond; Hydro substation and residential beyond; and, Residential. 21 Numerous groundwater monitoring wells were observed on the hydro substation property during the site inspection. The number of installations was considered to be greater than what would normally be expected to be present as a result of a "due diligence" investigation, suggesting that significant effort was made in characterizing groundwater at the adjoining property. While such characterization may not have been made for environmental site assessment purposes, the presence of the hydro substation represents a potential environmental concern. No potential environmental concerns were identified with respect to the remaining neighbouring properties. 3.3 INTERVIEWS Ms. Janice Green, City of Toronto, Facilities Management Department: The following information was provided by Ms. Green prior to the site inspection: $ The site is currently a school owned by the Conseil Scolaire de District Catholique Centre-Sud; $ EMS requires a Phase I and Phase II ESA for due diligence purposes; and, $ Two reports pertaining to the site were available to the City (refer to Section 3.1.6); Ms. Green provided copies of these reports to Terrapex. Mr. Richard Nsengimana, Conseil Scolaire de District Catholique Centre-Sud, Planning Agent: The following information was provided by Mr. Nsengimana during the site inspection and subsequent follow-up s: $ Mr. Nsengimana has been with the school board for approximately 5 years; $ The site was built in 1975; $ The site has not undergone any major renovations since it was built; $ The site is serviced with water, sewer, and hydro; $ The building is heated via electric baseboard units and heating/cooling units are located in each classroom; and, $ Designated Substance Surveys (DSS) have been previously conducted at the school. 22 4.0 SUMMARY AND CONCLUSIONS The site is located on the north side of Wilson Avenue approximately 200 m east of Lexfield Avenue, in Toronto, Ontario. The site is roughly rectangular in shape, is approximately 1.23 ha (3.03 acres) in size. The site is developed with one a two-storey building, which is currently occupied by St. Noel Chabanel elementary school. The site is owned and operated by Conseil Scolaire de District Catholique Centre-Sud (South-Central District Catholic School Board). Vehicular access to the site is obtained from Wilson Avenue. The surface at the site is asphalt paved with a grassy area to the north of the building. According to historical documents, the site was first developed prior to 1947 as a residential property. In 1975, construction of the current building was completed and began operating as École Saint-Noel-Chabanel, a French Catholic School. A tributary of Black Creek formerly transected the central portion of the site in an east/west direction. During the late 1970s the stream was channelized into a culvert that extends beneath the property, and which is currently still present at site. Surrounding land uses have historically been predominantly residential with some institutional and commercial land use. No on-site environmental concern with respect to potential soil and groundwater quality at the site was identified at the subject site. A previous report confirmed the presence of asbestos at the subject site in vinyl floor tiles and plaster in several locations in the school. The potential presence of other designated substances such as lead, mercury and PCBs were noted in a previous work program. Other hazards, such as mould located below sink area were also noted. Several off-site environmental concerns with respect to potential soil and groundwater qualities at the site were identified at adjacent properties: an electrical sub-station is located on the property immediately west of the site; numerous groundwater monitoring wells were also observed at this property; Technical Standards and Safety Authority Records relating to the "discovery of product" at 1303 and 1307 Wilson Avenue (apartment buildings located approximately 20 m south of the site, across Wilson Avenue) were identified, although no other details regarding these instances was provided; and, the Apartment building located on the property immediately east of the site has been granted a Certificate of Approval for the operation of a backup generator, indicating a potential for an above ground or underground fuel storage tank to be present on the property. 23 Based on the records review, interviews and site inspection, the Phase I ESA did identify sources of potential sources of environmental concern with respect to soil and groundwater quality at the site. The database was created when surveys submitted to the Ministry were converted in the Assessment File Research Image Database (AFRI) project. 87 Environmental Registry: 1994-Jun 2013 Provincial EBR The Environmental Registry lists proposals, decisions and exceptions regarding policies, Acts, instruments, or regulations that could significantly affect the environment. Through the Registry, thirteen provincial ministries notify the public of upcoming proposals and invite their comments. For example, if a local business is requesting a permit, license, or certificate of approval to release substances into the air or water; these are notified on the registry. Data includes: Approval for discharge into the natural environment other than water (i.e. Air) - EPA s. 9, Approval for sewage works - OWRA s. 53(1), and EPA s Approval for a waste disposal site. Environmental Compliance Approval: Oct 31, 2011-Jun 2013 Provincial ECA On October 31, 2011, a smarter, faster environmental approvals system came into effect in Ontario. In the past, a business had to apply for multiple approvals (known as certificates of approval) for individual processes and pieces of equipment. Today, a business either registers itself, or applies for a single approval, depending on the types of activities it conducts. Ontario Regulation 347 Waste Generators Summary: 1986-Apr 2012 Provincial GEN Regulation 347 of the Ontario EPA defines a waste generation site as any site, equipment and/or operation involved in the production, collection, handling and/or storage of regulated wastes. A generator of regulated waste is required to register the waste generation site and each waste produced, collected, handled, or stored at the site. This database will include spills and leaks from diesel, fuel oil, gasoline, natural gas, propane and hydrogen recorded by the TSSA. Landfill Inventory Management Ontario: 2010 Provincial LIMO The Landfill Inventory Management Ontario (LIMO) database is updated every year, as the ministry compiles new and updated information. The inventory will include small and large landfills. Additionally, each year the ministry will request operators of the larger landfills complete a landfill data collection form that will be used to update LIMO and will include the following information from the previous operating year. This will include additional information such as estimated amount of total waste received, landfill capacity, estimated total remaining landfill capacity, fill rates, engineering designs, reporting and monitoring details, size of location, service area, approved waste types, leachate of site treatment, contaminant attenuation zone and more. The small landfills will include information such as site owner, site location and certificate of approval # and status. Canadian Mine Locations: Private MINE This information is collected from the Canadian & American Mines Handbook. The Mines database is a national database that provides over 290 listings on mines (listed as public companies) dealing primarily with precious metals and hard rocks. Listed are mines that are currently in operation, closed, suspended, or are still being developed (advanced projects). Their locations are provided as geographic coordinates (x, y and/or longitude, latitude). As of 2002, data pertaining to Canadian smelters and refineries has been appended to this database. Mineral Occurrences: 1846-Apr 2013 Provincial MNR In the early 70's, the Ministry of Northern Development and Mines created an inventory of approximately 19,000 mineral occurrences in Ontario, in regard to metallic and industrial minerals, as well as some information on building stones and aggregate deposits. Please note that the "Horizontal Positional Accuracy" is approximately /- 200 m. Many reference elements for each record were derived from field sketches using pace or chain/tape measurements against claim posts or topographic features in the area. The primary limiting factor for the level of positional accuracy is the scale of the source material. The testing of horizontal accuracy of the source materials was accomplished by comparing the planimetric (X and Y) coordinates of that point with the coordinates of the same point as defined from a source of higher accuracy. Our inventory provides information on the facility name, location, spill ID #, spill date, type of spill, as well as the quantity of substance spilled & recovered. National Defence & Canadian Forces Waste Disposal Sites: 2001-Apr 2007* Federal NDWD The Department of National Defence and the Canadian Forces maintains an inventory of waste disposal sites located on DND lands. Where available, our inventory provides information on the base name, location, type of waste received, area of site, depth of site, year site opened/closed and status. National Environmental Emergencies System * Federal NEES (NEES): In 2000, the Emergencies program implemented NEES, a reporting system for spills of hazardous substances. For the most part, this system only captured data from the Atlantic Provinces, some from Quebec and Ontario and a portion from British Columbia. Data for Alberta, Saskatchewan, Manitoba and the Territories was not captured. However, NEES is also a repository for all previous Environment Canada spill datasets. NEES is composed of the historic datasets ' or Trends ' which dates from approximately 1974 to present. NEES Trends is a compilation of historic databases, which were merged and includes data from NATES (National Analysis of Trends in Emergencies System), ARTS (Atlantic Regional Trends System), and NEES. In 2001, the Emergencies Program determined that variations in reporting regimes and requirements between federal and provincial agencies made national spill reporting and trend analysis difficult to achieve. The OGSR Library has over 20,000 wells in their database. Information available for all wells in the ERIS database include well owner/operator, location, permit issue date, well cap date, licence no., status, depth and the primary target (rock unit) of the well being drilled. All geology/stratigraphy table information, plus all water table information is also provide for each well record. Inventory of PCB Storage Sites: 1987-Oct 2004 Provincial OPCB The Ontario Ministry of Environment, Waste Management Branch, maintains an inventory of PCB storage sites within the province. Ontario Regulation 11/82 (Waste Management - PCB) and Regulation 347 (Generator Waste Management) under the Ontario EPA requires the registration of inactive PCB storage equipment and/or disposal sites of PCB waste with the Ontario Ministry of Environment. TSSA Pipeline Incidents: June 2009-Mar 2012 Provincial PINC TSSA's Fuels Safety Program administers the Technical Standards & Safety Act 2000, providing fuel-related safety services associated with the safe transportation, storage, handling and use of fuels such as gasoline, diesel, propane, natural gas and hydrogen. Under this Act, TSSA regulates fuel suppliers, storage facilities, transport trucks, pipelines, contractors and equipment or appliances that use fuels. This database will include spills, strike and leaks from recorded by the TSSA. Private and Retail Fuel Storage Tanks: * Provincial PRT The Fuels Safety Branch of the Ontario Ministry of Consumer and Commercial Relations maintained a database of all registered private fuel storage tanks and licensed retail fuel outlets. This database includes an inventory of locations that have gasoline, oil, waste oil, natural gas and/or propane storage tanks on their property. The MCCR no longer collects this information. This information is now collected by the Technical Standards and Safety Authority (TSSA). 92 Ontario Regulation 347 Waste Receivers Summary: Provincial REC Part V of the Ontario Environmental Protection Act ("EPA") regulates the disposal of regulated waste through an operating waste management system or a waste disposal site operated or used pursuant to the terms and conditions of a Certificate of Approval or a Provisional Certificate of Approval. Regulation 347 of the Ontario EPA defines a waste receiving site as any site or facility to which waste is transferred by a waste carrier. A receiver of regulated waste is required to register the waste receiving facility. This database represents registered receivers of regulated wastes, identified by registration number, company name and address, and includes receivers of waste such as: landfills, incinerators, transfer stations, PCB storage sites, sludge farms and water pollution control plants. This information is a summary of all years from 1986 including the most currently available data. Record of Site Condition: 1997-Sept 2001, Oct Provincial RSC Jun 2013 The Record of Site Condition (RSC) is part of the Ministry of the Environment's Brownfields Environmental Site Registry. Protection from environmental cleanup orders for property owners is contingent upon documentation known as a record of site condition (RSC) being filed in the Environmental Site Registry. In order to file an RSC, the property must have been properly assessed and shown to meet the soil, sediment and groundwater standards appropriate for the use (such as residential) proposed to take place on the property. The Record of Site Condition Regulation (O. Reg. 153/04) details requirements related to site assessment and clean up. RSCs filed after July 1, 2011 will also be included as part of the new (O.Reg. 511/09). Retail Fuel Storage Tanks: 1999-Jun 2010 Private RST This database includes an inventory of retail fuel outlet locations (including marinas) that have on their property gasoline, oil, waste oil, natural gas and / or propane storage tanks. Scott's Manufacturing Directory: 1992-Mar 2011 Private SCT Scott's Directories is a data bank containing information on over 200,000 manufacturers across Canada. Even though Scott's listings are voluntary, it is the most comprehensive database of Canadian manufacturers available. Information concerning a company's address, plant size, and main products are included in this database. Ontario Spills: 1988-Aug 2012 Provincial SPL This database identifies information such as location (approximate), type and quantity of contaminant, date of spill, environmental impact, cause, nature of impact, etc. Information from was part of the ORIS (Occurrence Reporting Information System). The SAC (Spills Action Centre) handles all spills reported in Ontario. Regulations for spills in Ontario are part of the MOE's Environmental Protection Act, Part X. Wastewater Discharger Registration Database: Provincial SRDS Information under this heading is combination of the following 2 programs. The Municipal/Industrial Strategy for Abatement (MISA) division of the Ontario Ministry of Environment maintained a database of all direct dischargers of toxic pollutants within nine sectors including: Electric Power Generation; Mining; Petroleum Refining; Organic Chemicals; Inorganic Chemicals; Pulp & Paper; Metal Casting; Iron & Steel; and Quarries. All sampling information is now collected and stored within the Sample Result Data Store (SRDS). Anderson's Storage Tanks: * Private TANK The information provided in this database was collected by examining various historical documents, which identified the location of former storage tanks, containing substances such as fuel, water, gas, oil, and other various types of miscellaneous products. Information is available in regard to business operating at tank site, tank location, permit year, permit & installation type, no. of tanks installed & configuration and tank capacity. Data contained within this database pertains only to the city of Toronto and is not warranted to be complete, exhaustive or authoritative. The information was collected for research purposes only. Transport Canada Fuel Storage Tanks: 1970-Mar 2007 Federal TCFT With the provinces of BC, MB, NB, NF, ON, PE, and QC; Transport Canada currently owns and operates 90 fuel storage tanks. Waste Disposal Sites - MOE 1991 Historical Approval Up to Oct 1990* Provincial WDSH Inventory: In June 1991, the Ontario Ministry of Environment, Waste Management Branch, published the "June 1991 Waste Disposal Site Inventory", of all known active and closed waste disposal sites as of October 30st, For each "active" site as of October 31st 1990, information is provided on site location, site/ca number, waste type, site status and site classification.
